On July 20, 1986 a crew member disappeared after jumping from a life float into the river whilst on a pleasure trip from the anchored vessel to a local beach. Immediately rescue efforts by the four remaining persons on board the life float were unsuccessful. The crew member was not wearing a lifejacket but was reported to be a good swimmer. Despite and extensive search of the immediate area, the body was not found until five days later.

While preparing other holds, which were empty, for the next cargo, a seaman inadvertently opened the access hatch to No.4 hold and , on noting what appeared to be water in the hold with the concentrates, reported this to the bosun. After informing the bridge as to what had been reported, the bosun went down into the hold to investigate but within a short time, he collapsed,falling onto the cargo.This report deals with the investigation into the death of the bosun.

Book excerpt: This report examines the catastrophic vessel failure and fire that occurred on March 4, 1998, near Pitkin, Louisiana, at an oil and gas production facility owned by Sonat Exploration Company. Four workers were killed in the incident. The root causes of the incident are identified, and recommendations are made concerning engineering and design management systems and the development of good-practice guidelines. The Chemical Safety and Hazard Investigation Board (CSB) is an independent federal agency whose mission is to ensure the safety of workers and the public by preventing or minimizing the effects of chemical incidents. The CSB is a scientific investigative organization; it is not an enforcement or regulatory body. Established by the Clean Air Act Amendments of 1990, the CSB is responsible for determining the root and contributing causes of accidents, issuing safety recommendations, studying chemical safety issues, and evaluating the effectiveness of other government agencies involved with chemical safety. No part of the conclusions, findings, or recommendations of the CSB relating to any chemical incident may be admitted as evidence or used in any action or suit for damages arising out of any matter mentioned in an investigation report. See 42 U.S.C. § 7412(r)(6)(G). The CSB makes public its actions and decisions through investigation reports, summary reports, safety studies, safety recommendations, special technical publications, and statistical reviews. Book excerpt: During the morning of December 6, 1989 , the Bahamian vessel "Adriane E" was loading a cargo of packaged lumber at Eurocan Terminal Wharf, Kitimat, B.C. using the vessel's deck cranes. While ranging and positioning timber deck cargo lashing chains using a deck crane, the swinging motion of the crane hook's counterweight caused a chain to momentarily tighten, causing a shiploader who had been standing near the edge of the cargo, and whose foot was on the chain, to lose his balance and fall to the dock below. (Shiploader is the term used locally to describe the occupation of persons who would more commonly be designated stevedores or longshoremen.The term 'counterweight' is used in this report to describe the combined weight and swivel and the end of the crane's hoisting wire and to which the hook is attached. This report deals with the investigation of the above accident.
