Book excerpt: Integrated care systems (ICSs) now cover all areas of the country and will soon be established as statutory bodies with major responsibilities for NHS planning and funding. But most of the heavy lifting involved in integrating care and improving population health will happen more locally in the places where people live, work and access services, meaning place-based partnerships within ICSs will play a key role in driving forward change. These will need to involve a wide range of partners to act on the full range of factors that influence health and wellbeing. The Kings Fund reviewed existing evidence and experience on place-based working, explored the development of place-based partnerships within three systems and undertook targeted engagement with local leaders from ICSs, local authorities and voluntary and community sector organisations. Book excerpt: This thesis project is an exploration of "healthcare" and how we perceive its success as a major component of a community. Community Care asks the question how can a healthcare network improve and under served community. Community care is an attempt to redefine healthcare. Healthcare traditionally for most people is a hospital or doctor's office which defines a singular enclosed entity that serves a single purpose ranging from small to larger facilities. Community care goes against the nature of the self-enclosed box facility, and tries to create a network of healthy practices, that includes farming fresh produce, exercise, health consulting, and social growth in the form of group activities. Community care aims to fight against major barriers to healing, which include access to health, lack of community resources, and high cost of care to name a few. By creating access to health, engaging the community, collaborating with residents and business we can begin to create community empowerment.To evaluate this project a design was made surrounding the paradigms called (Building Blocks to Health), which is healing the biological, sociological, and psychological nature of people. This project is made to nestle within a growing community to understand how dated communities, or communities that will undergo gentrification and obliteration may begin experiencing great success and stability. Healthcare is more than a place, it is a way of life, and community care is attempting to create both the space for this to take place and the ideal situation to begin affecting people lives on a more holistic level. When we learn together, work together, heal together, and play together we create a stronger healthier network.

Despite this potential crisis, new opportunities are emerging to support both healthcare providers and the population. Advances in understanding will change how behaviour can prevent and mitigate ill health. Our approach to health must become more ‘social’. The Health of People – a report compiled by the Campaign for Social Sciences – investigates a range of ways to cut the cost of health interventions and to improve patient outcomes as well as ways of preventing people becoming patients. The report includes arguments for and case studies in favour of a more rounded, social science informed view of health and wellbeing. It concludes with an invitation to clinicians and policy makers to think outside the box of ‘care’ about the causes and prevention of ill health.

Without better information about the effectiveness of different treatment options, the resulting uncertainty can lead to the delivery of services that may be unnecessary, unproven, or even harmful. Improving the evidence-base for medicine holds great potential to increase the quality and efficiency of medical care. The Annual Meeting, held on October 8, 2007, brought together many of the nation's leading authorities on various aspects of the issues - both challenges and opportunities - to present their perspectives and engage in discussion with the IOM membership.

Book excerpt: The catalyst for this book is the fact that noted sociologist Charles Tilly, upon his death in 2008, left one completed chapter of an unfinished manuscript entitled “Cities, States, and Trust Networks,” examining the relationships between cities and nation-states over the sweep of history, and in particular the role of trust networks in mediating this relationship. Though this was the catalyst, the book serves a broader purpose: to survey recent frontier work on cities, nation-states, and the relations between the two in historical and contemporary perspective. Essays in the book will address four main themes: city-state relations, trust networks and commitment, democracy and inequality, and the importance of historical legacies in shaping state structures, practices, and capacities. They will be global in scope, with research on the United States, Latin America, Europe, Asia, and Africa; a number of the pieces will be comparative. They will also be interdisciplinary, including works of geography, history, political science, sociology, urban planning. The book addresses several confluent needs of readers. One is to simply update themes addressed in earlier edited work such as Bringing the State Back In (1985). The author confronts current prevailing discourse by proposing that internal structures of mutual recognition, rather than the market logic of productivist hybrids, underpin successful alternative farms. By adopting a Vygotskian developmental approach, necessarily grounded in communities of practice, the author recognises the expansion of alternative farming in the social-cultural context of ‘scaling out’, rather than in the clinical economic context of ‘scaling up’ which is currently supported by the Irish state. The book relies on Honneth’s recognition theory and theories of extended cognition as an analytical starting point. Volunteering, participatory observation, and interviewing shaped the immersive component of the research which supports this book, and which is influenced by grounded theory, critical theory and cognitive ethnography.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.
