Book excerpt: With nearly half of the world's population living in a rural or remote area, meeting the health needs of rural populations, where over 80% of the world's extremely poor live, is imperative in achieving universal health coverage. Leaving no one behind means ensuring that health workers are available in rural and remote areas. Health, social and economic inequities remain cross-cutting challenges for rural populations. Rural populations tend to be poorer, have worse health outcomes, and experience higher rates of unemployment, underemployment and informal employment. It is estimated that about 51-67% of rural populations are without adequate access to essential health services , translating to about 2 billion people being left behind. In some countries, rural populations have access to numbers of health workers that are 10 times less than the numbers available to urban populations. The deficiency in numbers and mix of trained motivated health workers to provide the needed health services is a critical health system issue. This inequitable access to health workers and health services impacts health outcomes and increases socioeconomic disadvantages. Higher under-5, maternal and preventable mortality rates, increased morbidity, decreased life expectancy, and more costs to access distant care are seen across rural areas.

Book excerpt: Since 2004, the U.S. government has supported the global response to HIV/AIDS through the President's Emergency Plan for AIDS Relief (PEPFAR). The Republic of Rwanda, a PEPFAR partner country since the initiative began, has made gains in its HIV response, including increased access to and coverage of antiretroviral therapy and decreased HIV prevalence. However, a persistent shortage in human resources for health (HRH) affects the health of people living with HIV and the entire Rwandan population. Recognizing HRH capabilities as a foundational challenge for the health system and the response to HIV, the Government of Rwanda worked with PEPFAR and other partners to develop a program to strengthen institutional capacity in health professional education and thereby increase the production of high-quality health workers. The Program was fully managed by the Government of Rwanda and was designed to run from 2011 through 2019. PEPFAR initiated funding in 2012. In 2015, PEPFAR adopted a new strategy focused on high-burden geographic areas and key populations, resulting in a reconfiguration of its HIV portfolio in Rwanda and a decision to cease funding the Program, which was determined no longer core to its programming strategy. The last disbursement for the Program from PEPFAR was in 2017. Evaluation of PEPFAR's Contribution (2012-2017) to Rwanda's Human Resources for Health Program describes PEPFAR-supported HRH activities in Rwanda in relation to programmatic priorities, outputs, and outcomes and examines, to the extent feasible, the impact on HRH and HIV-related outcomes. The HRH Program more than tripled the country's physician specialist workforce and produced major increases in the numbers and qualifications of nurses and midwives. Partnerships between U.S. institutions and the University of Rwanda introduced new programs, upgraded curricula, and improved the quality of teaching and training for health professionals. Growing the number, skills, and competencies of health workers contributed to direct and indirect improvements in the quality of HIV care. Based on the successes and challenges of the HRH program, the report recommends that future investments in health professional education be designed within a more comprehensive approach to human resources for health and institutional capacity building, which would strengthen the health system to meet both HIV-specific and more general health needs. The recommendations offer an aspirational framework to reimagine how partnerships are formed, how investments are made, and how the effects of those investments are documented.

It is crucial that policymakers attempting to redress these issues have accurate and timely information so that workforce planning and policy-making is well-informed. In particular, it is critical that policy interventions effectively optimise the turnover and retention of the existing rural and remote primary health care workforce, as these are frequently a scarce and valuable resource. The aim of this research, therefore, is to understand the patterns, determinants and metrics of rural and remote Australian primary health care workforce turnover and retention, with a view to developing appropriate indicators and benchmarks to support rural and remote health service workforce retention and inform rural and remote health workforce policy-making.The research of this thesis takes a quantitative approach to investigate rural health workforce turnover and retention. Firstly, five metrics, well suited for measuring turnover and retention in rural and remote Australian contexts, are identified. These include simple metrics, such as turnover rates and retention rates, as well as metrics requiring more advanced analytical capabilities, such as survival probabilities and proportional hazards ratios. These metrics, particularly those calculated using survival analysis, are applied to five different Australian rural and remote primary health care workforce datasets to explore how rural primary health care workforce retention differs according to profession, geographical location, population size and a range of other financial and economic, professional and organisational, educational and regulatory, and personal and family factors. The empirical findings are then used to derive tentative benchmarks for length of stay of primary health care professionals that differ according to profession and geographic location.This research reveals substantial and significant differences in rural and remote Australian primary health care workforce retention according to profession and geographical location and population size. Doctors and allied health professionals have approximately 1.80 times the risk of leaving a rural or remote health service at any point in time compared with nurses and Aboriginal health workers. Substantial differences in retention are evident within the allied health professions. It is not clear what impact these changes will have on medical and public health preparedness programs around the country. Although there has been tremendous progress since 2005 and Hurricane Katrina, there is still a long way to go to ensure the health security of the Country. There is a commonly held notion that preparedness is separate and distinct from everyday operations, and that it only affects emergency departments. But time and time again, catastrophic events challenge the entire health care system, from acute care and emergency medical services down to the public health and community clinic level, and the lack of preparedness of one part of the system places preventable stress on other components. The implementation of the ACA provides the opportunity to consider how to incorporate preparedness into all aspects of the health care system. The Impacts of the Affordable Care Act on Preparedness Resources and Programs is the summary of a workshop convened by the Institute of Medicine's Forum on Medical and Public Health Preparedness for Catastrophic Events in November 2013 to discuss how changes to the health system as a result of the ACA might impact medical and public health preparedness programs across the nation.
