While African religion was relatively marginal in inspiring or contributing to AIDS activism during the early days of the epidemic, this situation has changed dramatically. In order to account for these changes, contributors provide answers to pressing questions. How does the entrance of religion into public debates about AIDS affect policymaking and implementation, church-state relations, and religion itself? How do religious actors draw on and reconfigure forms of transnational connectivity? How do resource flows from development and humanitarian aid that religious actors may access then affect relationships of power and authority in African societies? How does religious mobilization on AIDS reflect contestation over identity, cultural membership, theology, political participation, and citizenship? Addressing these questions, the authors draw on social movement theories to explore the role of religious identities, action frames, political opportunity structures, and resource mobilization in African religions’ reaction to the AIDS epidemic. The book’s findings are rooted in fieldwork conducted in Tanzania, Uganda, Zambia, Zimbabwe, Ghana, and Mozambique, among a variety of religious organizations. This book draws on research studies ranging in context from sites in sub-Saharan Africa to New York City in the USA to examine the complexity of responding to the epidemic both globally and locally. Religious systems of meaning, practices and institutions have been central to the articulation of projects for social change and inversely sometime strongly resistant to change in diverse institutional responses to HIV and AIDS. Sometimes, religious movements provided powerful forces for community mobilisation in response to the social vulnerability, economic exclusion and health problems associated with HIV. In other contexts, religious cultures have reproduced values and practices that have seriously impeded more effective approaches to mitigate the epidemic. By highlighting these complex and sometimes contradictory social processes, this book provides new insights about the potential for religious institutions to address the HIV epidemic more effectively. More broadly, it shows how research can be done on religion in the area of global public health, showing how civil society organizations shape opportunities for health promotion: a crucial and new area of global public health research. Book excerpt: The story of an innovative program of treatment for AIDS in Africa that succeeded in the face of international development agencies’ “afro pessimism” Until this century, Western governments and foundations framing policies for AIDS relief in Africa maintained that prevention alone was a preferable alternative to prevention-plus-treatment, which would be costly and impractical in Africa, or would benefit only the prosperous and well-connected. Sant'Egidio’s Dream argues that this initial, failed approach to AIDS in African countries reflects a global moral blindness to the imperative to save lives–which was not lost on the Community of Sant’Egidio, an Italian, Catholic social movement rooted in “the gospel and friendship” and present in 70 countries. Drawing on two decades of peacemaking and humanitarian experience in Africa, the movement grasped the evidence that HIV, if treated, does not lead to AIDS and to death–and saw that treatment gives hope in Africa just as it does in the developed world. By enabling large numbers of people to live with a chronic disease, and involving family and neighbors in free and effective care, it offers a dream of a society surviving and even thriving in spite of HIV. In 2002, Sant Egidio established the DREAM (Drug Resource Enhancement Against AIDS and Malnutrition) project, a community-based approach to the AIDS crisis, rooted in medicine, epidemiology, and public health, that has proven effective in ten countries where it has been implemented–and has emerged as a model for healthcare in the global South.
