Although designing comprehensive quality system policies and procedures at the department level would appear to be the logical approach in the right direction, precision & accuracy may not be easy to achieve due to various factors. While there can be no doubt that the data provided by practitioners of anatomic pathology is highly relevant and valuable to the practice of evidence based medicine in the clinical arena one would do well to keep in mind that the knowledge base in this speciality is predominantly observational; which can create a butterfly effect in clinical outcome. Clinical practitioners play an essential role in error reduction through several avenues such as effective test ordering, providing accurate and pertinent clinical information, procuring high-quality specimens, providing timely follow-up on test results, effectively communicating on potentially discrepant diagnoses, and advocating second opinions on the pathology diagnosis in specific situations. This is indeed the right time to consider a re-look into the functioning of our anatomic pathology departments; Multidisciplinary approach is ideal way forward in analyzing our final Diagnosis.

Clinical practitioners play an essential role in error reduction through several avenues such as effective test ordering, providing accurate and pertinent clinical information, procuring high-quality specimens, providing timely follow-up on test results, effectively communicating on potentially discrepant diagnoses, and advocating second opinions on the pathology diagnosis in specific situations. This is indeed the right time to consider a re-look into the functioning of our anatomic pathology departments, especially in light of litigation threats looming large over today's medical practice. Although designing comprehensive quality system policies and procedures at the department level would appear to be the logical approach in the right direction, precision & accuracy may not be easy to achieve due to various factors. An interdisciplinary approach would be mandatory to formulate effective treatment plans to resolve the pressing problems in medical science, particularly cancer management.

These markers are evaluated in the context of genomic, transcriptomic and proteomic profiles, which is integral to the practice of precision medicine. The application of next generation sequencing (NGS) has provided new insights in the regulation of genomic and transcriptomic structure and function. Alterations in DNA such as mutations and single nucleotide polymorphisms (SNPs) have been correlated with outcomes and provide for novel therapeutic approaches. These NGS analyses have also revealed the extensive contributions of epigenetic mechanisms such as histone modifications, non-coding RNA and alternative splicing. All of these changes together contribute to alterations in proteome. Newer assays that allow greater stability and analytical consistency are emerging. These alterations in tumor profiles can be also now detected by imaging techniques. The heterogeneity of both tumor and tumor microenvironment, an inevitable reality, is discussed in detail with particular focus on cancer stem cells and immune signaling. A chapter is dedicated to the emerging technology of “liquid biopsy”, which opens a novel approach for “continuous” monitoring of cancer that might be superior to conventional diagnostics, “Molecular Pathology of Breast Cancer” provides a quick and easy, not to mention essential, tour for clinicians, pathologists and scientists who are seeking to understand the integration of molecular biology into the diagnosis, prognosis and management of breast cancer.

The IDTP format provides the brevity and clarity that the user is not likely to receive in a textbook, even if adequately indexed. With the inclusion of descriptions for terms used in toxicology, drug metabolism/pharmacokinetics, and regulatory science, the scope of the IDTP is considerably broadened and decidedly unique in its appeal to all safety scientists. With over 800 photos and illustrations to provide visual context,* an important aim of the IDTP is to present pathological changes as reference examples for terminology, nomenclature, and term descriptions for the entry entry-level as well as seasoned toxicologic pathologist. The editors and contributing scientists (board-certified veterinary pathologists, board-certified toxicologists, allied health safety scientists, health regulatory representatives) have experience from bench-level pathology and toxicology to managing global preclinical safety units in leading pharmaceutical companies. They have considerable experience mentoring pharmaceutical industry project team members, interacting with industry clinicians and representatives of decision-making bodies within the industry, as well as with global health authorities, such as the FDA and EMA. The first is an introduction to the instruments and activities of COST.This is followed by sections dealing with: the state-of-the-art in pathology; pathology business modeling; standards and specifications in pathology; the analysis, processing, retrieval and management of images; technology and automation in pathology; and strategic developments and emerging research. As well as being a comprehensive overview of the IC0604 COST program, the book includes a selection of papers from American and Japanese researchers working in the same field.
