Book excerpt: This book has been prompted by recent advances in the safe prevention of thromboembolism by subcutaneous heparin prophy laxis, in particular postoperativt>ly. It has been correctly called by S. Sherry a major breakthrough in medicine. Although thrombo embolism was first recognized by Laennec in 1819 and defined by Virchow in 1846, its development was not well understood and its prevention escaped our best efforts until now. This all-too-com mon, sudden, unexpected and unwanted form of morbidity and mortality, always referred to in the surgical patient as postoper ative pulmonary thromboembolism, has now become the major complication of all surgery. However, it occurs with equal fre quency in hospitalized non-operative patients as well, so the latter are also in need of this prophylaxis if this calamity is to be avoided. The mass ofliterature generated in the past few years has produced some confusion as to which of a number of methods of heparin prophylaxis gives the best results. It is the intention of this book to help clarify the situation and thereby resolve the problem by offering a procedure of subcutaneous heparin prophylaxis which is acceptable, safe and simple to administer. Every hospital and clinic should have active management guidelines for the prevention of venous thromboembolism (DVT and PE). The prevention of venous thromboembolism (VTE) consists of early mobilisation, compression bandages or anti-embolism stockings, low molecular weight heparin (LMWH) and warfarin if long-term prophylaxis is indicated. The following anticoagulants may be used in special cases: fondaparinux (patients at high-risk of VTE, heparin allergy), dabigatran, rivaroxaban or apixaban (oral agents for use after elective knee and hip replacement surgery). Rivaroxaban may also be used for secondary prevention of venous thromboembolism and pulmonary embolism in an uncomplicated patient. Idiopathic VTE (no identified risk factors) is three times more likely to recur than provoked VTE (risk factors present). Indefinite treatment is therefore often recommended after idiopathic VTE. If VTE develops without any provoking factors in a patient aged less than 50 years, tests for hereditary coagulation disorders are indicated to facilitate the choice of treatment duration (indefinite or temporary) . Aspirin is mainly used for the prevention of arterial occlusion, but there is accumulating evidence on aspirin and statins in preventing recurrence of venous thromboembolism and pulmonary embolism. A patient who has a previous history of VTE, or is otherwise predisposed to thrombus formation, must be given guidance regarding high risk situations.

Book excerpt: The heart and lung are intricately linked. When the heart is affected by disease, the lungs will often show some related pathological or clinical conditions and vice versa. Pulmonary heart disease is by definition a condition when the lungs cause the heart to fail. The left ventricle in combination with the other structures in the “left heart” pumps blood throughout the body. The right ventricle (and structures of the “right heart”) pumps blood to the lungs where it is oxygenated and returned to the left heart for distribution. In normal circumstances, the right heart pumps blood into the lungs without any resistance. The lungs usually have minimal pressure and the right heart easily pumps blood through. However when there is lung disease present, like emphysema, chronic obstructive lung disease (COPD) or pulmonary hypertension- the small blood vessels become very stiff and rigid. The right ventricle is no longer able to push blood into the lungs and eventually fails. This is known as pulmonary heart disease. Pulmonary heart disease is also known as right heart failure or cor pulmonale. The chief cause of right heart failure is the increase in blood pressure in the lungs (pulmonary artery).

Sudden death is often the first clinical manifestation. Only a reduction in the incidence of venous thromboembolism can reduce sudden death owing to pulmonary embolism and venous stasis syndrome owing to deep vein thrombosis. Improvement in the incidence of venous thromboembolism will require (i) better recognition of persons at risk, (ii) improved estimates of the magnitude of risk, (iii)avoidance of risk exposure when possible, (iv)more widespread use of safe and effective prophylaxis when risk is unavoidable, and (v) targeting of prophylaxis to those persons who will benefit most. Early and timely diagnosis and management of deep vein thrombosis and pulmonary embolism considerably reduces the subsequent morbidity and mortality.
