Book excerpt: Depuis 5 ans les anticoagulants oraux directs (AOD), (anciennement NACO) sont commercialisés mais le recul sur les pratiques et les conditions d’utilisation est encore insuffisant. La pharmacovigilance consiste en la surveillance et en la prévention du risque d’effets indésirables. Suite aux nombreuses alertes et recommandations des autorités de santé, nous avons voulu analyser au Centre Régional de Pharmacovigilance de Marseille-Provence-Corse, les cas d’effets indésirables (EI) sous AOD en 2013. L’objectif principal est de décrire les EI sous AOD, ainsi que le profil des patients et les modalités de prescription afin d’évaluer leur bon usage. Le recueil de données a été principalement prospectif et réalisé à partir des cas saisis dans la base nationale de pharmacovigilance et en interrogeant les médecins traitants. Au total 107 cas d’EI sous AOD ont été rapportés et les EI les plus nombreux sont hémorragiques. Près de 95 % des EI sont graves dont 7 % ont entrainé un décès. L’évolution de l’état de santé à court terme des patients est globalement favorable mais 8 % d’entre eux garderont des séquelles. Notre étude montre que les personnes les plus concernées sont âgées et atteintes d’insuffisance rénale mais elle montre également un mésusage dans l’utilisation des AOD. Malgré les arguments avancés en faveur de ces nouvelles thérapeutiques, cela ne doit pas empécher les professionnels de santé de les prescrire ou de les dispenser avec prudence en respectant les règles de bon usage. Plusieurs pistes de travail sont ainsi à améliorer, l’information des médecins, l’accès à la clairance rénale pour le pharmacien d’officine et l’éducation thérapeutique du patient.
