Book excerpt: Adolescent mothers are statistically less likely to breastfeed their infants, and African-American adult women are less likely to breastfeed their infants (CDC, 2006). Previous studies with primarily African-American adolescent participants date back greater than 10 years. The purpose of this study was to identify perspectives of first-time, African-American adolescent mothers on breastfeeding. Methods. Two mini-focus groups were conducted with a total of six adolescents, at a teen pregnancy support center in an urban area. Participants were between the ages of 17 and 20, self-identified as African-American, and were either pregnant or had an infant less than one year old. Results. The perspectives of the adolescents were captured in four themes: Perceived benefits of breastfeeding, perceived barriers to breastfeeding, major influences on decision-making, and racial perspectives. Benefits identified by the adolescents included infant and maternal health, infant intelligence, bonding, and decreased cost associated with breastfeeding. Most expressed a desire to breastfeed, but cited pain, lack of resources, and the sole responsibility for infant feeding as the most influential barriers. Individuals influencing the adolescent's feeding decision included the adolescent's mother, family members, friends, prenatal or parenting classes, school personnel, and the father of the baby. When examining racial perspectives, participants reported no African-American breastfeeding role models. Conclusions. These results contradict current literature, which reports a minimal interest in breastfeeding in this population. Findings suggest that future studies should focus on the impact of prenatal educational interventions, as well as postnatal support from a certified lactation consultant.

Book excerpt: This longitudinal dissertation research study explored how adolescent African American/black pregnant and parenting mothers experienced motherhood and their babies. The research study was focused on the reorganization of the adolescent self, motherhood as part of their reorganization of the self, and their perceptions about their babies. It is based on data generated from a randomized sample of participants in the University of Chicago Doula Project. The data set for this dissertation research project targeted two data collection points (the Time 1, prenatal interview and Time 2 the 4-month interview) in 220 (n=220) adolescent black mothers' lives. Classical content analysis (Denzin & Lincoln, 2000) was used to extract and organize themes from mothers' responses to five questions. Responses were coded and rated for positivity and richness. A McNemar's Chi Square statistic was used to test for changes in usage of identified themes from Time 1 to Time 2. A paired t-test revealed that an increas in richness an positivity occurred from Time 1 to Time 2. Future implications for research and practice were also addressed.

The impact of parents may never be greater than during the earliest years of life, when a child's brain is rapidly developing and when nearly all of her or his experiences are created and shaped by parents and the family environment. The experience of parenting also impacts parents themselves. Parenting Matters identifies parenting knowledge, attitudes, and practices associated with positive developmental outcomes in children ages 0-8; universal/preventive and targeted strategies used in a variety of settings that have been effective with parents of young children and that support the identified knowledge, attitudes, and practices; and barriers to and facilitators for parents' use of practices that lead to healthy child outcomes as well as their participation in effective programs and services. This report makes recommendations directed at an array of stakeholders, for promoting the wide-scale adoption of effective programs and services for parents and on areas that warrant further research to inform policy and practice. It is meant to serve as a roadmap for the future of parenting policy, research, and practice in the United States.

Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

Babies who are breastfed are less likely to become overweight and obese. Many mothers in the United States want to breastfeed, and most try. And yet within only three months after giving birth, more than two-thirds of breastfeeding mothers have already begun using formula. By six months postpartum, more than half of mothers have given up on breastfeeding, and mothers who breastfeed one-yearolds or toddlers are a rarity in our society. October 2010 marked the 10th anniversary of the release of the HHS Blueprint for Action on Breastfeeding, in which former Surgeon General David Satcher, M.D., Ph. D., reiterated the commitment of previous Surgeons General to support breastfeeding as a public health goal. This was the first comprehensive framework for national action on breastfeeding. It was created through collaboration among representatives from medical, business, women's health, and advocacy groups as well as academic communities. The Blueprint provided specific action steps for the health care system, researchers, employers, and communities to better protect, promote, and support breastfeeding. I have issued this Call to Action because the time has come to set forth the important roles and responsibilities of clinicians, employers, communities, researchers, and government leaders and to urge us all to take on a commitment to enable mothers to meet their personal goals for breastfeeding. Mothers are acutely aware of and devoted to their responsibilities when it comes to feeding their children, but the responsibilities of others must be identified so that all mothers can obtain the information, help, and support they deserve when they breastfeed their infants. Identifying the support systems that are needed to help mothers meet their personal breastfeeding goals will allow them to stop feeling guilty and alone when problems with breastfeeding arise. All too often, mothers who wish to breastfeed encounter daunting challenges in moving through the health care system. Furthermore, there is often an incompatibility between employment and breastfeeding, but with help this is not impossible to overcome. Even so, because the barriers can seem insurmountable at times, many mothers stop breastfeeding. In addition, families are often unable to find the support they need in their communities to make breastfeeding work for them. From a societal perspective, many research questions related to breastfeeding remain unanswered, and for too long, breastfeeding has received insufficient national attention as a public health issue. This Call to Action describes in detail how different people and organizations can contribute to the health of mothers and their children. Rarely are we given the chance to make such a profound and lasting difference in the lives of so many. I am confident that this Call to Action will spark countless imaginative, effective, and mutually supportive endeavors that improve support for breastfeeding mothers and children in our nation."--Page v.
