Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. It accompanies two other Observatory publications Organization and financing of public health services in Europe and The role of public health organizations in addressing public health problems in Europe: the case of obesity alcohol and antimicrobial resistance (both forthcoming).

This white paper outlines the Government's proposals to protect the population from serious health threats; help people live longer, healthier and more fulfilling lives; and improve the health of the poorest. It aims to empower individuals to make healthy choices and give communities and local government the freedom, responsibility and funding to innovate and develop ways of improving public health in their area. The paper responds to Sir Michael Marmot's strategic review of health inequalities in England post 2010 - "Fair society, healthy lives" (available at http://www.marmotreview.org/AssetLibrary/pdfs/Reports/FairSocietyHealthyLives.pdf) and adopts its life course framework for tackling the wider social determinants of health. A new dedicated public health service - Public Health England - will be created to ensure excellence, expertise and responsiveness, particularly on health protection where a national response is vital. The paper gives a timetable showing how the proposals will be implemented and an annex sets out a vision of the role of the Director of Public Health. Book excerpt: In its final report the Independent Commission on Banking (ICB) recommended a package of measures, consisting of ring-fencing vital banking services and increasing banks' loss-absorbency. The Government strongly supports the ICB's objectives and dual approach. The Government agrees that vital banking services - in particular, the taking of retail deposits - should only be provided by 'ring-fenced' banks', and that these banks should be prohibited from undertaking certain investment banking activities. On increased loss-absorbency, also supported are the ICB recommendations for higher equity requirements for large ring-fenced banks, a minimum leverage ratio, loss-absorbing debt, insured depositor preference and higher levels of loss-absorbing capacity for banks that are difficult to resolve. With regards to the principle that systemically important banks hold a minimum about of loss-absorbing capacity on a group-wide basis, however, the requirement should not apply to non-UK operations where it can be shown that those operations to do not pose a risk to UK financial stability. The Government also believes that depositor preference needs further analysis and consultation. On competition, the Government also strongly supports all the ICB recommendations. The Government estimates the aggregate private costs to UK banks at £3.5bn - £8bn, producing a gross reduction in GDP of £0.8bn - £1.8bn. Against these costs though should be set the potentially much larger benefits with the ICB's recommendations yielding an estimated incremental economic benefit of £9.5bn per annum. Significantly too the Government wants to see relevant legislation completed by the end of this Parliament in May 2015 as opposed to the ICBs recommended 2019

Book excerpt: The government proposes to give local authorities more freedom and powers to meet the needs of their citizens and communities. Local authorities will be encouraged to develop neighbourhood charters setting out local standards and priorities; to manage services at the level of the neighbourhood; to work more closely with neighbourhood policing teams. Local people will receive more information about services and standards, and will be able to question and get a response from local councilors through a new service, Community Call for Action. Executive power will be invested in the leader of the council, and there will be three choices of leadership model: a directly elected mayor, a directly elected executive of councillors, or a leader elected by fellow councillors with a four year mandate. New training opportunities will be provided for councillors. The making of byelaws will be fully devolved to local authorities. The authorities will be encouraged to bring together local partners to help improve services, and to develop a delivery plan - the Local Area Agreement - setting out a single set of priorities for local partners, for the Sustainable Community Strategy that they are already required to prepare. The performance framework for local government will be simplified: there will be about 35 priorities for each area, with a set of some 200 outcome based indicators replacing the many hundreds of indicators currently required by central government. Ambitious efficiency gains will be required as part of the 2007 comprehensive spending review. The second volume shows how these proposals will apply to major local public service areas and cross-cutting issues: community safety; health and well-being; vulnerable people; children and families; climate change; and economic development, housing and planning.

Climate change can have catastrophic impacts in numerous cities across the world. It is time for us to react – quickly and effectively. The European Community (EC) has been leading the fight against climate change, making it one of its top priorities. We have introduced the most ambitious targets of their kind, known as the “20/20/20 by 2020” initiative within the “Climate Action and Renewable Energy Package.” As a result, European Member States have taken on a commitment to curb their CO emissions by at least 20% by 2020. 2 These targets are indeed commendable; however, they are only the start if we are to avoid the consequences of global warming. Whilst top level coordination from the European Institutions and Member State governments is vital, the role of mitigating and adapting to climate change at local level must not be forgotten. In fact, here cities, regions and their citizens play a significant a role. It is therefore vital they become directly involved in the climate change challenge. The European Commission therefore launched in 2008 a new initiative, the Covenant of Mayors, which brings together a network of European mayors in a voluntary effort to go beyond the European Union’s already ambitious targets. Half of our greenhouse gas emissions (GHGs) are created in and by cities.
