La lutte contre les accidents iatrogènes générés par les AVK, notamment les accidents hémorragiques qui viennent au premier rang, constitue un véritable problème de santé publique en France. Le médecin généraliste se retrouve être la pierre angulaire d'un bon suivi d'un patient sous AVK. Un audit clinique de façon rétrospective sur 5 mois a été entrepris sous forme d'autoévaluation pour décrire l'état des lieux actuel des prescriptions des AVK en médecine générale dans le département de la Loire. 26 médecins généralistes et 205 de leurs patients ont été inclus. Les médecins ont presque tous un dossier médical informatisé bien tenu avec l'adresse du patient, le téléphone, l'étiologie de la prescription des AVK et une dernière INR datant de moins de deux mois. Plus des deux tiers des patients connaissent leur valeur cible d'INR, mais la moitié seulement disposent d'un carnet de suivi et connaissent les risques de complications iatrogènes (signes de surdosages, médicaments contre indiqués et situations à risque) avec un caractère péjoratif de l'âge. La traçabilité de l'INR sur les ordonnances thérapeutiques et biologiques reste le point noir de cette étude avec moins de 15% de réponses positives. Malgré des recommandations répétées de la HAS et des rappels annuels de bon usage des AVK par l'AFSSAPS, la formation médicale continue doit poursuivre dans cette direction jusqu'à l'application rigoureuse par les médecins des référentiels.

Book excerpt: Notre travail était d'évaluer la pratique et l'implication des médecins généralistes en Isère, Savoie et Haute-Savoie, en matière de gestion de traitement par antivitamine K : respect des indications, des durées de traitement, et des règles de surveillance. 2000 questionnaires ont été envoyés à 200 médecins. Chaque questionnaire portait sur un patient traité par antivitamine K et était rempli par le médecin. 52 médecins généralistes ont répondu, 389 patients ont été inclus. La moyenne d'âge était de 73ans, 36% avaient un carnet de suivi. L'indication était inappropriée dans 10% des cas, la durée de traitement était dépassée dans 12% des cas. Le généraliste était le prescripteur initial dans 3% des cas, le traitement était justifié selon lui dans 96% des cas et il aurait pu l'arrêter sans avis spécialisé dans 5% des cas. L'INR était dosé 2 à 3 jours après une modification de posologie une fois sur 5 et une fois sur 2 en cas d'introduction d'un nouveau médicament. L'INR cible théorique était erroné dans 16% des cas. Les pratiques doivent encore être améliorées, et le médecin plus impliqué.

Book excerpt: Introduction: Les anti-vitamine K (AVK) ont de nombreuses indications chez le sujet âgé, principalement la prévention des embolies cruoriques d’origine cardiaque et la maladie thrombo-embolique veineuse. Méthode : Nous avons mené une enquête prospective observationnelle d’un an sur la population des patients âgés de plus de 65 ans et hospitalisés dans le service de médecine interne de l'Hôtel Dieu à Paris. L'objectif principal était d'évaluer le nombre de traitements par AVK adaptés aux recommandations des sociétés savantes françaises ; Nous avons également recherché les facteurs limitant leur prescription dans cette population. Résultats : 1040 patients ont été hospitalisés durant cette période, dont 602 âgés de plus de 65 ans. Parmi eux, 131 patients (21%) avaient une indication pour les AVK, leur moyenne d'âge était de 84 ans. La moitié (62/131, 47%) n'étaient pas traités par AVK alors qu'il y avait une indication. Les facteurs de non prescription sont : le grand âge, un antécédent de chute, le MMS anormal dès 25, la consommation d’alcool. Les antécédents hémorragiques ne constituent pas la non prescription complète des AVK. La seule pathologie pour laquelle les recommandations ne sont pas suivies est la fibrillation auriculaire. Conclusion : Près de la moitié des patients de plus de 65 ans de notre étude, qui justifient d’une indication réelle aux AVK, n’en bénéficient pas. La prescription est limitée par des facteurs liés au malade, peu par son mode de vie. Il est souhaitable de réaliser des études prospectives randomisées dans cette population afin d’évaluer précisément le rapport bénéfice/risque des AVK. Ces études devront évaluer la qualité de vie autant que la durée de la survie sans accident thrombo-embolique et /ou hémorragique.

In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.
