Book excerpt: Notre étude est une analyse des résultats d'une série rétrospective de 146 reprises de prothèse totale de hanche par le système PP, pour descellement aseptique, au recul moyen de 9 ans. En raison des décès et des perdus de vue, l'analyse clinique et radiologique repose sur la revue de 54 patients.L'implant résultats cliniques de reprise utilisé était une tige modulaire, non cimentée à traitement de surface total.A la révision, le score de Harris était en moyenne de 71 points et le score de Postel et Merle d'Aubigné de 12,8 points. Les patients étaient satisfaits ou très satisfaits dans 70% des cas. Au recul, les mesures d'épaisseur corticale ont montré une reconstruction osseuse significative. Le taux de survie était de 92,5% à 10 ans en considérant l'évènement “reprise de l'implant fémoral pour descellement aseptique”. Notre travail montre donc des résultats cliniques satisfaisants et une reconstruction osseuse à long terme avec un faible taux de ré-intervention.

Les implants à double mobilité ont pour but d'éviter ces complications. Nous avons souhaité évaluer les résultats cliniques et radiologiques de l'implant acétabulaire à double mobilité Stafit®. Matériel et méthode : il s'agit d'une étude rétro-prospective continue, monocentrique, comprenant cent-quatre-vingt-treize patients opérés d'une prothèse totale de hanche avec un implant acétabulaire à double mobilité Stafit® entre 2010 et 2013. Cette série comprenait 129 femmes et 64 hommes avec un âge moyen de 76 ans (53-91). Tous les patients ont été opérés par voie postéro-externe de Moore. L'évaluation a été réalisée à minimum un an post-opératoire et elle était clinique (score de Harris, score de Postel Merle d'Aubigné) et radiologique (usure, descellement). Nous avons également recherché la survenue de complications. Résultats : au recul de 24 mois, vingt-trois patients étaient perdus de vue et dix patients étaient décédés. Nous avons analysé cent- soixante-treize prothèses de hanche chez cent- soixante patients. The author adhered tenaciously to this theory, involving the use of a small-diameter femoral head, throughout the epoch when the large-diame ter, metal-to-metal design dominated the field. During that considerable period general satisfaction with the early results rendered criticisms of the large-diameter head unwelcome. There was a formidable array of counter criticism: the small head would pierce a film of synovial fluid; the small head would wear the socket too rapidly; the small head would always have a high risk of dislocation; detachment of the trochanter, to achieve precise orientation for the small head, was unacceptable. But all these objections have now been largely overcome. Lubrication of high molecular weight polyethylene (HMWP) on metal is now accepted as being mainly by the boundary regime with thick fluid films playing no part. We now know that HMWP can indeed tolerate the very high stresses imposed by the small head and in tribological theory there may even be some advantage in high stress. Dislocation is now known not to be an automatic sequel to the small head.

The initial injury management is crucial. Depending on the type and the severity of the lesions, different people may get involved: the players themselves or their entourage, physiotherapists, club doctors, emergency physicians, family doctors, sports doctors or hand surgeons. Treatment is usually conservative: the long fingers require rapid mobilization to prevent stiffness and contrary to that, the thumb requires stability. Surgery may be necessary to reach these goals especially for athletes, because of the demands of their sport. Chronic lesions are also in part related to repeated trauma, requiring specialized long-term multidisciplinary treatment.
