Book excerpt: These two volumes, entitled "Purine Metabolism in Man IV" con tain the papers presented at the Fourth International Symposium on Human Purine and Pyrimidine Metabolism", held in Maastricht (The Netherlands), June 1982. The proceedings of the three previous meet ings in Tel Aviv (Israel, 1973), Baden (Austria, 1976) and Madrid (Spain, 1979) were also published by Plenum Press. In the past few years interest in purine and pyrimidine metabo lism under normal and pathological conditions has been growing rapid ly. Apart from the more or less classical topics such as hyperuricae mia, clinical gout and urolithiasis, an increasing number of papers relating to other fields have been presented at successive meetings. Knowledge derived from the study of purine metabolism in relation to lymphocyte function, for instance, has opened up new possibilities for immunomodulation and leukaemia chemotherapy, with eventual conse quences for other types of cancer. At previous meetings there have been pointers implicating purine metabolism in relation to normal cardiac and skeletal muscle function. During the present meeting mych new data on both issues have been re ported which indicate clear differences in the pathways of ATP metabo lism. The widening of the field of interest is also illustrated by the recent work on infectious disease: exploitation of the differences in purine metabolic pathways in certain parasites compared with those in human cells has resulted in new rationales for therapy being devel oped.

Book excerpt: These volumes contain the papers which were presented at the Third International Symposium on Purine Metabolism in Man held in Madrid (Spain) in June, 1979. The previous meetings in the series were held in Tel Aviv (Israel) and in Baden (Austria) in 1973 and 1976, respectively. The proceedings were also published by Plenum. Knowledge of the pathophysiology of the purines has developed greatly since the 1950's when it was mainly related to clinical gout, and it is now relevant to many fields of Medicine and Biology. These volumes include papers reporting new work on clinical gout and urolithiasis as well as on some of the subjects which have featured prominently in the previous volumes, including: regulatory aspects of the intermediary metabolism of purines and related com pounds, enzymology, methodology, and the results of mutations which affect purine metabolism. However, there have been many new develop ments during the last three years and the scope of the communications reflects not only increasing depth of knowledge, but also a widening of the field. This publication has clinical and fundamental impli cations for internal medicine, pediatrics, urology, biochemistry, immunology, genetics, and oncology.

Book excerpt: However, there have been many new developments during the last three years and the scope of the communications reflects not only increasing depth of knowledge, but also a widening of the field. This publication has clinical and fundamental implications for internal medicine, pediatrics, urology, biochemistry, immunology, genetics, and oncology. It is interesting to compare the scope of this volume with that of its predecessors. The main emphasis has shifted from the study of gout and the dissection of metabolic pathways to encompass investigations in the fields of oncology, immunology, and lymphocyte physiology. There are pointers to possible implications in relation to cardiology and neuromuscular diseases, which may well prove to be growing points for the future. In spite of considerable work on the mechanism of urinary stone formation, the inter-relationship between uric acid and calcium oxalate urolithiasis remains obscure.

Since the first of this series of meetings was held in Israel in 1973, conventions were organized every three years in different parts of the world including the USA, Japan, and Europe. The different aspects of purine and pyrimidine metabolism bring together re searchers working in molecular genetics, biochemical pharmacology, biochemistry, devel opmental biology, immunology, epidemiology and the clinics. Oriented research in the field has been seminal for the development of potent anticancer and antiviral drugs. As the number of genes which are cloned, grows, the understanding of metabolism is increas ingly enlarged and might provide leads to further improve therapeutic concepts and to bet ter understand mechanisms responsible for the development of resistance against these drugs. In certain diseases purine and pyrimidine analogs represent not only the drugs of choice but in fact are the sole therapeutic alternative at present. The field has also taken an early lead in attempting to correct inborn errors of purine and pyrimidine metabolism by gene therapy. The organization of this meeting involved a large number of people who dedicated their time in an effort to make this symposium a success. We thank the Abstract Review Committee, the International Advisory Board and in particular the Symposium Secretariat for doing a wonderful job.

It is only recently, however, that great progress has been made in the understanding of the processes involved in purine metabolism and its disorders in man. The key enzymes active in the various pathways of purine synthesis and degradation have become known and their properties are the subject of intensive study. Major contributions to the knowledge of normal purine metabolism in man have derived from the study of inborn errors in patients with purine disorders, specifically complete and partial hypoxanthine-guanine phosphoribosyltransferase deficiency. Mutations of other enzymes involved in purine metabolism are being discovered. A great step forward has been made in the treatment of gout with the introduction of uricosuric drugs and more recently of the hypoxanthine analogue allopurinol, a synthetic xanthine oxidase inhibitor. Furthermore, the complex nature of the renal handling of urie acid excretion, although still posing difficult problems, appears to approach clari fication.

The series of international meetings at three yearly intervals have previously been held initially in 1973 in Israel, then Austria, Spain, the Netherlands, USA and Japan. The European society for the Study of Purine and Pyrimidine Metabolism in Man (ESSPPM) which has its own executive and some finance first met in Switzerland in 1987, then in Germany in 1989. The steady evolution of the science in this series of meetings is intellectually satisfying; the subsequent clinical progress is emotionally and economically reassuring. As befits the position of purines and pyrimidines at the centre of biochemistry, there has been steady scientific development into molecular genetics and now onto developmental controls and biochemical pharmacology. The complexities of the immune system are being unravelled but an understanding of the human brain largely eludes us. Laboratory based scientists now predominate over those who work as clinical specialists in vii rheumatology, immunology, oncology and paediatrics. However, there continue to be major clinical objectives since large sections are concerned with major causes of death like ATP depletion, cancer and now AIDS; the laboratory work is providing clinical solutions.
