Book Public Financing and Delivery of HIV AIDS Care

Download or read book Public Financing and Delivery of HIV AIDS Care written by Institute of Medicine and published by National Academies Press. This book was released on 2005-11-04 with total page 375 pages. Available in PDF, EPUB and Kindle. Book excerpt: Each year it is estimated that approximately 40,000 people in the U.S. are newly infected with HIV. In the late 1990s, the number of deaths from AIDS dropped 43% as a result of highly active antiretroviral therapy. Unfortunately, the complex system currently in place for financing and delivering publicly financed HIV care undermines the significant advances that have been made in the development of new technologies to treat it. Many HIV patients experience delays in access to other services that would support adhering to treatment. As a result, each year opportunities are missed that could reduce the mortality, morbidity, and disability suffered by individuals with HIV infections. Public Financing and Delivery of HIV/AIDS Care examines the current standard of care for HIV patients and assesses the extent the system currently used for financing and delivering care allows individuals with HIV to actually receive it. The book recommends an expanded federal program for the treatment of individuals with HIV, administered at the state level. This program would provide timely access and consistent benefits with a strong focus on comprehensive and continuous care and access to antiretroviral therapy. It could help improve the quality of life of HIV/AIDS patients, as well as reduce the number of deaths among those infected.

This book was released on 2017-09-13 with total page 28 pages. Available in PDF, EPUB and Kindle. Book excerpt: The CARE Act, a federal effort to address the HIV/AIDS epidemic, is administered by HHS. The Act uses formulas based upon a grantee's number of AIDS cases to distribute funds to eligible metropolitan areas (EMA), states, and territories. The use of AIDS cases was prescribed because most jurisdictions tracked and reported only AIDS cases when the grant programs were established. HIV cases must be incorporated with AIDS cases in CARE Act formulas no later than fiscal year 2007. GAO was asked to discuss factors that affect the distribution of CARE Act funding. This testimony is based on HIV/AIDS: Changes Needed to Improve the Distribution of Ryan White CARE Act and Housing Funds, GAO-06-332 (Feb. 28, 2006). GAO discusses how specific funding-formula provisions contribute to funding differences among CARE Act grantees and what distribution differences could result from using HIV cases in CARE Act funding formulas.

Available in PDF, EPUB and Kindle. Book excerpt: The Ryan White Comprehensive AIDS Resources Emergency (CARE) Act gives funding to cities, states, and other public and private entities to provide care and support services to individuals with HIV and AIDS who have low-incomes and little or no insurance. The CARE Act is a discretionary program that relies on annual appropriations from Congress to provide care for low-income, uninsured, or underinsured individuals who have no other resources to pay for care. Despite its successes, funding has been insufficient to address all of the inequalities and gaps in coverage for people with HIV. In response to a congressional mandate, an Institute of Medicine committee was formed to reevaluate whether CARE allocation strategies are an equitable and efficient way of distributing resources to jurisdictions with the greatest needs and to assess whether quality of care can be refined and expanded. Measuring What Matters: Allocation, Planning, and Quality Assessment for the Ryan White CARE Act proposes several types of analyses that could be used to guide the evaluation and improvement of allocation formulas, as well as a framework for assessing quality of care provided to HIV-infected persons.

This book was released on 2010-11 with total page 35 pages. Available in PDF, EPUB and Kindle. Book excerpt: Under the Ryan White CARE Act, funds are made available to assist over 530,000 individuals affected by HIV/AIDS. Grantees directly provide services to individuals or arrange with service providers to do so. The Health Resources and Services Admin. (HRSA), which administers CARE Act programs, is required to cancel balances of grants that are unobligated after one year and redistribute amounts to grantees in need. Under the CARE Act, states and territories receive grants for AIDS Drug Assistance Programs (ADAP), which provide HIV/AIDS drugs. This report reviews: (1) HRSA's implementation of the unobligated balance provisions; (2) HRSA's actions to collect client-level data; and (3) the status of ADAP waiting lists. Charts and tables.

