The work is the output of the disease reference group on zoonoses and marginalized infectious diseases (DRG6), which is part of an independent think tank of international experts, established and funded by the Special Programme for Research and Training in Tropical Diseases (TDR), to identify key research priorities through the review of research evidence and input from stakeholder consultations. The report covers a diverse range of diseases including zoonotic helminth protozoa, viral and bacterial infections considered to be neglected and associated with poverty. Disease-specific research issues were elaborated under individual disease sections and many common priorities were readily identified among the disease such as need for new and/or improved drugs and regimens, diagnostics and, where appropriate, vaccines. The disease specific priorities are described as micro priorities compared with the macro level priorities which will drive such policies as the need for improved surveillance; the need for inter-sectoral interaction between health, livestock, agriculture, natural resources and wildlife in tackling the zoonotic diseases; and the need for a true assessment of the burden of the zoonoses. This is one of ten disease and thematic reference group reports that have come out of the TDR Think Tank, all of which have contributed to the development of the Global Report for Research on Infectious Diseases of Poverty.

This report presents an evaluation of current research and challenges in controlling the helminthiases of public health importance, including onchocerciasis, lymphatic filariasis, soil-transmitted helminthiases, schistosomiasis, food-borne trematodiases and taeniasis/cysticercosis. The evaluation covers five major themes -- intervention, epidemiology and surveillance, environmental and social ecology, data and modelling, and fundamental biology. Despite the recent demonstrated successes and expansion of tools for the helminthiases outlined here, and the development of some research capacity, the evaluation found major deficiencies in our current control tools, in diagnostics, and in our fundamental knowledge of helminth biology and transmission dynamics, as well as in capacity and policy for health research. Thus the current research issues are summarized here, and opportunities for improving disease control and reducing poverty are identified. Recommendations are presented to inform public health policy, guide implementation programmes, and focus the research community on the needs of disease control and the opportunities for bettering human welfare. Preventive chemotherapy in this context means using drugs that are effective against a broad range of worm infections to simultaneously treat the four most common diseases caused by worms: river blindness (onchocerciasis), elephantiasis (lymphatic filariasis), schistosomiasis, and soil-transmitted helminthiasis. Significant opportunities also exist to integrate these efforts with the prevention and control of diseases such as trachoma. The new approach provides a critical first step in combining treatment regimens for diseases which, although different in themselves, require common resources and delivery strategies for control or elimination.

This report reviews the connections between environmental change, modern agricultural practices and the occurrence of infectious diseases--especially those of poverty--and proposes a methodology that can be used to prioritize research on such diseases. Although there is some comprehension of the underlying and growing systemic influence of today s large-scale social and environmental changes on some infectious diseases, the significance and potential future impacts of these changes are poorly understood. Nevertheless, such changes now constitute a significant influence on the working of the Earth's systems that will have increasing consequences for patterns of occurrence of infectious diseases. Many of these changes are illustrated in this report. A common theme of this report is bidirectional causation, effectively "trapping" complex, linked eco-social systems in stable states that are resistant to intervention. For example, poverty is associated with ill health, low education and often with poor diets, either because of under-nutrition (and diarrhoea) or intakes that have excessive calories but insufficient micronutrients. In either case, poverty impairs health; and ill health impairs the escape from poverty. Another example is provided by a recent abundant agricultural harvest in India that has far exceeded storage capacity. A substantial fraction of this harvest will be wasted due to inadequate storage. Some grain that is badly stored will be contaminated by aflatoxins and other fungi, which increases the risk of cancer. This report presents the case for a more integrated approach across sectors, research disciplines and diseases, taking greater account of the increasingly widespread and systemic influences on disease emergence and spread.

