Book excerpt: Au cours de l'année 2000, 12 720 000 passages aux urgences ont été enregistrés en France 16 % ont été adressés via leur médecin traitant soit, environ, 2 035 200 passages d'où l'importance de l’interaction entre médecins traitant et médecins urgentistes. Le but de cette étude est d'évaluer la relation entre médecins généralistes (MG) et médecins urgentistes (MU) afin de rechercher des moyens d'améliorer celle ci pour faciliter leurs exercices et ainsi faciliter l'accès aux soins hospitaliers aux patients si possible sans passages obligatoires par les SAU. Il s'agit d'une étude prospective du 1er décembre 2013 au 15 février 2014. L'étude a permis d'obtenir les réponses de 210 MU et les réponses de 527 MG. Les 2 professionnels avouent ne pas se connaître et avoir des problèmes de compréhension. Les MG avouent avoir de grandes difficultés pour accéder au plateau technique de l'hôpital. Ils sont aussi d'accord pour dire que certains passages aux urgences pourraient être mais faute d'autre solution nécessaire. Les 2 professions ont noté leur relation de 0 à 10 ils la jugent tous deux moyenne (entre 5 et 7). Pour eux ce qui altère leur relation est principalement le manque de connaissance et de compréhension mutuelle associé à un service d'urgences souvent surchargé. Les deux professions s'accordent pour dire que mieux se connaître améliorerait leurs relations ils sont d'ailleurs en grande majorité prêt à se rencontrer. Tout deux pensent que l'utilisation d'un interlocuteur au sein de l'hôpital pour les demandes des MG serait nécessaire et pour tous deux cette personne devrait être un médecin. Il reste à convaincre les autres praticiens car cette personne pourrait les privées de certains lits. L'étude a montré que leurs relations étaient correctes mais qu'il existe des solution pour l'améliorer. L'étude n'a pas montré de grandes tensions entre ces deux professions ce qui est rassurant car ils sont déjà en difficulté dans leur exercice quotidien

Book excerpt: Introduction: la prise en charge des patients au SAU n'est pas une chose aisée du fait que l'histoire médicale des patients n'est pas connue par les médecins urgentistes. Une bonne communication entre médecin traitant et médecin urgentiste est nécessaire pour l'optimisation de la prise en charge du patient. Objectif: évaluer les relations entre médecin traitant et médecin urgentiste afin de déterminer les paramètres à améliorer pour le bien du patient. Matériels et Méthodes: Deux études ont été menées. Un questionnaire a été envoyé aux médecins généralistes du bassin montpelliérain concernant leur rapport avec les urgences (étude1) et une évaluation du degré d'information des courriers d'admission des patients aux urgences adultes du CHU de Lapeyronie à Montpellier a été réalisée (étude2). Une analyse descriptive a été faite. Résultats: Pour l'étude 1, 160 médecins traitants sur 464 ont répondu (34.5%). les relations avec les urgences sont jugées comme satisfaisantes. Il n'est cependant pas facile de contacter un médecin des urgences. La qualité des lettres accompagnants les patients est souvent influencée par la situation d'urgence. Plus de la moitié des médecins traitants interrogés (59%) sont prêts à suivre une formation concernant la gestion des situations de crise. Pour l'étude 2, 493 patients inclus dont 63 adressés par les médecins traitants (12.8%) et 171 adressés après une régulation du centre 15 (34.7%). 90.5% des patients adressés par les médecins traitants avaient un courrier avec un degré moyen d'information du courrier de 7,31/10. 56.7% des patients adressés par le centre 15 avaient un courrier avec un degré moyen d'information du courrier de 6.64/10. Conclusion: Ces études montrent des résultats plutôt satisfaisant dans le bassin montpelliérain comparativement à d'autres études. Il existe cependant des paramètres à améliorer notamment la joignabilité des médecins urgentistes et le retour d'information aux médecins traitants après le passage du patient aux urgences

Book excerpt: Introduction : La réorientation professionnelle des médecins ne fait pas exception et ces dernières années celle des urgentistes a été placée au centre des débats avec en ligne de mire la pénibilité au travail. L’objectif de notre étude était d’étudier les facteurs et motivations de la réorientation vers la médecine générale d’anciens urgentistes. Les objectifs secondaires étaient de recueillir leurs opinions sur la mise en place du DES de médecine d’urgence à l’horizon 2017 et d’évaluer les apports et limites d’une pratique préalable de la médecine d’urgence à la médecine générale. Matériel et Méthodes : Etude qualitative menée par entretiens semi-dirigés de médecins généralistes anciennement urgentistes dans le Languedoc-Roussillon, avec le support d’un guide d’entretien, et réalisés jusqu’à saturation des données. L’analyse des résultats a été thématique. Résultats : Huit entretiens ont été réalisés. Les motivations principales à la réorientation étaient d’une part les facteurs liés à la pratique de médecine d’urgence, le rythme et les conditions de travail, l’évolution de la médecine d’urgence et la peur de l’erreur médicale et d’autre part les facteurs liés à la médecine générale, la relation médecin-patient, le choix du type de pratique, la vocation, l’indépendance et l’auto-détermination. Des difficultés ont été rencontrées lors de la reconversion mais elles ont pu être surmontées par le recours à des formations, au partage d’expérience entre confrère et à un support logistique. Les opinions sur la création du DES de médecine d’urgence divergent. Malgré un apport certain en terme de formation et de reconnaissance de la filière universitaire, des craintes subsistent concernant les possibilités de réorientation. Conclusion : La création du DES de médecine d’urgence va considérablement limitée les trajectoires de réorientation telles que celles empruntées par ces anciens urgentistes. Mais les motivations présentées par ces médecins reconvertis offrent des pistes de réflexion aux instances décisionnaires concernant la démographie médicale actuelle de la médecine générale et de la médecine d’urgence

Chapter one has been largely rewritten to give an outline of current approaches to a model of comprehension of spoken language. Chapter two has a new initial section but otherwise remains as it was. Chapter three incorporates a new section on "pause" and how this interacts with rhythm, and rather more on the function of stress. Chapter four has an extended initial section but otherwise remains largely as it was. Chapter five on intonation contains several sections which have been rewritten to varying extents. Chapter six of the first edition has disappeared: in 1977, very little work had been published on "fillers" and it seemed worthwhile incorporating a chapter that sat rather oddly with the phonetic/phonological interests of the rest of the book. Not that there is a great industry of descriptions of the forms and functions of these and similar phenomena there seems no reason to retain this early but admittedly primitive account. The chapter on "paralinguistic vocal features", now chapter six, has some rewriting in the early part but considerable rewriting in the last sections. The final chapter on "teaching listening comprehension" has grown greatly in length. It still incorporates some material from the original chapter but most of it is completely rewritten.
