A Diagnostic Dilemma. Several dermatoses and skin tumours affect the genitalia in a unique and distinct manner that they warrant separate discussion. The normal characteristics of common dermatoses are modified on genitals. For most of them this may be the only one of the many sites involved, while in others it may be predominantly confined to the genitalia. The features are frequently modified by moisture in local environment. Dermatoses involving the genital region which are not sexually transmitted are referred to as non-venereal genital dermatoses. Any genital lesion or symptom is often misinterpreted as being sexually transmitted. The vulva is a difficult site for self-examina- tion which along with hesitation to seek medical care for genital lesions leads to anxiety and fear. Being able to recognize and treat the vulval dermatoses is vital for the wellbeing of the patient. The dermatoses may also be a sign of systemic diseases and proper examination can give a clue to establish the diagnosis. Thus, it is strived in this Booklet to spotlight the importance of considering Non-venereal Dermatoses in the differential diagnosis of genital lesions, thus avoiding the misbelief that all genital lesions are sexually transmitted. Nasemann and Sauerbrey wrote the introduction to the first edition of Haut krankheiten und venerische Infektionen. they addressed this question. They promised their book would be concise and profusely illustrated to best teach the fundamentals of dermatology. The German text, now in its fourth edition, has been widely successful. In undertaking an American revised translation of their work, it was my hope to meet a need in the English-language literature similar to that met by the German text. I believe that both students and non dermatologic physicians will find this volume a useful introduction to the art and science of treating skin disorders. The practice of dermatology differs from country to country. This text is therefore not simply a translation, but an adaptation that incorporates much new material. In the pages that follow, I have built on the successful framework of the German text, incorporating its excellent photographs and other teaching aids. Discussion of therapy has been extensively revised to reflect current practice in the United States. Original chapters on male infertility and proctology (two dermatologic domains in Germany) are not included in this book. They have been replaced by wholly new chapters on cutaneous surgery and tropical dermatology. We are grateful to Ronald G. Wheeland, M.D., for contributing the surgical chapter.

This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.

Book excerpt: ​The burden of cutaneous drug reactions is significant, in both outpatient and inpatient settings, and can result in morbidity and even mortality. This book is unique in its approach to this problem. This text is divided into basic principles, common drug reactions, skin conditions mimicked by drug reactions, drug reactions to the skin appendages, life-threatening drug reactions, less common drug reactions, and special groupings of drug reactions. For the clinician, the skin can only morphologically react in to many limited ways. This is also true for the pathologist. Combining these two forever linked specialties is a synergistic paradigm that greatly enhances diagnosis, and ultimately therapy, for these pernicious conditions. Drug reactions in the skin remain a common complication of therapy. True incidences of drug reactions are not available. For general discussion, the rule of 3% can be applied with reasonable assuredness. Approximately 3% of all hospitalized patients develop an adverse cutaneous drug reaction. Approximately 3% of these reactions are considered severe. Outpatient data is even more obscure, but at least 3% of dermatology clinic outpatient visits are due to a drug reaction. Cutaneous drug reactions compromise approximately 3% of all drug reactions. Even more challenging is the fact that the most vulnerable populations to drug reactions are increasing and include the elderly patients on prolonged drug therapy, and patients that use multiple drugs at the same time.
