Löwy traces the history of prophylactic amputations through a century of preventive treatment and back to a long tradition of surgical management of gynecological problems. In the early twentieth century, surgeons came to believe that removing precancerous lesions—a term difficult to define even today—averted the danger of malignancy. This practice, Löwy finds, later led to surgical interventions for women with a hereditary predisposition to cancer but no detectable disease. Book excerpt: The 2002 National Security Strategy suggested preventive attacks, diplomacy, deterrence, and other policies as means of curtailing threats presented by the spread of nuclear, biological, and chemical (NBC) weapons to terrorists and rogue states. The author analyzes which mix of these policies might best and most cost effectively address the NBC threat, with special focus on preventive attacks. The past performances of preventive attacks, diplomacy, deterrence, and other policies as means of curtailing the NBC threat are analyzed. The central findings are that preventive attacks are generally unsuccessful at delaying the spread of NBC weapons; that deterrence, especially nuclear deterrence, is highly successful at preventing the use of NBC weapons by states; and that diplomacy has had moderate and perhaps unappreciated success at curtailing the spread of NBC weapons. The author also discusses how funds spent on preventive wars, which are much more expensive than diplomacy or deterrence, might be better spent to combat threats from terrorism and proliferation, on initiatives such as fissile material recovery, ballistic missile defense, and port security.

Book excerpt: Following the terrorist attacks against the United States on September 11, 2001, U.S. leaders recast the national security strategy to place greater emphasis on the threats posed by terrorists and by states from which they might acquire weapons of mass destruction, and announced that in the future the United States would take advantage of opportunities to strike at potential adversaries before they attacked. RAND Project Air Force examined the nature and implications of this doctrine of striking first. This study focused on three central questions: First, under what conditions is preemptive or preventive attack worth considering or pursuing as a response to perceived security threats? Second, what role should such strategies be expected to play in future U.S. national security policy? Finally, what implications do these conclusions have for planners and policymakers in the U.S. Air Force and the other armed services as they design military capabilities and strategies to deal with national security threats in the next decade? The study concluded preparing for such operations should not be a key driver for change in U.S. military capabilities because large-scale U.S. first strike will be infrequent and present few unique operational military requirements, although the intelligence requirements for these strategies are highly demanding. Planners should also recognize that this doctrine may affect the types of threats posed against U.S. forces and interests by potential adversaries.

Book excerpt: Twenty-three years ago, Israeli fighter pilots destroyed the Osiraq nuclear reactor and made a profound statement about global nuclear proliferation. In light of the recent preventive regime change in Iraq, a review of this strike reveals timely lessons for future counterproliferation actions. Using old, new, and primary source evidence, this thesis examines Osiraq for lessons from a preventive attack on a non-conventional target. Before attacking Osiraq, Israeli policymakers attempted diplomatic coercion to delay Iraq's nuclear development. Concurrent with diplomatic actions, Israeli planners developed a state of the art military plan to destroy Osiraq. Finally, Israeli leaders weathered the international storm after the strike. The thesis examines Israeli decisionmaking for each of these phases. The thesis draws two conclusions. First, preventive strikes are valuable primarily for two purposes: buying time and gaining international attention. Second, the strike provided a one-time benefit for Israel. Subsequent strikes will be less effective due to dispersed/hardened nuclear targets and limited intelligence.
