Book excerpt: Master's Thesis from the year 2020 in the subject Health - Nursing Science - Nursing Management, grade: 1.0, , language: English, abstract: The focus of this paper was to identify factors that increase the probability of COVID-19 cases in nursing homes and to provide an exemplary concept for the application of the findings using machine learning algorithms to allow future research to derive appropriate countermeasures in practice. The findings are based on 13,069 US nursing homes, and the results are mostly consistent with most recent studies around this topic. Thus, this study provides not only additional evidence for previously studied factors based on a larger population of nursing homes with a holistic approach but also complements these with features not yet examined, such as most importantly the competitive environment of a nursing home. The findings show evidence of a relationship between COVID-19 infections and fatalities and (1) the size of a nursing home, (2) a facility's age, (3) whether a nursing home is for-profit, (4) whether a nursing home is urban or rural, (5) the number of federal deficiencies, (6) the total amount of fines, (7) the concentration of residents with Medicaid, (8) the share of residents from a racial or ethnic minority, (9) the excess of beds in the respective county of a nursing home, (10) the number of infections per 100,000 people in a county, and (11) the number of deaths per 100,000 people in a county, (12) the occupancy rate, (13) the overall CMS facility rating, (14) the total reported RN staffing levels, (15) the total reported nurse staffing levels and (16) the Herfindahl Index.

Book excerpt: Nursing homes and other long term-care facilities account for a disproportionate share of COVID-19 cases and fatalities worldwide. Outbreaks in U.S. nursing homes have persisted despite nationwide visitor restrictions beginning in mid-March. An early report issued by the Centers for Disease Control and Prevention identified staff members working in multiple nursing homes as a likely source of spread from the Life Care Center in Kirkland, Washington to other skilled nursing facilities. The full extent of staff connections between nursing homes--and the crucial role these connections serve in spreading a highly contagious respiratory infection--is currently unknown given the lack of centralized data on cross-facility nursing home employment. In this paper, we perform the first large-scale analysis of nursing home connections via shared staff using device-level geolocation data from 30 million smartphones, and find that 7 percent of smartphones appearing in a nursing home also appeared in at least one other facility--even after visitor restrictions were imposed. We construct network measures of nursing home connectedness and estimate that nursing homes have, on average, connections with 15 other facilities. Controlling for demographic and other factors, a home's staff-network connections and its centrality within the greater network strongly predict COVID-19 cases. Traditional federal regulatory metrics of nursing home quality are unimportant in predicting outbreaks, consistent with recent research. Results suggest that eliminating staff linkages between nursing homes could reduce COVID-19 infections in nursing homes by 44 percent.

Future Directions for the Demography of Aging is, in many ways, the successor to that original volume. The Division of Behavioral and Social Research at the National Institute on Aging (NIA) asked the National Academies of Sciences, Engineering, and Medicine to produce an authoritative guide to new directions in demography of aging.
