Book excerpt: La traumatologie dite « non vitale » représente une part importante de l'activité des services d'accueil des urgences (SAU), dont certaines lésions peuvent affecter le pronostic fonctionnel des patients. Nous avons identifié 3 pathologies fréquentes et à risque d'erreur diagnostic (luxation gléno-humérale, entorse du genou et traumatisme du rachis cervical). L'objectif de notre étude est de décrire les modalités de prise en charge des patients présentant des pathologies traumatiques non vitales dans les SAU de la Région Auvergne Rhône-Alpes. Nous avons réalisé 2 enquêtes de pratiques par questionnaire Google Form© auprès des médecins urgentistes de l'hôpital Nord-ouest (HNO) de Villefranche-sur-Saône et auprès de l'ensemble des SAU de la région Auvergne Rhône-Alpes. Concernant l'organisation des SAU, 39,4% ne possèdent pas de filière traumatologique. Pour ceux en possédant une, celle-ci est mutualisée dans 54% des cas avec une filière courte médicale. Les luxations gléno-humérales ne bénéficient d'une consultation post-urgence que dans 51% des cas. Seuls 20% des SAU proposent une formation continue concernant les traumatismes du genou. Concernant les traumatismes cervicaux, seuls 18% des SAU se basent sur des critères validés pour la prescription de radiographies. La prise en charge de la traumatologie dite « non vitale » dans les SAU est relativement homogène. La prise en charge des traumatismes cervicaux pourrait bénéficier de l'utilisation d'algorithmes de prescriptions. Concernant les traumatismes du genou et les luxations gléno-humérales, l'optimisation des prises en charge passe par l'organisation des consultations de réévaluation spécialisée et de la rééducation. L'enjeu est celui de la récupération fonctionnelle et la prévention de la récidive.

Book excerpt: Introduction Le médecin généraliste (MG) est en première ligne pour la prise en charge des pathologies courante, dont les pathologies traumatiques. Nous avons cherché à évaluer son activité de traumatologie. Matériel et méthode Il s'agit d'une étude cas-témoin, déclarative, multicentrique, entre décembre 2015 et mars 2016. Nous avons sollicité des MG titulaires de la capacité de médecine du sport, que nous avons apparié à des MG installés dans les mêmes villes. Le critère de jugement principal était la prise en charge de la traumatologie au sein du cabinet ainsi que celle de 4 pathologies simples. Résultats 23 MG formés (groupe 1) et 46 MG non titulaires du diplôme (groupe 2) ont été inclus. Tous les médecins du groupe 1 pratiquaient la traumatologie et 37 (80,4%) médecins du groupe 2, p=0,002. 19 (82,6%) médecins du groupe 1 suturaient les plaies cutanées superficielles et 27 (58,7%) dans le groupe 2. 22 (95,6%) médecins du groupe 1 prenaient en charge les entorses de cheville et 36 (78,2%) dans le groupe 2 ; p=0,024. 11 (47,8%) médecins du groupe 1 prenaient en charge les fractures non chirurgicales de tête radiale et 14 (30,4%) dans le groupe 2 ; p=0,051. Enfin, 15 (65,2%) médecins du groupe 1 prenaient en charge les fractures non chirurgicales de métacarpiens contre 29 (63,1%) dans le groupe 2.Conclusion L'activité traumatologique en ambulatoire par le MG est une réalité. Elle mérite d'être valorisée, participant du rôle de premier recours du médecin généraliste auprès de ses patients. La poursuite de la formation du médecin généraliste à la traumatologie parait donc indispensable.

Book excerpt: The subject of the apallic syndrome is one which has long been familiar to me, although I have not personally studied it as deeply as I would have wished. I became acquainted with this syndrome long before the last war, when my neurosurgical colleague Hugh Cairns (1952), made his pioneer contribution under the term "akinetic mutism" . This was an ar resting title, but it was one which did not altogether satisfy some of his colleagues, includ ing myself. We found it difficult to suggest an alternative. That is one reason why I wel come the expression "apallic syndrome" . Forensic practice has forced me from time to time to consider rather more deeply this distressing syndrome, and to try and marshal my ideas in a form which would satisfy my colleagues in the legal profession. More than once I have been instructed to make a medico legal assessment of these unfortunate patients. The points which have concerned my lawyer friends have not been matters of diagnosis, or of morbid anatomy, or of etiology. The fac tual problem which has been put before me was to make some approximate assessment as to the expectation of life. Vague guess-work is unacceptable in such circumstances. What the lawyers require is a precise and dogmatic answer.

In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.

Book excerpt: Advances in trauma care have accelerated over the past decade, spurred by the significant burden of injury from the wars in Afghanistan and Iraq. Between 2005 and 2013, the case fatality rate for United States service members injured in Afghanistan decreased by nearly 50 percent, despite an increase in the severity of injury among U.S. troops during the same period of time. But as the war in Afghanistan ends, knowledge and advances in trauma care developed by the Department of Defense (DoD) over the past decade from experiences in Afghanistan and Iraq may be lost. This would have implications for the quality of trauma care both within the DoD and in the civilian setting, where adoption of military advances in trauma care has become increasingly common and necessary to improve the response to multiple civilian casualty events. Intentional steps to codify and harvest the lessons learned within the military's trauma system are needed to ensure a ready military medical force for future combat and to prevent death from survivable injuries in both military and civilian systems. This will require partnership across military and civilian sectors and a sustained commitment from trauma system leaders at all levels to assure that the necessary knowledge and tools are not lost. A National Trauma Care System defines the components of a learning health system necessary to enable continued improvement in trauma care in both the civilian and the military sectors. This report provides recommendations to ensure that lessons learned over the past decade from the military's experiences in Afghanistan and Iraq are sustained and built upon for future combat operations and translated into the U.S. civilian system.
