I submit, in the light of increasing knowledge of the phenomena of cellular pathology: - (1) That this interpretation ignores such additional factors in the sum-total of infection as autolysis and other anomalies of cell-metabolism, as well as the toxic products of such anomalies, and is therefore far too restricted. (2) That its acceptance accounts for the limited success that has so far attended all hetero-inoculative methods of fighting infective disease. (3) That until this be realised no great advance is possible in the specific treatment and prevention, not only of conditions admittedly the result of bacterial invasion, but of many other diseases in no way related thereto. In order to maintain such propositions as these I shall bring evidence to show: - (1) That spontaneous auto-inoculation, being a process of natural cure of infection by auto-serum and auto-vaccine therapy, incites the restraint of all the factors concerned, and (2) That artificial auto-inoculation is on that account immeasurably superior to hetero-inoculation, whenever it can be conveniently applied. I shall also call attention to a novel method of interpreting temperature charts in terms of immunity curves, and shall refer to the advantages of measuring the antitryptic powers of the blood. I have endeavored to show the importance of local treatment in the early stage of the affection, particularly in the chronic form, especially where follicular degeneration has preceded the development of tubercles in the lungs. Great pains were taken in the examination of the cases of phthisis which presented themselves for treatment. Of 558 cases of phthisis that came under my observation, 311 were males and 247 females. Of this number, 377 had disease in the right lung, and 234 in the left. Of the females (247) 147 were first diseased in the right lung, and 100 in the left. In the treatment of a large number of cases during many years of observation, about the same proportion in the nature of the lesion and in its location was remarked. Hence, the plan of treatment I have recommended, was adopted, and has been carried out during a period of over twenty-five years. Book excerpt: Excerpt from Pulmonary Tuberculosis Recent research has also shown that infection with tubercle bacilli endows an organism with a certain degree of resistance, or even im munity, against further and renewed exogenic infection with the same virus. Experimental investigations have proved that it is impossible to reinfect a tuberculous animal with tubercle bacilli. Many clinical phenomena, which have hitherto baffled those who studied the disease, such as the rarity of conjugal phthisis, or of tuberculous disease in those living and working among phthisical patients, and of soldiers in the armies. Are now explained by this immunity of the tuberculous against reinfection with tubercle bacilli. Phthisis is at present considered a manifestation of immunity. Prophylaxis of infection has been shifted to the child, while that of phthisis involves more than prevention of infection. In the discussion of the clinical aspects of phthisis an attempt has been made to elaborate on the constitutional symptoms, which are still the sheet-anchor of the physician who is charged with deciding whether a patient is ill and in need of treatment. Bacteriology and serology are excellent helps in showing whether the patient has been infected with tubercle bacilli; skiagraphy reveals airless areas of lung tissue; but they do not give conclusive proof that the patient is sick and in need of prolonged and costly treatment. We also know that unity of causation is not always an indication of unity of resulting clinical phenomena in tuberculosis: The clinical picture of tuberculosis in infants is different from that in children; in adults some, irrespective of the treatment applied, show a marked tendency to sclerosis or fibrosis of the lesion; in others caseation and destruction of lung tissue go on progressively; in still others there is a sluggish course, marked by periods of illness alternating with periods of comparative comfort. For these reasons several types of the disease, or syndromes, have been described, each of which has not only a different clinical course, but also a different outlook as to recovery, and the treatment differs markedly in each form of the disease.
