In addition to overall analyses, these ears were divided into 3 groups (normal hearing, mild to moderately severe hearing loss, and severe to profound hearing loss) according to their average PTA thresholds for individual group analyses. CERA results measured using tone burst stimuli, and ASSR results measured using 100% amplitude modulated and 10% frequency modulated tone stimuli at a modulation frequency of 40 Hz were compared with reference to PTA thresholds. Overall correlations of CERA and ASSR thresholds with PTA thresholds were high, with viii Pearson r coefficients exceeding .97 and .95 respectively for each of the test frequency. Individual group correlations, however, were significant at all test frequencies only in the hearing impaired groups. CERA and ASSR thresholds differed significantly from PTA thresholds in the normal hearing group. In addition, overall and individual group mean discrepancies between CERA and PTA thresholds were in general less than those between ASSR and PTA thresholds. 78.2% of the test ears had CERA thresholds within 10 dB of PTA thresholds and 71.0% of test ears had ASSR thresholds within 15 dB of PTA thresholds, showing that CERA thresholds better predicted PTA thresholds than ASSR thresholds. Results of the present study confirmed findings from previous research that both ASSR and CERA testing were accurate in predicting PTA thresholds for adult subjects with sensorineural hearing loss, but not as accurate for those with normal hearing. Cheung for the degree of Master of Science in Audiology at the University of Hong Kong in May 2004 Apart from cortical evoked response audiometry (CERA), speech audiometry has also been used to identify non-organic hearing loss in English-speaking communities. The lack of a standardized speech test is one of the reasons why speech audiometry is not used for this purpose in Cantonese-speaking communities. The aim of the present study is to evaluate how well reception thresholds for sentences (RTS) measured by Cantonese materials and CERA thresholds relate to pure tone audiometric results. Pure tone audiometry was performed and results were used as the reference data in the present study. RTS were measured by CHINT in four conditions, quiet, noise front, noise right and noise left. CERA thresholds were measured at 0.5, 1, 2 and 4 kHz in both ears. Results of the present study suggested that RTS measured by CHINT had good correlations with pure tone audiometric results, and most participants had RTS in quiet within 10 dB A of pure tone averages, with the mean discrepancy values ranging from -6.45 to 1.44 dB A. The present study also suggested that CERA thresholds were highly correlated with pure tone thresholds, and most of the participants had CERA thresholds within 15 dB of pure tone thresholds, with the mean discrepancy values of 8.05, 9.25, 9.75 and 9.33 dB at 0.5, 1, 2 and 4 kHz respectively. As RTS measured by CHINT and CERA thresholds were correlated with pure tone vii audiometric results, and both tests yield thresholds within 10 dB to 15 dB of pure tone audiometric results for most participants, they are considered to provide a good estimate of hearing thresholds, and may be useful for identifying the presence of non-organic hearing loss. viii DOI: 10.5353/th_b3047671 Subjects: Audiometry, Evoked response - China - Hong Kong Hearing disorders - Diagnosis - China - Hong Kong

The National Research Council convened an expert committee at the request of the SSA to study the issues related to disability determination for people with hearing loss. This volume is the product of that study. Hearing Loss: Determining Eligibility for Social Security Benefits reviews current knowledge about hearing loss and its measurement and treatment, and provides an evaluation of the strengths and weaknesses of the current processes and criteria. It recommends changes to strengthen the disability determination process and ensure its reliability and fairness. Book excerpt: The Auditory Steady State Response (ASSR) procedure has been established as a frequency specific, objective audiologic measure, which can provide reliable thresholds to within 10 dB of the behavioral thresholds. In order for ASSR to find its place in the existing framework of audiometric procedures, the full potential of the procedure needs to be explored. The aim of this study was to determine the accuracy of monotic ASSR in estimating hearing thresholds in a group of 15 normal hearing subjects and 15 hearing-impaired subjects. A comparative research design was implemented. Indicating that results obtained in the study was compared to relevant literature where dichotic multiple ASSR was implemented. This was done in order to ascertain ASSR's capabilities with regard to stimulus presentation methods. Monotic single ASSR predicted behavioural thresholds in the normal hearing subjects within an average of 24 dB across the frequency range (0.5, 1, 2 & 4 kHz). In the hearing-impaired group, ASSR thresholds more closely resembled behavioural thresholds, with an average difference of 18 dB, which is consistent with recent literature. The literature suggests that better prediction of behavioural thresholds will occur with greater degrees of hearing loss, due to recruitment. The focus in this group also centered on the accurate prediction of the configuration of the hearing loss. It was found that ASSR could reasonably accurately predict the configuration of the hearing loss. In the last instance, monotic single and dichotic multiple ASSR were compared with regard to threshold estimation and prediction of configuration of the hearing loss in the hearing-impaired group. Little difference was reported between the two techniques with regard to the estimation of thresholds in both the normal hearing and hearing impaired groups. In conclusion it was established that monotic ASSR could predict behavioural thresholds of varying degrees and configurations of hearing loss in normal and hearing-impaired subjects with a reasonable amount of accuracy. At this stage, however, more research is required to establish the clinical validity of the procedure, before it is routinely included within an objective test battery.

