Book excerpt: Introduction : l’étude réalisée évalue le devenir réfractif et les fonctions visuelles d’une population d’enfants prématurés traités pour une ROP au centre hospitalier universitaire d’Amiens à l’âge de 2 ans en fonction du schéma de traitement. Matériel et méthodes : étude de cohorte monocentrique, rétrospective et observationnelle ; ont été inclus les enfants ayant bénéficié d’un traitement par photocoagulation laser transpupillaire, IVT d’anti-VEGF ou chirurgie pour le traitement d’une ROP de type 1 ; avec un suivi ophtalmologique de 2 ans minimum après le début du traitement. Le critère de jugement principal est l’étude de la réfraction sous cycloplégie à l’âge de 2 ans selon le traitement reçu. Nous étudions ensuite le taux de strabisme et d’amblyopie en fonction du traitement. Résultats : 34 yeux de 17 patients, ont été inclus dans l’étude et répartis en 4 groupes. 8 yeux ont été traités par IVT, 9 yeux par laser, 12 yeux ont bénéficié d’un traitement combiné laser et IVT, enfin 5 yeux ont eu une intervention chirurgicale. Les résultats réfractifs (équivalent sphérique) en termes de moyenne à l’âge de 2 ans dans les groupes IVT, laser, IVT + laser et chirurgie sont respectivement de -2,78 (+/- 1,73), 2,01 (+/- 2,02), -0,33(+/- 1.64) et 2,35(+/- 8.32). Myopie et astigmatisme semblent être plus importants dans le groupe IVT (p=0.071 et p=0.017 respectivement). La fonction visuelle des enfants du groupe IVT semble meilleure, avec une absence d’amblyopie et une fréquence moindre de strabisme par rapport aux autres groupes de traitement. Discussion : notre étude met en évidence une proportion de myopie plus élevée pour le groupe IVT par rapport aux autres traitements. Cela peut s’expliquer par le traitement systématique des enfants de notre étude par le bevacizumab ; il induit davantage de myopie que les autres anti-VEGF (86). Conclusion : il semble exister après traitement des ROP, une prévalence myopique plus importante après IVT. Malgré tout, à l’âge de 2 ans, les enfants traités par IVT présentent une meilleure fonction visuelle avec un taux moindre de strabisme et d’amblyopie.

Our questions to her, though, must be clear and unambiguous. Otherwise, the answers we receive are likely to be misleading and confusing. As I have matured as a clinician, I have tried to improve my questions to increase my chances of receiving an answer. For the past decade, I have been pondering the subject of strabismus with which I have busied myself for practically two and one-half decades. I began to realize that my time, my share of wisdom, my abilities to carry out the prodigious work necessary to create a book out of nothing but thought, reading, and reflection on the work of others, as well as my own experience, were perhaps becoming limited. I do not doubt that they will become even more limited! Thus I have been led to write this book. Further, I am left in greater awe of prolific writers, particu larly those who write with the precision and attention to detail necessary for a medical text. Let me warn the reader at the outset that my approach in this book is "teleologi cal. " I am well aware of the conflict between science's notion of causality as only local and instrumental as opposed to the anthropomorphic notion of purpose or design in nature implied by the choice of this teleology.
