Book excerpt: Les services de médecine reçoivent quotidiennement des patients en insuffisance respiratoire aiguë (IRA) modérée. La ventilation non invasive (VNI) est souvent pratiquée sur le site. Le devenir de ces patients reste mal précisé dans la littérature. Dans une étude rétrospective, nous avons tenté d’évaluer le profil et le devenir de patients admis dans notre service pour urgence respiratoire. L’échec de notre prise en charge était défini par la nécessité d’une intubation ou le décès. 163 hospitalisations (136 patients) ont été analysées. Age moyen 66 ± 15 années. La provenance des patients était : urgences 52%, domicile 24%, réanimation 19%, autres services de médecine 5%. 105 hospitalisations (94 patients) concernaient une IRA modérée dont 50 (40 patients) nécessitaient une VNI. 73% des patients en IRA présentaient une maladie respiratoire chronique. Dans la population avec IRA, le taux de succès était de 79,4% pour une durée moyenne de séjour de 10,5 jours et un taux de mortalité de 13,5%. L’existence d’une IRA (p = 0,03), le score de gravité IGS II (p=0,04) et surtout une acidose respiratoire avec pH
