Book excerpt: Introduction : L'immunothérapie par inhibiteurs de points de contrôle immunitaire (checkpoint inhibitors, CPI) a révolutionné l'approche des traitements anti tumoraux dans les années 2010 en oncologie. Par son spectre large d'activité et sa bonne tolérance, elle représente une option attractive chez les patients âgés atteints de cancers. Afin d'optimiser leur utilisation, notamment dans une population de patients âgés vulnérables ou fragiles, des biomarqueurs pronostiques et prédictifs sont nécessaires. Le score pronostic LIPI (Lung Immune Prognostic Index) est un score développé initialement chez des patients atteints de tumeurs thoraciques qui combine le dNLR (derived neutrophil to lymphocyte ratio) prétraitement (neutrophiles/(leucocytes-neutrophiles) et le lactate déshydrogénase (LDH). Dans une population de patients traités par anti-programmed death (ligand) 1 (PD-(L) 1 ) en monothérapie pour des tumeurs bronchiques, le LIPI a démontré son impact pronostic et prédictif. Son utilisation n'a pas été validée dans une cohorte de patients plus âgés quel que soit le type tumoral. Objectifs : L'objectif principal de cette étude est de valider le score LIPI chez les patients âgés de 70 ans ou plus atteints de cancers solides en stade avancé traités par anti PD-(L)1 en monothérapie. Méthodes : Le dNLR et LDH ont été collectés prospectivement chez 191 patients âgés de 70 ans ou plus atteints de cancers solides métastatiques dejuin 2014 à octobre 2017 à Gustave Roussy. Le LIPI stratifié la population en 3 groupes pronostiques différents : favorable (LIPI good) (dNLR3 et LDH à la normale), intermédiaire, (LIPI intermediate) (dNLR3 ou LDH à la normale) et défavorable (LIPI bad) si les deux facteurs sont élevés (dNLR>3 et LDH>à la normale). L'impact du LIPI a été évalué sur la survie globale (OS), la survie sans progression (PFS) et le taux de réponse. Résultats : Sur les 191 patients traités par anti PD-(L)1, 160 (84 %) avaient un ECOG PS (Performance Status) de 0-1 avec un âge médian au traitement de 77 ans (70-93). Les types tumoraux les plus fréquents étaient : le mélanome (66 %) et le carcinome bronchique non à petites cellules (15 %). Le suivi médian était de 18,8 mois (m) [IC95% 14,7 à 24,2], Parmi les patients, 38 (23 %) avaient un LIPI score favorable, 84 (51 %) un LIPI score intermédiaire et 43 (26%) un LIPI score défavorable. La survie globale (OS) médiane était de 20,7 m [IC à 95 %, 12,6 - non atteinte] vs 11,2 m [IC à 95 %, 8,41-22,2] et 4,7 m [IC à 95 %, 2,2-11,3] respectivement pour le groupe LIPI favorable, LIPI intermédiaire et LIPI défavorable (p = 0,0003) alors que la survie sans progression (PFS) médiane était de 9,2 mois [IC à 95 % : 6,2-18,1] pour le LIPI favorable vs 7,2 m [IC 95 %, 5,4-13] pour l'intermédiaire vs 3,9 [IC95 %, 2,3-8,2] pour le LIPI faible (p=0,09). Conclusions : Un LIPI score défavorable était associé à une moins bonne survie globale chez les patients âgés traités par anti PD-(L)1. Le LIPI score est un outil simple et accessible nécessitant simplement les résultats d'une prise de sang et qui pourrait servir de facteur pronostique avant le traitement par anti PD-(L) 1 en complément des autres facteurs pronostiques et prédictifs pour identifier les patients âgés vulnérables ou fragiles notamment qui ne bénéficieront pas d'un tel traitement.
