Book excerpt: The U.S. Census Bureau has reported that 56.7 million Americans had some type of disability in 2010, which represents 18.7 percent of the civilian noninstitutionalized population included in the 2010 Survey of Income and Program Participation. The U.S. Social Security Administration (SSA) provides disability benefits through the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. As of December 2015, approximately 11 million individuals were SSDI beneficiaries, and about 8 million were SSI beneficiaries. SSA currently considers assistive devices in the nonmedical and medical areas of its program guidelines. During determinations of substantial gainful activity and income eligibility for SSI benefits, the reasonable cost of items, devices, or services applicants need to enable them to work with their impairment is subtracted from eligible earnings, even if those items or services are used for activities of daily living in addition to work. In addition, SSA considers assistive devices in its medical disability determination process and assessment of work capacity. The Promise of Assistive Technology to Enhance Activity and Work Participation provides an analysis of selected assistive products and technologies, including wheeled and seated mobility devices, upper-extremity prostheses, and products and technologies selected by the committee that pertain to hearing and to communication and speech in adults.

Children who once would have died of congenital heart conditions survive with the aid of implanted pacemakers, mechanical heart valves, and devices that close holes in the heart. Responding to a Congressional request, the Institute of Medicine assesses the system for postmarket surveillance of medical devices used with children. The book specifically examines: The Food and Drug Administration's monitoring and use of adverse event reports The agency's monitoring of manufacturers' fulfillment of commitments for postmarket studies ordered at the time of a device's approval for marketing The adequacy of postmarket studies of implanted devices to evaluate the effects of children's active lifestyles and their growth and development on device performance Postmarket surveillance of medical devices used with children is a little investigated topic, in part because the market for most medical products is concentrated among older adults. Yet children differ from adults, and their special characteristics have implications for evaluation and monitoring of the short- and long-term safety and effectiveness of medical devices used with young patients.

Book excerpt: Objective: The primary objective of the presented study was to determine the influence of upper-limb prosthesis use on standing trunk posture and postural sway dynamics in both healthy and upper limb deficient (ULD) children. Methods: Five children with ULD and five age- and sex- matched healthy control children were recruited. Subjects in both groups performed two static standing conditions with and without a prosthetic or simulator device. Prosthetic simulators were used for the healthy children in the control group to account for the level of amputation of the matched child in the ULD group. Motion and force capture were used to assess overall trunk range of motion and postural sway dynamics. In addition, a questionnaire was utilized to observe musculoskeletal complaints in specific regions of the body. Results: Trunk motion was found to be increased in healthy controls compared to ULD children, with a significant interaction (p = 0.0040) and main effects by group (p = 0.0048) and condition (p = 0.0063) in lateral bending motion. Model decomposition found significant differences by condition in the control group (p = 0.0005) and group differences when wearing ( p = 0.0002) and not wearing (p = 0.0211) a prosthetic device. No significant differences in postural sway dynamics were found between- or within- groups. Conclusions: The results of the current investigation show differences in lateral bending and flexion/extension motions of the trunk when using a prostheses in healthy children, where the ULD population did not. This presents possible variations in motor development during maturation in congenital limb loss children at a young age in compensation for the loss of a limb as compared to young healthy children.
