Book excerpt: Over the past two decades Vietnam has made enormous progress towards achieving universal coverage (UC) for its population. Significant challenges remain, however, in terms of improving equity with continuing low rates of enrollment. Ensuring financial protection also remains an elusive goal. The Master Plan for Universal Coverage approved in 2012 by the Prime Minister directly addresses both these deficiencies in coverage. The objective of this report is to assess the implementation of Vietnam SHI and provide options for moving towards UC. This is a joint assessment with development partners, World Health Organization, United Nations Children's Fund (Unicef) and Rockefeller Foundation. Expanding breadth of coverage, particularly for those hard to reach groups such as the near-poor and informal sector would require substantially increasing general revenue subsidies and fully subsidizing the premiums for the near-poor. High enrollment rates would, however, have little impact on financial protection and equity if OOP costs remain high. Achieving UC will require sustained efforts to improve efficiency in the system, and gain better value for money from available budgetary resources; without these efforts, any further progress towards UC would be financially unsustainable. There is considerable scope for improving efficiency in Vietnam. Fragmentation in the pooling of funds gives rise to unnecessary costs. Inefficiencies in resource allocation and purchasing arrangements include: (i) an overly generous benefits package; (ii) provider payment mechanisms and the mix of incentives facing providers which result in an oversupply of services; (iii) high prices, overconsumption and inappropriate use of pharmaceuticals; and (iv) the structure and incentives embedded within the delivery system. The organization, management and governance of SHI are fragmented and often dysfunctional. The present institutional setting for SHI needs to be assessed and changed.

Since the country's transition to a "market economy with socialist orientation" in the mid-1980s, however, some of these achievements have been reversed as the "renovation" of national systems for welfare and health leaves gaps in the social safety net. L’ouvrage examine les politiques de CSU de chaque pays à partir de 3 thèmes communs : (1) l’économie politique et le processus politique d’adoption, d’atteinte et de maintien de la CSU; (2) les politiques financières d’amélioration de la couverture sanitaire; et (3) les ressources humaines en santé au service des politiques d’atteinte de la CSU. Les constats tirés de ces études pays sont une source d’enseignements auxquels peuvent faire référence les pays qui aspirent à l’adoption, à l’atteinte et au maintien de la CSU. Même si la voie vers la CSU reste spécifique à chaque pays, tous peuvent profiter des expériences des autres en apprenant des différentes approches et en évitant les risques potentiels.

The loss should have caused U.S. policy makers to question the value of their efforts to train and equip the ARVN troops, but they failed to perceive the battle's significance. In this book, a longtime U.S. Army officer and history professor at West Point provides the most comprehensive treatment of the battle in print. David Toczek not only analyzes the operation in detail but places it in the larger context of the war to better evaluate the meaning of what happened. He shows that U.S. civilian and military leadership missed an opportunity early on to learn from their mistakes when they failed to draw any connection between the ARVN's dismal performance at Ap Bac and American policies toward South Vietnam. Toczek notes that while a few tactical changes resulted from the battle, no policy changes were made, not even to the structure of the advisory system. The author also takes a look at the actions of John Paul Vann, the outspoken U.S. Army advisor at Ap Bac that Neil Sheehan wrote about in his Pulitzer Prize-winning book A Bright Shining Lie. Such a careful examination of a battle seen as a metaphor of the entire Vietnam War will prove useful to readers today eager to avoid the pitfalls of the past as they consider how best to fight insurgents of the 21st century.
