Les recommandations sur la diversification alimentaire, phase clé pour la santé future du nourrisson, sont parfois contradictoires et mal connues des médecins. L'objectif de ce travail était de faire une synthèse des recommandations françaises à la lumière des données actuelles de la littérature dans les pays développés. Le résultat sera publié sur le site LACTACLIC, base de données Evidence Based Medecine sur le thème parents-enfant à disposition des médecins et des parents. Matériel et méthode. Ce travail a utilisé la méthodologie de la Haute autorité de Santé pour 1'« Élaboration d'un document écrit d'information à l'intention des patients et des usagers du système de santé ». La phase de recherche a généré une revue narrative de la littérature. La phase d'optimisation des documents de synthèse a réuni un groupe de travail comprenant parents et professionnels de santé, concertés lors de trois réunions. La phase des tests de satisfaction a été réalisée auprès d'un échantillon de parents et de professionnels de santé en répondant à un questionnaire. Résultats. 22 articles et 7 documents officiels de sociétés savantes ont été retenus. Deux versions, « Parents » et « Professionnels de santé » ont été créés, composées d'une frise chronologique commentée, et dix règles d'or, détaillées et justifiées, bibliographie à l'appui, dans la seconde version. Conclusion. La diversification alimentaire demeure un vaste secteur de recherches en constante évolution impliquant une mise à jour régulière des médecins généralistes.

Book excerpt: La diversification alimentaire constitue une étape clé dans la vie d'un nourrisson, en impactant son développement et sa santé future. La Société européenne de gastroentérologie, hépatologie et nutrition pédiatrique (ESPGHAN) encourage un allaitement exclusif jusqu'à quatre mois avant le passage à la diversification alimentaire, entre quatre et sept mois. Depuis les années 2000, une méthode alternative à la diversification à la cuillère se répand au sein de divers pays : la diversification alimentaire menée par l'enfant (DME). La DME propose que les enfants consomment de manière autonome dès six mois, des aliments solides adaptés en taille et en texture pour leur âge. L'engouement pour ce concept s'explique par de possibles bénéfices tels qu'un meilleur apprentissage des signaux de satiété, une exposition à une alimentation plus saine et une meilleure préservation de l'obésité. Néanmoins, des craintes sont régulièrement soulevées : le risque d'étouffement, la carence en fer et en micronutriments et des apports nutritifs insuffisants. Dans ce contexte, le but de notre revue de la littérature était alors d'étudier les bénéfices et les risques inhérents à la pratique de la DME comme méthode de diversification alimentaire. Notre travail a été réalisé à partir de trois bases de recherches scientifiques, Pubmed, Embase et Cochrane Library, ainsi que Google Scholar pour la littérature grise, pour un total de trente articles intégrés. Ainsi, pratiquer la DME n'exposerait ni à un plus grand risque d'étouffement pour l'enfant ni à un risque accru de carences en fer et en zinc et les apports nutritifs seraient suffisants dès lors que les parents proposent des aliments adaptés à l'âge de leur enfant, en termes de taille et de valeurs nutritives. La DME encouragerait le partage des repas en famille et les enfants sembleraient exposés à une alimentation plus équilibrée. Ils seraient également moins difficiles et développeraient un meilleur sentiment de satiété mais ces derniers résultats pourraient être influencés par les attitudes des parents, limitant les conclusions. Concernant les effets de cette méthode sur l'obésité, les recherches, trop peu nombreuses, ne permettent pas d'apporter des éléments de réponse concluants. Il en est de même pour l'impact sur le développement moteur et les préférences alimentaires des nourrissons diversifiés par la DME. Des études complémentaires paraissent nécessaires pour une mise en pratique élargie de cette méthode, car la plupart de ces observations se limitent à des liens de corrélation. La question des allergies dans ce contexte d'introduction des solides seulement vers six mois se pose aussi. Cependant, les parents désireux de pratiquer la DME, pourraient à priori le faire sans risque à condition qu'un cadre soit respecté.

Book excerpt: Introduction : l'alimentation du jeune enfant est un sujet fréquemment abordé lors d'une consultation de médecine générale. Elle a un impact majeur sur la prévention de certaines maladies, le surpoids, l'obésité et le développement global de l'enfant. Cette étude vise à évaluer les connaissances et pratiques des jeunes parents concernant l'alimentation de leur enfant de l'âge de la diversification alimentaire à l'âge de 3 ans. Méthode : il s'agit d'une étude quantitative, descriptive, transversale réalisée auprès de parents rencontrés en soins premiers et utilisateurs de réseaux sociaux. Élaboré à partir d'une étude de la littérature et des recommandations, le questionnaire a été diffusé entre mai et août 2019. Résultats : les parents ayant répondu au questionnaire sont majoritairement des mères (86,7%), de catégorie socio-professionnelle favorisée (37%). 4,5% des enfants étaient en situation de surpoids ou d'obésité au moment du recueil des réponses. Dans la majorité des questions portant sur les connaissances (74%), les parents ont répondu correctement par rapport à ce qui était attendu. Concernant la diversification, les méconnaissances concernaient l'ajout de matière grasse dans la purée, l'âge d'introduction des protéines animales, du gluten, des laitages et des fritures, panures, charcuteries. A partir du passage en morceaux, les méconnaissances concernaient principalement la composition du dîner avec une confusion sur la nécessité ou non d'y apporter des protéines animales et des féculents. Discussion : les résultats montrent une bonne connaissance globale des recommandations de la part des jeunes parents dans un contexte socio-professionnel favorisé, ainsi qu'un respect de la mise en pratique de ces recommandations dans la majorité des cas. On note cependant une consommation encore trop importante de produits sucrés chez les enfants et une méconnaissance de certaines recommandations récentes chez les parents qui peut nous interroger sur la connaissance des médecins eux-mêmes. Notre étude comporte un biais de sélection évident et n'est pas représentative de l'ensemble de la population française. Cependant, cette étude montre que quelle que soit la catégorie socio-professionnelle, les professionnels de santé devraient mettre davantage l'accent sur l'importance de l'alimentation des 1000 premiers jours de l'enfant, ainsi que se référer davantage au nouveau carnet de santé. Conclusion : la majorité des parents de notre cohorte, issus de milieux sociaux favorisés, ont de bonnes connaissances des recommandations actuelles avec toutefois la persistance de certaines croyances erronées. Les parents ressentent globalement le besoin de recevoir plus de conseils et d'informations de la part des professionnels de santé. Ces derniers devraient davantage se référer au carnet de santé et le présenter aux parents.

The small French town of Ainay-le-Ch�teau and its environs are the site of a "family colony" for men, established in 1900. Here the patients ("lodgers") live with ordinary families ("foster parents"), hold jobs, and are free to move about the countryside. Jodelet's chronicle of daily life in the colony is made rich and vivid by extensive ethnographic material as she unravels a complex set of relationships, ultimately finding that while some of the barriers between the "other" and the larger society have been overcome, new ones have arisen in their place. This unique social experiment provides invaluable social and cultural insights, illuminating many fundamental issues in psychology, psychiatry, and sociology.

Naouri argues that the evolution of our species gave the father a central role to play in his child's development, endowing the child with the consciousness of time, balancing the mother's natural tendency to satisfy without delay every need of a child she could scarcely imagine was no longer a part of her. Given the father's diminished role, how can today's child acquire an awareness of time? Perhaps, says Naouri, infant care must be entirely re-examined, because it was based on the recommendations of pediatricians whose bias was overly maternal.
