Book excerpt: Although the novel, V oyages el avantures de] aques Masse, caused some thing of a stir during the first half of the eighteenth century, its author, Simon Tyssot de Patot (1655-1738), remained largely unknown in his lifetime, and it is only in this century that he has been recognized as one of the countless soldiers in the vast army of philosophes that assaulted the bastions of religious, political and sodallife in Europe of the late seven 1 teenth and early eighteenth centuries. Tyssot was a Huguenot who lived most of his life in Holland where he pursued a career as professor of mathematics in the sodal and cultural 1 Tyssot and his work seem to have been first brought to the attention of modem writers by the German critics during their investigation of the type of desert island or robinsonade literature that preceded and followed Defoe's Robinson Crusoe. The earliest reference I have found occurs in A. Kippenberg, Robinson in Deutschland bis zur Insel Felsenburg (1713-43), Hanover, 1892, pp. 66-67. Tyssot's name and work appear to have been first linked with the development of socialism in A. Lichtenberger, Le Socialisme au XVIIIe siecle, Paris, 1895, p. 44. Tyssot's Voyages et avantures de]aques Masse was discussed for its literary merits in A. LeBreton, Le Roman au dix huitieme siecle, Paris, 1898. LeBreton did not know that Tyssot was the author.

Book excerpt: Contexte : La recommandation de l'HAS publiée en 2009 sur « le diagnostic et la prise en charge de l'asthme du nourrisson de moins de 36 mois » donne des outils concrets pour détecter ces asthmes encore sous diagnostiqués et donc sous traités. Méthode : L'étude est mono-centrique descriptive et analytique, réalisée sur des données prospectives collectés à l'aide de questionnaire orale composé de deux parties, la première intégrant la totalité des nourrissons présentant une symptomatologie respiratoire et la seconde consacrée aux nourrissons diagnostiqués asthmatique avec retard. Cette étude a été réalisée dans le service de pédiatrie du Centre hospitalier de l'arrondissement de Montreuil sur Mer du premier novembre 2013 au 28 février 2014. Étaient intégrés à l'étude, tous les nourrissons de moins de 36 mois consultants aux urgences pédiatriques ou étant hospitalisés en pédiatrie pour un motif respiratoire. Une partie du questionnaire était consacrée à la prise en charge réalisée par le pédiatre du CHAM à travers la réalisation d'une radiographie thoracique et la mise en route du traitement au long cours. Résultats : Sur les 270 entrées aux urgences pédiatriques, 101 consultaient pour un motif respiratoire, dont 44% pour une toux (IC95%[34-54]). 13% d'entres eux présentaient une exacerbation d'asthme méconnu (IC95%[7-21]). Le diagnostic était posé en moyenne à l'âge de 16 mois. 92% de ces nourrissons diagnostiqués tardivement (IC95%[64-81]) avaient reçu une corticothérapie inhalée brève, des bronchodilatateurs et de la kinésithérapie respiratoire lors des épisodes antérieurs. 62% (IC95%[32-84%]) avaient reçu une antibiothérapie et 38% (IC95%[15-67]) avaient reçu un sirop antitussif. Le pédiatre du CHAM a réalisé une radiographie thoracique dans 85% des cas (IC95%[55-99]) et un traitement au long cours a été instauré pour 92% des cas (IC95%[64-99]). Conclusion : Diagnostiquer à temps ces nourrissons permet d'éviter des hospitalisations itératives, des exacerbations répétitives et de faire le bilan de cette maladie à distance pour éliminer les diagnostics différentiels. Cela permet également d'éviter des traitements inutiles voire dangereux lors des épisodes aigües.
