Available in PDF, EPUB and Kindle. Book excerpt: A history of surgery with a difference: a fine focus on surgical gastroenterology. The authors tell the story of how progress was made, with an emphasis on the character and work of each surgical pioneer in chronological order. The last chapter gives a biographical summary of each pioneer, including the briefest mention of their non-gastroenterological achievements. Finally, unlike any currently available book on the history of surgery, coverage of surgical evolution is taken as far as the dawn of minimal invasive ("hey-hole") surgery.

Book Contentious Issues in Surgical Gastroenterology ECAB

Download or read book Contentious Issues in Surgical Gastroenterology ECAB written by and published by Elsevier Health Sciences. This book was released on 2012-06-13 with total page 121 pages. Available in PDF, EPUB and Kindle. Book excerpt: All surgeons are convinced that they provide the best possible care to their patients and fortunately most patients are happy with their surgeons. Where safety and quality were once considered to be self-evident, these issues need to be made more explicit in modern surgery. Doctors and the general public alike are nowadays more aware of the variations in quality that exist among surgeons and institutions and of the unwarranted harmful effects surgery can have. Super- or sub-specialization seems one way to improve both its quality and safety. Surgical Gastroenterology or Gastrointestinal Surgery is one of these new superspecialties. The path towards this specialization is not easy and turf battles between General Surgery and Surgical Gastroenterology are still being fought. Professor Kaushik recounts the history and development of this superspecialty in India. He recounts that although the history of Surgical Gastroenterology in this country is short, its future is bright but, as has happened in other countries, its further division into organ-based superspecialties can be expected. ‘Practice makes one perfect’ is the motto that is probably true for the high-risk surgery which is still being performed in small institutions or by surgeons with only little expertise. The inverse correlation between volume and outcome expressed as mortality and morbidity has been shown all over the world. Wouters et al systematically analyze the literature covering this topic. They also show that concentration and regionalization do not automatically lead to an improved quality of surgery, since other issues for quality assurance are of equal importance, such as adherence to evidence-based guidelines, feedback, and monitoring. Despite all the inputs in training and instruction by experts, a uniformly high quality of surgery is hard to achieve in day-to-day gastrointestinal surgery. Sudhir Joseph and his colleagues show that a dedicated checklist can save lives and reduce postoperative morbidity in many countries with different systems and levels of healthcare. A similar beneficial effect was found in The Netherlands using a more comprehensive checklist that covers the whole period of the hospital stay. The new type of surgeon is aware of the principles of evidence-based medicine and he/she will challenge the dogmas of the ‘old’ surgery. Over the years our surgical masters have told us about the importance of good pre-operative and postoperative care for our patients. The bowel should be put at rest before operation and oral feeding restored only slowly after surgery starting with nothing by mouth to sips of clear water. Bed rest was essential for recovery. He introduced the ERAS (Enhanced Recovery after Surgery) strategy also named Fast Track Surgery. Mitchell et al provide ample evidence that all surgeons should adopt the ERAS strategy. Implementation of this will lead to better and more cost-effective operations.

Book Facing the Pancreatic Dilemma

Download or read book Facing the Pancreatic Dilemma written by Paolo Pederzoli and published by Springer. This book was released on 1994-11-29 with total page 504 pages. Available in PDF, EPUB and Kindle. Book excerpt: This volume is a compendium of the lectures, presentations, and workshops of the International Congress entitled "Facing the Pancreatic Dilemma" and of the 17th Meeting of the Italian Association for the Study of the Pancreas, held in Verona on 16 -19 June 1993. Pancreatology is still a relatively young science. Many pathophysiological aspects of pancreatic disease are as yet obscure, and a fair number of the etio pathogenetic mechanisms adduced are no more than hypothetical. We have therefore chosen to compare and contrast in a single volume the various hypo theses and interpretations put forward by leading experts in the field. In view of the swift evolution of this branch of medical science, it is sound policy to bear in mind that yesterday's heresies have become today's orthodoxy, and that todays's heresies may well turn out to be accepted orthodoxy in a not too distant future. The various "pancreatic dilemmas" are addressed systematically in these pages, and we believe that the book will prove an invaluable tool for the special ist and for any physician dealing with pancreatic disease. The book also represents a tribute by the Verona School of Pancreatology to the memory of Prof. L. A. Scuro, founder of the School and one of the outstanding Italian pioneers of pancreatology.

Book Principles of Laparoscopic Surgery

Download or read book Principles of Laparoscopic Surgery written by Maurice E. Arregui and published by Springer New York. This book was released on 1995 with total page 888 pages. Available in PDF, EPUB and Kindle. Book excerpt: In the five years since laparoscopic methods have gained widespread use in general surgery, a wealth of experience has accumulated. The surgical pioneers who rec ognized the potential value of laparoscopy for treatment many years previously certainly deserve a great deal of credit for their farsightedness. In a sense, however, the changes of the past few years amount to the development of a whole new branch of surgery. The rate of change has produced an intense demand for the surgical community to gather, structure, and disseminate the newly acquired knowledge. The safety and efficacy of this enormous shift in surgical practice hinges on this process. In Principles of Laparoscopic Surgery, the editors have asked a select group of expert surgeons to present what amounts to an encyclopedic account of current laparoscopic surgical practice. The detailed but varying opinions on the manage ment of gallbladder and complex pancreaticobiliary disease are unique to this vol ume. Not only are varying surgical viewpoints presented, but gastroenterologists and radiologists also describe alternative methods of management. Because lapa roscopic cholecystectomy is the most developed and most common procedure, Chapters 11 and 12 give an especially detailed overview, including clinical data to support each author's views. The editors have also included information from re lated fields in evolution, including minimally invasive urology, gynecology, and vas cular and cardiovascular surgery. Aiming to be comprehensive, the coverage in cludes advances as well as basic instrumentation and techniques.

Book Surgical Renaissance in the Heartland

Download or read book Surgical Renaissance in the Heartland written by Henry Buchwald and published by U of Minnesota Press. This book was released on 2020-05-12 with total page 230 pages. Available in PDF, EPUB and Kindle. Book excerpt: In 1960, fresh out of a stint in the Air Force, Henry Buchwald was recruited by Dr. Owen H. Wangensteen to join the Department of Surgery at the University of Minnesota’s medical school. For an American born in Austria, a child of the Holocaust, a position in a city then considered by some to be the “anti-Semitic capital of the United States” might seem an uneasy fit, but in the culture of innovation created by Wangensteen, Buchwald, who had chafed against the rigidity of East Coast medical practice, found everything an imaginative young surgeon could have asked for. Surgical Renaissance in the Heartland is the story of a golden era in American surgery, ushered in by Wangensteen’s creative approach to medical practice, told by one who lived it. Buchwald describes the roots, heritage, and traditions of this remarkable period at the University of Minnesota’s medical school, where the foundations of open-heart procedures, heart and pancreas transplantation, bariatric surgery, implantable infusion pump therapies, and other medical landmarks originated. Buchwald’s account of the Wangensteen era brings to life a medical culture that thrived on debate and the expression of ideas, a clinical practice bound only by the limits of a surgeon’s inspiration and imagination. As entertaining as it is informative, Surgical Renaissance in the Heartland effectively conjures the character—and characters—of a time that forever changed medicine and the lives of millions.