Book The Social Impact of AIDS in the United States

Download or read book The Social Impact of AIDS in the United States written by National Research Council and published by National Academies Press. This book was released on 1993-02-01 with total page 337 pages. Available in PDF, EPUB and Kindle. Book excerpt: Europe's "Black Death" contributed to the rise of nation states, mercantile economies, and even the Reformation. Will the AIDS epidemic have similar dramatic effects on the social and political landscape of the twenty-first century? This readable volume looks at the impact of AIDS since its emergence and suggests its effects in the next decade, when a million or more Americans will likely die of the disease. The Social Impact of AIDS in the United States addresses some of the most sensitive and controversial issues in the public debate over AIDS. This landmark book explores how AIDS has affected fundamental policies and practices in our major institutions, examining: How America's major religious organizations have dealt with sometimes conflicting values: the imperative of care for the sick versus traditional views of homosexuality and drug use. Hotly debated public health measures, such as HIV antibody testing and screening, tracing of sexual contacts, and quarantine. The potential risk of HIV infection to and from health care workers. How AIDS activists have brought about major change in the way new drugs are brought to the marketplace. The impact of AIDS on community-based organizations, from volunteers caring for individuals to the highly political ACT-UP organization. Coping with HIV infection in prisons. Two case studies shed light on HIV and the family relationship. One reports on some efforts to gain legal recognition for nonmarital relationships, and the other examines foster care programs for newborns with the HIV virus. A case study of New York City details how selected institutions interact to give what may be a picture of AIDS in the future. This clear and comprehensive presentation will be of interest to anyone concerned about AIDS and its impact on the country: health professionals, sociologists, psychologists, advocates for at-risk populations, and interested individuals.

Book Structural Barriers to Continuity of HIV Care

Download or read book Structural Barriers to Continuity of HIV Care written by Steven J. Erly and published by . This book was released on 2021 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: The Ryan White HIV/AIDS Drug Assistance programs (ADAPs) are the largest source of medical care for people living with HIV (PLWH) in the United States. They pay for insurance and medical care for 20% of people living with HIV in the United States, and people on the program have high rates of viral suppression, the central measure of successful HIV treatment. In spite of the benefits of the program, clients have difficulty staying enrolled in the program and subsequently lose benefits. Until recently, federal policy required that ADAP clients provide documentation of their eligibility every 6 months or be removed from the program. In October of 2021, these restrictions were relaxed, and states were given the authority to set their own recertification procedure. Although this policy change has the potential to reduce the burden of recertification, there is a lack of information about how recertification affects clients and the cost of the program. This dissertation sought to fill this knowledge gap by quantifying the effect of disenrollment on the ADAP clients and building a model to project the effect of extending the recertification timeline to every 12-months. First, we used Ryan White data from Washington State to describe the prevalence of ADAP disenrollment and identify factors associated with being removed from the program. We categorized all PLWH enrolled in ADAP clients by the success of their recertification applications as continuously enrolled, ruled ineligible, disenrolled if they failed to recertify. We compared individuals who were disenrolled to those who were continuously enrolled by demographic and socioeconomic characteristics and use of case management services. Next, we sought to quantify the impact of disenrollment from ADAP on viral suppression. Using this same population, we estimated the risk difference of viral suppression before and after enrollment using clients who were continuously enrolled as a comparator. We used quantitative bias analysis to identify how much of the effect of client disenrollment could be attributed to other unmeasured confounders. Lastly, we used the results of the first two aims to develop a Markov model to analyze the cost and health impact of changing the existing 6-month recertification schedule to a 12-month schedule. We predicted the change in annual program costs, program enrollment, and population viral suppression over a 5-year time horizon. We found that disenrollment is common and disproportionately affects marginalized populations in Washington State. Over the two-year study, 26% of clients were disenrolled from the program at least once due to failure to recertify, which is much greater than the 18% of clients who were removed due to ineligibility. Compared to those who were continuously enrolled, disenrolled PLWH were more likely to be Black (prevalence ratio vs White 1.31, 95% CI 1.17-1.46), uninsured (PR vs private insurance 1.24, 95% CI 1.10-1.40), and younger (PR 25-34 vs 35-44 years 1.23 95% CI 1.08-1.41). We also found that disenrollment negatively impacts the viral suppression of PLWH who are removed from the program. Of the 1336 ADAP clients who were disenrolled, 83% were virally suppressed before disenrollment versus 69% after (RD 12%, 95%CI 9-15%). Our quantitative bias analysis suggested that unmeasured confounders are unlikely to explain the entirety of this effect. Our budget impact analysis suggested that a 12-month recertification policy would yield a program that costs 7% more per year ($40.2M vs $37.7M, 95% CI 6-8%), but produces greater health benefits (245 more individuals virally suppressed by the end of 2025). The results of this dissertation demonstrate that the current ADAP recertification policies, which were formerly required by federal policy, are disruptive to the health of a large proportion of ADAP clients. In Washington state, a change to a 12-month recertification policy has the potential to reduce the number of virally unsuppressed PLWH by 10% at a modest cost relative to the overall cost of the program.