Book excerpt: "The presence, or absence, of neglected tropical diseases (NTDs) can be seen as a proxy for poverty and for the success of interventions aimed at reducing poverty. Today, coverage of the public-health interventions recommended by the World Health Organization (WHO) against NTDs may be interpreted as a proxy for universal health coverage and shared prosperity - in short, a proxy for coverage against neglect. As the world's focus shifts from development to sustainable development, from poverty eradication to shared prosperity, and from disease-specific goals to universal health coverage, control of NTDs will assume an important role towards the target of achieving universal health coverage, including individual financial risk protection. Success in overcoming NTDs is a "litmus test" for universal health coverage against NTDs in endemic countries. The first WHO report on NTDs (2010) set the scene by presenting the evidence for how these interventions had produced results. The second report (2013) assessed the progress made in deploying them and detailed the obstacles to their implementation. This third report analyses for the first time the investments needed to achieve the scale up of implementation required to achieve the targets of the WHO Roadmap on NTDs and universal coverage against NTDs. INVESTING TO OVERCOME THE GLOBAL IMPACT OF NEGLECTED TROPICAL DISEASES presents an investment strategy for NTDs and analyses the specific investment case for prevention, control, elimination and eradication of 12 of the 17 NTDs. Such an analysis is justified following the adoption by the Sixty-sixth World Health Assembly in 2013 of resolution WHA6612 on neglected tropical diseases, which called for sufficient and predictable funding to achieve the Roadmap's targets and sustain control efforts. The report cautions, however, that it is wise investment and not investment alone that will yield success. The report registers progress and challenges and signals those that lie ahead. Climate change is expected to increase the spread of several vector-borne NTDs, notably dengue, transmission of which is directly influenced by temperature, rainfall, relative humidity and climate variability primarily through their effects on the vector. Investments in vector-borne diseases will avoid the potentially catastrophic expenditures associated with their control. The presence of NTDs will thereby signal an early warning system for climate-sensitive diseases. The ultimate goal is to deliver enhanced and equitable interventions to the most marginalized populations in the context of a changing public-health and investment landscape to ensure that all peoples affected by NTDs have an opportunity to lead healthier and wealthier lives."--Publisher's description.

This notion implies different evolutionary strategies of host exploitation. Typically, parasites establish long-term infections and induce relatively little mortality, as they often limit pathological changes by modulating host cells and downregulating adverse immune responses. Their pattern of distribution tends to be endemic rather than epidemic. Despite these seemingly benign traits, parasites usually cause substantial chronic morbidity, thus constituting an enormous socioeconomic burden in humans, particularly in resource poor countries, and in livestock worldwide. Parasite-induced fitness costs are an evolutionary force that can shape populations and contribute to species diversity. Therefore, a thorough understanding of parasites and parasitic diseases requires detailed knowledge of the respective biochemical, molecular and immunological aspects as well as of population genetics, epidemiology and ecology. This Research Topic (RT) bridges disciplines to connect molecular, immunological and wildlife aspects of parasitic infections. The RT puts emphases on four groups of parasites: Plasmodium, Toxoplasma, Giardia and intestinal helminths. Co-infections are also covered by the RT as they represent the most common form of parasite infections in wildlife and domestic animal populations. Within the four types of parasites the following topics are addressed: (1) Experimental models: hypothesis testing, translation and limits. (2) Critical appraisal of experimental models. (3) Natural systems: Technological advances for investigations in natural parasite-host systems and studies in natural systems. (4) The urgent need for better models and methods in natural parasite systems. The aim is to help the microscopist ascertain the presence of parasites in feces, whether they be minute protozoan cysts or large helminth eggs, and to identify them correctly. With this goal in mind, the bench aids include pertinent laboratory instructions as well as high-quality images. The photomicrographs illustrate diagnostic features of each of the parasites as they appear in different preparations and at different magnifications. Each photomicrograph is produced with a measuring bar and accompanied by a short explanatory legend, which draws attention to distinctive features that help confirm diagnosis. Helminth eggs are illustrated in the first 36 photomicrographs, which show the diagnostic stages of the most common helminthes, including nematodes, cestodes, schistosomes, and other trematodes.