Book excerpt: This study attempted to determine the most appropriate clinical protocol for using the Auditory Steady-State Response for the prediction of pure tone behavioral thresholds. Behavioral and Auditory Steady-State Response thresholds were measured at 500 - 4000 Hz from sixteen normal-hearing subjects. All subjects were tested using both the single-frequency ASSR test approach as well as the multiple-frequency ASSR test approach in an attempt to determine which approach was the most accurate predictor of behavioral pure-tone thresholds. Additional test parameters, such as the EEG recording channel (ipsilateral versus contralateral) and overall test duration were evaluated in order to determine the most appropriate clinical protocol for obtaining Auditory Steady-State Response thresholds. The results of this study revealed that using a combination of the multiple-frequency ASSR test approach to initially bracket threshold and the single-frequency ASSR test method to better define threshold would most likely yield the most accurate estimation of pure-tone behavioral thresholds in the most acceptable amount of time. The study also determined that a two channel recording (ipsilateral and contralateral) should be employed to yield the most sensitive ASSR thresholds.

Book excerpt: In the evaluation of special populations, such as neonates, infants and malingerers, audiologist often have to rely heavily on objective measurements to assess hearing ability. Current objective audiological procedures such as tympanometry, the acoustic reflex, auditory brainstem response and transient evoked otoacoustic emissions, however, have certain limitations, contributing to the need of an objective, non invasive, rapid, economic test of hearing that evaluate hearing ability in a wide range of frequencies. The purpose of this study was to investigate distortion product otoacoustic emissions (DPOAEs) as an objective test of hearing. The main aim was to attempt to predict hearing ability at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz with DPOAEs and artificial neural networks (ANNs) in normal and hearing-impaired ears. Other studies that attempted to predict hearing ability with DPOAEs and conventional statistical methods were only able to distinguish between normal and impaired hearing. Back propagation neural networks were trained with the pattern of all present and absent DPOAE responses of 11 DPOAE frequencies of eight DP Grams and pure tone thresholds at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. The neural network used the learned correlation between these two data sets to predict hearing ability at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. Hearing ability was not predicted as a decibel value, but into one of several categories spanning 10-15dB. Results indicated that prediction accuracy of normal hearing was 92% at 500 Hz, 87% at 1000 Hz, 84% at 2000 Hz and 91% at 4000 Hz. The prediction of hearing-impaired categories was less satisfactory, due to insufficient data for the ANNs to train on. The variables age and gender were included in some of the neural network runs to determine their effect on the distortion product. Gender had only a minor positive effect on prediction accuracy, but age affected prediction accuracy considerably in a positive way. The effect of the amount of data that the neural network had to train on was also investigated. A prediction versus ear count correlation strongly suggested that the inaccurate predictions of hearing-impaired categories is not a result of an inability of DPOAEs to predict pure tone thresholds in hearing impaired ears, but a result of insufficient data for the neural network to train on. This research concluded that DPOAEs and ANNs can be used to accurately predict hearing ability within 10dB in normal and hearing-impaired ears from 500 Hz to 4000 Hz for hearing losses of up to 65dB HL.
