Book excerpt: But : le but de cette étude est d'essayer de comprendre l'absence de prise en charge de la schizophrénie en orthophonie en se demandant si les représentations des futur.e.s orthophonistes dépendent des mêmes critères que ceux retrouvés dans la littérature auprès d'une population de professionnels de santé.Matériel et méthode : un questionnaire en ligne composé en majorité d'échelles de Likert en 7 points a été diffusé au 21 CFUO de France pour interroger les représentations et analyser statistiquement la dépendance de certaines variables.Résultats : les représentations des futur.e.s orthophonistes au sujet de la schizophrénie ne semblent pas entièrement dépendre des mêmes critères que ceux retrouvés dans la littérature auprès des professionnels de santé.Conclusion : ces résultats sont donc mitigés et ne permettent pas de statuer de manière certaine sur la raison de l'absence de prise en soin des patient.e.s avec un diagnostic de schizophrénie. Une étude, utilisant ce même questionnaire, pourrait approfondir les réflexions sur ce sujet.

Ces symptômes persistent généralement malgré la prise d'un traitement adapté et ils constituent un handicap important à l'insertion sociale et professionnelle des patients, tout en contribuant à réduire leur qualité de vie. L'orthophonie offre des possibilités quant à l'évaluation spécifique de ces troubles et leur accompagnement. Or, l'intérêt et le rôle que peuvent jouer les orthophonistes dans le parcours de soin des patients avec schizophrénie semblent méconnus dans les structures de psychiatrie adulte. Cette étude a pour but l'élaboration d'un livret d'information à destination des équipes soignantes des structures de psychiatrie adulte, pour les informer à ce sujet. Un questionnaire est élaboré et envoyé à cette population de professionnels afin de mesurer l'efficacité du livret par l'évaluation de leurs connaissances avant et après lecture de l'outil. Les résultats montrent une différence significative des deux mesures, mettant en avant une évolution positive de l'apprentissage concernant la pertinence et les moyens de l'orthophonie dans ces prises en soin suite à la lecture du livret. Cet outil d'information semble efficace dans les objectifs qu'il s'est fixés.

Elle présente une symptomatologie très riche, avec notamment des troubles du langage. Il s'agit principalement de troubles affectant les niveaux pragmatique et extralinguistique du langage. Ces troubles ont un retentissement important sur ·la communication interpersonnelle des patients et sur leur insertion socio-professionnelle. Par ailleurs, des études ont noté des similitudes entre les troubles du langage présentés par les sujets schizophrènes et ceux présentés par les sujets cérébrolésés droits. La prise en charge de ces troubles chez les cérébrolésés droits est du ressort de l'orthophonie. Or, la prise en charge orthophonique dans les services de psychiatrie adulte est à ce jour quasi inexistante. A travers cette étude, nous avons cherché à démontrer que l'orthophoniste peut jouer un rôle dans la prise en charge pluridisciplinaire des patients schizophrènes, notamment grâce à des techniques utilisées en rééducation neurologique. Nous avons mis en place une prise en charge ortbophonique d'une durée de quatre mois auprès de six patients schizophrènes suivis en hôpital de jour. Nous avons évalué ces patients avant et après notre intervention afin de comparer leurs performances. Notre intervention a été axée sur deux domaines : le langage implicite (langage métaphorique, actes de langage indirects et inférences) et la prosodie émotionnelle, sur les versants expressif que réceptif. Cette prise en charge a été très bien accueillie par la totalité des patients et nous avons pu constater une nette amélioration de leurs performances après notre intervention. Ce constat nous amène alors à réenvisager la place de l'orthophonie auprès de patients atteints de schizophrénie.

In 1994, the diagnosis of transsexualism was replaced with ”gender identity disorder" by the DSM-IV committee. Transsexualism is associated with distress because the individual, due to his/her physical characteristics, is not treated as a representative of the gender he/she identifies with and cannot therefore live in a gender role with which he/she identifies. The sexual anatomy of the body also feels alien and inappropriate. Transsexualism that causes distress is treated with sex reassignment therapy. The aim is to alter body characteristics and anatomy closer to those of the target gender as well as to facilitate the integration into the social role of the opposite sex. Investigations and surgery are often only available in specialist centres.
