Book excerpt: Les choix de la contrainte et de la mobilité des inserts dans les prothèses totales de genou (PTG) sont largement débattus afin d'optimiser la cinématique articulaire et de limiter les descellements. La prothèse Vanguard Deep-Dish Rotating Platform® (Biomet), mise sur le marché en 2008, est une prothèse à conservation du ligament croisé postérieur (LCP), avec un insert mobile rotatoire et une congruence fémoro-tibiale augmentée. Notre objectif était d'évaluer les résultats cliniques, les complications et la survie à moyen terme de cet implant. Entre 2010 et 2011, une étude prospective a inclus 144 PTG primaires (majoritairement sans ciment avec patella non resurfacée), chez 136 patients : 94 femmes (69%) et 42 hommes (31%), à un âge moyen de 70 ans (44-88), et un IMC moyen de 29 kg/m2 (16-48). Les critères d'exclusion étaient : une déficience du LCP, une déformation frontale supérieure à 20° ou un flessum irréductible supérieur à 15°. Les étiologies étaient : 136 arthroses primitives (94%), 4 nécroses (3%), 2 arthroses post-traumatiques (1%) et 2 polyarthrites rhumatoïdes (1%). Les scores KSS, WOMAC, et Oxford, les complications et la survie selon Kaplan-Meier ont été analysés. Le recul moyen était de 6 ans (4-7), six patients sont décédés PTG en place au recul moyen de 5 ans (4-7) et quatre patients ont été perdus de vue. Cent trente-quatre PTG ont pu être analysées. However, in the past there has also been considerable progress in the development of our understand ing of paediatric surgical problems, otherwise we would not have pro gressed to our present achievements. The editors, therefore, do not apologize for compiling this volume but, on the contrary, feel that the publication of this volume is most timely. Modern paediatric surgery has now been practised for three genera tions. The handful of pioneers who were the founders of our specialty worked mainly before the last world war. A few dozen of the inter mediary generation started work immediately after the war, while the new generation who are now dominating our specialty must be counted in thousands. Two factors have radically altered paediatric surgery as practised by the intermediary and the present generation of surgeons.

Book excerpt: For more than a century, the condition now known as Idiopathic Hydronephrosis has been recognised as a clinical entity, and following the original description by Rayer in 1841 a variety of procedures were devised in attempts to correct the condition surgically. Most of these early methods were introduced in the last decade of the nineteenth century by several illustrious clinicians, including Trendelenburg, KOster, Fenger and Sutton. For many years diagnosis was based purely upon the patients presenting signs and symptoms and not until the early part of this century was technology available to assist in the pre-operative diagnosis of the condition. Early methods depended upon radiological techniques, and the introduction of the retrograde pyelogram by Voelcker and Lichtenberg in 1906 represented a significant advance in diagnostic methodology. Other methods also dependent upon radiographic techniques were subsequently introduced, including urography in the late 1930s by Swick, and more recently, the method of cineradio graphy, as pioneered with considerable success by Peter Narath in the decade following World War II. During the past 50 years a variety of surgical procedures have been introduced for the treatment of idiopathic hydronephrosis. That so many different methods have been devised suggests that no one specific technique is capable of achieving a complete cure in all cases."
