Malgré la banalisation de cet acte chirurgical, le risque de complications infectieuses existe. Certaines de ces infections résulteraient d'une contamination secondaire à partir d'un foyer infectieux initial. L'origine bucco-dentaire est suspectée dans certains cas de la littérature, elle serait de l'ordre de 0,04 à 0,2%. Toutefois, la relation entre l'état bucco-dentaire et une infection sur implant orthopédique est difficile à démontrer scientifiquement. Devant l'absence de certitudes, le principe de précaution prime. La consultation chez le chirurgien dentiste a pour objectifs d'assurer au patient une cavité buccale exempte de tout foyer infectieux. Le praticien intervient à deux niveaux : avant l'intervention, par le biais d'un bilan dentaire complet et après l'intervention, par le contrôle de la santé bucco-dentaire du patient. Le chirurgien dentiste participe ainsi à la prévention du risque infectieux des prothèses articulaires. Son rôle est dépendant de l'implication du patient dans son hygiène bucco-dentaire.

L'arthroplastie utilise différents matériaux pour des conceptions d'implants articulaires d'une grande diversité. L'infection d'une prothèse articulaire est la complication la plus dévastatrice ; elle peut être le résultat d'une contamination per-opératoire ou post-opératoire d'origine hématogène (infection focale). L'origine bucco-dentaire des sepsis articulaires est incriminée. Il est difficile de démontrer la relation de causalité entre des foyers infectieux ou des soins bucco-dentaires, et la survenue d’une infection seconde. Plusieurs études et rapports de cas tentent d’apporter la preuve de cette relation. Le développement d’un sepsis articulaire dépend de nombreux facteurs ; certains patients semblent plus susceptibles à l’infection que d’autres. Il n’existe pas de directives officielles concernant la prise en charge des patients porteurs d’implant articulaire, et notamment sur les modalités de l’antibioprophylaxie lors de soins bucco-dentaires ; seules des recommandations ont été émises par des sociétés savantes. A partir de ces recommandations et des données actuelles de la littérature, nous nous proposons d’établir des schémas thérapeutiques, pour les patients porteurs de prothèse articulaire ou candidat à la chirurgie, dans différentes situations.

La mise en place d’une antibioprophylaxie ou d’une antibiothérapie peut s’avérer nécessaire, des recommandations ont donc été éditées par l’AFSSAPS concernant leur utilisation.
