A series of expert consultations were organized to assess the findings of the systematic reviews and a guideline group was established to develop this provisional collaborative framework. The framework aims to guide national programmes, clinicians and others engaged in care of patients and prevention and control of diabetes and TB on how to establish a coordinated response to both diseases, at organizational and clinical levels. The framework is based on evidence collated from systematic reviews and existing guidelines on the diagnosis and management of TB and diabetes. The systematic reviews confirmed the weak evidence base for the effectiveness and cost-effectiveness of collaborative interventions. The framework is therefore provisional; several of its recommendations are provisional pending better evidence. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Resolution WHA62.15, adopted in 2009, calls on Member States to develop a comprehensive framework for the management and care of patients with drug-resistant TB. The recommendations contained in these guidelines address the most topical questions concerning the programmatic management of drug-resistant TB: case-finding, multidrug resistance, treatment regimens, monitoring the response to treatment, and selecting models of care. The guidelines primarily target staff and medical practitioners working in TB treatment and control, and partners and organizations providing technical and financial support for care of drug-resistant TB in settings where resources are limited.
