Au sein de la population, il existe en effet des disparités importantes qui peuvent expliquer et conditionner ce phénomène. Et ce, malgré le système de santé en France réputé pour prendre en charge largement les soins de ses habitants. Cette thèse s'intéresse particulièrement aux maladies rares bucco-dentaires, qui, nombreuses et variées, présentent des spécificités propres rendant complexe leur prise en charge et influencent le recours aux soins des patients atteints. Les maladies rares, et plus spécifiquement les maladies orales et dentaires, sont décrites, le système de santé français ainsi que les dispositifs spécifiques de prise en charge de ces maladies. Notre étude avait pour but d'évaluer le renoncement aux soins dentaires chez des patients porteurs de maladies rares bucco-dentaires, âgés de 18 ans et plus et suivis au Centre de Référence des maladies rares orales et dentaires du pôle de Médecine et Chirurgie Buccales de Strasbourg. Le premier groupe de patients est porteur d'anomalies dentaires de structure et le second est constitué de patients atteints d'anomalies dentaires de nombre et bénéficiant d'une ALD hors liste. L'étude est basée sur un questionnaire comportant des items spécifiques, relatifs aux facteurs susceptibles d'influencer le renoncement aux soins tels que l'obstacle financier, la peur, le type de couverture maladie, l'éloignement géographique, les pathologies bucco-dentaires, le type de soins nécessaires. Puis le traitement et l'analyse des données ouvre une réflexion et une discussion sur les principaux facteurs impliqués dans le renoncement aux soins et permet également d'évaluer si les dispositifs de prise en charge spécifiques de certaines maladies rares sont adaptés.

Ces patients ont un risque plus élevé d'avoir un état bucco-dentaire altéré. En effet, ils peuvent souffrir de déficiences physiques, sensorielles, intellectuelles ou psychiques, d'anomalies dentaires et de pathologies organiques associées ce qui peut conduire à des difficultés sur le plan alimentaire, de l'hygiène bucco-dentaire ou encore de la prise en charge des soins. Il faut donc aider ces patients à gérer au mieux leur vie avec la maladie pour limiter les complications. Les recommandations habituelles données aux patients n'étant pas toujours adaptées à ces situations particulières, l'éducation thérapeutique du patient (ETP) atteint de maladie rare a toute sa place en odontologie. Elle permet au patient de devenir autonome face à l'affection qui le touche, par la transmission de savoir et de savoir-faire. L'approche doit rester individuelle et tenir compte de toutes les particularités de chacun. Appliquer l'ETP en odontologie, et spécifiquement aux patients atteints de maladies rares, permettrait d'apporter un certain confort au quotidien, et contribuerait à améliorer leur état de santé général. L'objectif de ce travail est de définir le champ de l'ETP en odontologie et de réaliser un état des lieux des programmes d'ETP existant actuellement et comprenant un volet bucco-dentaire.

Brown applies this theory of transformational processes to a variety of topics, including the analyst’s receptivity, countertransference as transformation, the analytic setting, the paintings of J.M.W. Turner, "autistic transformations" and other clinical situations in the analysis of children and adults. The Analyst's Reveries finds increasing veneration for the analyst’s use of their reveries, while revealing important differences amongst post-Bionians in how reverie is defined and used clinically. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
