When a pregnant woman consumes fluoride, so does her baby. With every three cups of optimally fluoridated tap water a mother drinks, she consumes 0.5 mg of fluoride. Fetal fluoride levels average two-thirds of maternal levels, so her baby in the womb receives about 0.3 mg of fluoride. If a baby weighing less than six pounds receives 0.3 mg of fluoride in one day, this dosage exceeds its "Tolerable Upper Intake Level," beyond which "the risk of adverse effects increases." (Institute of Medicine, 1997) If a child 3 to 6 years old swallows more than 0.25 mg of fluoride in a pea-sized dab of toothpaste, the most that should be used for brushing, parents are warned to: "Get medical help or contact a Poison Control Center right away." (FDA, 1997) "The immature nervous system of an embryo or fetus is even more vulnerable to toxic exposures than is that of an infant." - National Scientific Council on the Developing Child (2009) A team of researchers at the EPA found "substantial evidence" that fluoride is a "developmental neurotoxicant" in the same category with alcohol, chlorpyrifos, lead, mercury, and nicotine. Book excerpt: The use of maternal urinary fluoride as a biomarker of prenatal fluoride exposure in epidemiology studies is increasing. However, the knowledge on maternal exposure to fluoride and its biomarkers, has not increased alongside. The objective of this dissertation was to improve our understanding of the dietary intake of fluoride (a major source of fluoride exposure), and spot urinary fluoride levels during pregnancy. Two secondary data analyses utilizing data from the Early Life Exposures in Mexico to ENvironmental Toxicants (ELEMENT) project were conducted, in a population of women living in a salt-fluoridated community. The first study estimated the dietary intake of fluoride over the course of pregnancy and assessed the influence of compliance with the dietary recommendations of intake of beneficial nutrients for pregnancy (calcium, iron, folate and protein) on the dietary intake of fluoride. The second study compared spot urinary fluoride in women during pregnancy and non-pregnancy (using one-year postpartum as a proxy for the non-pregnant state) and assessed associations between dietary factors (dietary fluoride intake, addition of table salt, calcium intake from diet and supplements, and dietary acid load) and urinary fluoride levels at each state. Results revealed that the median dietary intake of fluoride in pregnant women was 0.7 mg/day, increased with gestational age and in women who were moderately and highly compliant with recommendations of intake of beneficial nutrients for pregnancy. On the other hand, spot urinary fluoride levels during pregnancy did not significantly differ with those of women one-year postpartum, increased with gestational age, and decreased in calcium-supplemented women only during pregnancy. The dietary intake of fluoride, calcium, and dietary acid load were not associated with urinary fluoride in either state. Finally, reporting the addition of table salt to meals was associated with an increase in urinary fluoride only at one-year postpartum. The studies in this dissertation highlight the need for a deeper understanding of fluoride exposure and its biomarkers in the pregnant population.

Book excerpt: Background: The potential neurotoxicity of exposure to fluoride, which has sparked controversy about community water fluoridation, is poorly understood. Objective: To test the association between prenatal fluoride exposure and childhood IQ in 512 Canadian mother-child pairs. Methods: We measured fetal exposure to fluoride using: (a) maternal urinary fluoride (MUF) during pregnancy; (b) fluoride concentration in water; and (c) fluoride intake estimated from beverage consumption. We evaluated childrens IQ using the Wechsler Primary and Preschool Scale of Intelligence-III. Multiple linear regression analyses examined covariate-adjusted associations between fluoride predictors and IQ. Results: Higher MUF levels predicted lower IQ in males (B=-4.49, p=.02) but not females. Higher levels of water fluoride and fluoride intake predicted a main effect of diminished IQ. Conclusion: Exposure to fluoride during fetal development is associated with lower IQ scores. These findings, which suggest that fluoride is neurotoxic, underscore the need to critically evaluate the practice of water fluoridation.

Since publication of the 10th edition in 1989, there has been rising awareness of the impact of nutrition on chronic disease. In light of new research findings and a growing public focus on nutrition and health, the expert panel responsible for formulation RDAs reviewed and expanded its approachâ€"the result: Dietary Reference Intakes.
