This has changed the demands on radiologists and pathologists to a considerable degree. At the same time there has been a manifold increase in the diagnostic possibilities offered by modern radiology, with several new modalities affording a potential for morphologic depiction and tissue character ization that was unattainable a decade ago. Today, the definitive diagnostic work-up and treatment of patients with musculoskeletal tumors is most often done in tumor centers, by groups that ideally should be composed of an orthopedic surgeon, radiotherapist, oncologist, radiologist, pathologist and cytologist. It is necessary for all the members of this team to be well versed in the surgi cal and other treatment principles, in the pathologic concepts, and in the radiologic interpretation of musculoskeletal tumors. Moreover, it is important that the modern diagnostic approach to musculoskeletal tumors is well known also at the referring center, be it a private practi tioner's office or a large hospital. This will avoid unnecessary biopsies, and repetition of radiologic and other diagnostic procedures that have already been performed at the referring center.

While high accuracy of MRI for staging purposes has been proven, we will focus in this monograph on the characterization of primary bone and soft tissue tumors by MRI. The major purpose of this monograph is to provide an atlas of magnetic resonance features of primary bone and soft tissue tumors for radiologists, orthopedic surgeons and physiotherapists. The results presented are based on investigations of 94 primary bone and soft tissue tumors and mimicking conditions by magnetic resonance imaging. Although the scale of the material allows for statistical handling, the number of patients per subgroup is too small to come to definite conclusions. We will therefore limit ourselves to the description of and comments on a great number of cases to illustrate the diagnostic potential of this new imaging modality. Book excerpt: More than ten years have elapsed since the publication of the first edition of Tumors and Tumorlike Lesions of Bone and Joints in 1981. During this period significant progress has been made in diagnosis, and particularly in staging and treatment of bone tumors. In most centers, limb-sparing surgery has replaced the more radical ablative surgery, with or without preoperative chemotherapy. This became possible due to the introduction and increased application of newer high-quality imaging techniques (CT and MRI) and in particular the use of new cytomorphologic methods of diagnosis, especially immunohisto chemistry. These advances in diagnosis and treatment have been reflected by the publication of new editions of several important text books, since their authors - pathologists, orthopedic surgeons, and radiologists - felt the necessity of updating their works in recent years. However, in these publications different criteria of classification were adopted. The number of cases compiled at the Latin American Registry of Bone Pathology increased from 4913 primary bone tumors and more than 1100 tumorlike lesions at the time of publication of the first edition of this book, to 5274 bone tumors and 1665 tumorlike lesions by the end of December 1985, when I finished my work as head of the Registry and occupied the positions of Professor of Orthopedic Pathology at St. Louis University Medical Center and Distinguished Visiting Professor at Rush-Presbyterian-St. Luke's Medical Center in Chicago.

There are several reasons for this. As primary bone tumors are rare in childhood, practitioners in a number of pediatric subspecialties are not familiar with them. The clinical symptoms and signs are often elusive, the biochemical investigations usually normal and the radio graphic features often uncharacteristic. Even the pathologist, who is the final step in arriving at the proper diagnosis and who has all the available clinical, biochemical and radiographic data, may encounter difficulties. A good tissue sample is the basis for microscopic investigation. However, bone tumors often show an extreme variety of structures which confuse even experienced bone pathologists. Therefore, histo pathologic analysis must take into account all available clinical, biochemical and radiographic data. The close cooperation of the pathologist with clinicians and, especially, radiologists is of the utmost importance.
