Book excerpt: INTRODUCTION : Le programme sport-santé d’IDF Prescri’Forme permet aux patients hypertendus, obèses ou ayant une affection de longue durée de pratiquer du sport sur ordonnance. Prescri’Forme est organisé autour des centres référence ressources (CRR) qui évaluent et orientent les patients vers des programmes passerelles (PP), des clubs certifiés (CC) ou une pratique autonome. L’objectif de l’étude était d’analyser le parcours des patients entrés dans le programme en 2019. MATÉRIEL ET MÉTHODE : Il s’agissait d’une étude rétrospective, observationnelle descriptive. Les bilans d’activité, les médecins prescripteurs et les orientations des patients après une consultation médicale initiale ont été recueillis sur déclaration des CRR. Le nombre de médecins, d’éducateurs en APA et autres professionnels ont été recueillis, ainsi que le nombre de demi-journées de disponibilités des CRR et le nombre de CC de proximité connus des CRR. RÉSULTATS : Treize CRR ont été inclus. Sur 917 patients reçus en CMI, 14 % provenaient d’une prescription d’un médecin libéral, 63 % ont été orientés vers un PP, 2.51 % vers un CC. Il existait des disparités entre les CRR sur le volume de patients sur les moyens à leur disposition et sur les CC de proximité. CONCLUSION : Le programme Prescri’Forme est peu utilisé par les médecins libéraux. Axer les efforts sur l’information de ces acteurs, l’homogénéisation des pratiques des CRR et l’augmentation de l’offre des CC permettrait le développement de ce programme prometteur.

Ainsi, est née en janvier 2016 une loi permettant aux médecins généralistes de prescrire sur ordonnance une activité physique aux patients en ALD. A la lumière du décret d'application, a été crée en septembre 2017 à l'échelle francilienne, le programme " Prescri'forme". Néanmoins, une mission flash de l'assemblée nationale en 2018 montre que le dispositif peine à se déployer. Objectif : l'objectif de notre étude était de comprendre quels étaient les freins à la prescription du sport sur ordonnance des médecins franciliens, et de définir des pistes d'amélioration. Méthodes : il s'agit d'une étude observationnelle épidémiologique descriptive et analytique. Un questionnaire en ligne anonyme a été envoyé aux médecins généralistes inscrits aux formations médicales continues des universités franciliennes. Résultats : 227 médecins ont participé à notre étude. 73 % avaient connaissance du dispositif "sport sur ordonnance", et seul 15 % du dispositif 'Prescri'forme". Les freins majeurs retrouvés étaient un manque de formation à 73,5 %, une complexité logistique pour environ 1/4 des sondés. Ceux liés aux patients : un manque de motivation, un coût financier, et le manque de temps. Conclusion : le sport santé demeure fortement méconnu et complexe. Les pistes d'améliorations sont multiples : formations universitaires, diffusion de documentations, campagnes médiatiques, prise en charge financière partielle par la sécurité sociale, etc.

Une grande partie de la population a cependant un niveau d'activité physique insuffisant et des comportements sédentaires croissants. Les pouvoirs publics ont choisi de résoudre ce problème en permettant aux médecins généralistes de prescrire de l'activité physique aux patients porteurs de pathologies chroniques néanmoins l'impact de la loi n'a pas modifié les habitudes des prescriptions des médecins. L'un des freins à la généralisation de cette prescription est le manque de formation. Objectif : Évaluer les connaissances et les besoins des internes de médecine générale concernant la prescription médicale d'activité physique. Matériels et Méthodes : Réalisation d'une étude observationnelle descriptive transversale réalisée par l'intermédiaire d'un questionnaire électronique distribué aux internes de médecine générale d'Île-De-France entre le I er février 2022 et le 30 mars 2022. Étaient exclus les promotions d'internes antérieures à 2017 ou les internes non-inscrits en DES de médecine générale en Île-de-France.Résultats : Parmi les 426 réponses obtenues, 398 internes de médecine générale ont été inclus dans l'étude. Les participants avaient un âge moyen de 27,3 ans et 72% étaient des femmes. 49% des répondants estimaient connaître l'activité physique adaptée, 73% déclaraient ne jamais en avoir prescrit. 1,50/0 des répondants jugeaient leurs connaissances très bonnes sur le sujet du conseil en activité physique, 13% les jugeaient bonnes, 63% les jugeaient moyennes, 20% les jugeaient mauvaises et 28% les jugeaient mauvaises. Our knowledge of its dynamics has significantly progressed, thanks to an abundance of studies on social innovation both general and sector-specific. However, despite the valuable research conducted over the past years, the systematic analysis of social innovation is still contested and incomplete. The questions asked in the book will be the following: 1. What is the nature of social innovations? 2.What patterns can be identified in social innovations emerging at the local level? 3.How is the emergence and spread of social innovations related to urban governance? More precisely, which conditions and arrangements facilitate and hinders social innovation? We explore these questions using different types of data and methods, and studying different contexts. In particular, we focus on innovations that aim at solving problems of the young unemployed, single parents and migrants. This analysis is based on original research carried out in the period 2010-2013 in the framework of a European project with a specific empirical research strategy. Research was carried out in 20 cities in 10 different European countries.
