With the establishment of the germ theory of certain diseases and the development of information concerning immunity, new methods of specific treatment were made possible and are now practiced under the name of serum and vaccine therapy. As might be expected, the medical profession has been much interested in these methods of treatment, and applied them whenever possible. The development of vaccine and serum therapy has been slow, the methods have had to be revised and in some cases the results obtained have been found to be otherwise than was at first expected. Because of this, much confusion has arisen. The practitioner has not been able to keep pace with the developments and literature on these subjects, and finally has been forced to depend on the statements and recommendations coming from serum and vaccine laboratories, enthusiasts and ever exploiters. In this work an attempt has been made to state concisely and accurately the present knowledge concerning vaccines and immune sera. An effort has been made to establish theoretical and experimental evidence as well as clinical application of the specific treatment of bacterial diseases. To accomplish this, some space is given to infections in general, the theories of immunity, with especial emphasis on the opsonic theory of immunity, as well as the particular methods of vaccine and serum therapy. Considerable space has been given to opsonins, the opsonic index and the importance of opsonins in health and disease. This has been done because since 1904 no subject has appeared more prominently or frequently in medical literature than that concerning opsonins, opsonic immunity, and bacterial vaccines. In the first presentations of discoveries on this form of immunity and specific treatment of bacterial diseases, great possibilities were promised. Methods, which would, according to Wright, give uniform success in treatment of the large class of bacterial infections and diseases, naturally received immediate and general attention by the medical world and were at once quite generally applied. Book excerpt: DNA is a rapidly developing vaccine platform for cancer and infectious and non-infectious diseases. Plasmids are used as immunogens to encode proteins to be further synthesized in vaccine recipients. DNA is mainly synthetic, ensuring enhanced expression in the cells of vaccine recipients (mostly mammalians). Their introduction into the host induces antibody and cellular responses. The latter are often more pronounced, and mimic the events occurring in infection, especially viral. There are a few distinct ways in which the vaccine antigen can be processed and presented, which determine the resulting immune response and which can be manipulated. Routinely, the antigen synthesized within the host cell is processed by proteasome, loaded onto, and presented in a complex with MHC I molecules. Processing can be re-routed to the lysosome, or immunogen can be secreted for further presentation in a complex with MHC II. Apart from expression, vaccination efficacy depends on DNA delivery. DNA immunogens are generally administered by intramuscular or intradermal injections, usually followed by electroporation, which enhances delivery 1000-fold. Other techniques are also used, such as noninvasive introduction by biojectors, skin applications with plasters and microneedles/chips, sonication, magnetofection, and even tattooing. An intense debate regarding the pros and cons of different routes of delivery is ongoing. A number of studies have compared the effect of delivery methods at the level of immunogen expression, and the magnitude and specificity of the resulting immune response. According to some, the delivery route determines immunogenic performance; according to others, it can modulate the level of response, but not its specificity or polarity. The progress of research aiming at the optimization of DNA vaccine design, delivery, and immunogenic performance has led to a marked increase in their efficacy in large species and humans. New DNA vaccines for use in the treatment of infectious diseases, cancer, allergies, and autoimmunity are forthcoming. This Special Issue covers various aspects of DNA vaccine development.

The National Childhood Vaccine Injury Act, passed in 1986, was intended to bolster vaccine research and development through the federal coordination of vaccine initiatives and to provide relief to vaccine manufacturers facing financial burdens. The legislation also intended to address concerns about the safety of vaccines by instituting a compensation program, setting up a passive surveillance system for vaccine adverse events, and by providing information to consumers. A key component of the legislation required the U.S. Department of Health and Human Services to collaborate with the Institute of Medicine to assess concerns about the safety of vaccines and potential adverse events, especially in children. Adverse Effects of Vaccines reviews the epidemiological, clinical, and biological evidence regarding adverse health events associated with specific vaccines covered by the National Vaccine Injury Compensation Program (VICP), including the varicella zoster vaccine, influenza vaccines, the hepatitis B vaccine, and the human papillomavirus vaccine, among others. For each possible adverse event, the report reviews peer-reviewed primary studies, summarizes their findings, and evaluates the epidemiological, clinical, and biological evidence.
