The purpose of this book is to present a concise panorama of the pre sent status of knowledge of this topic. This knowledge is as impor tant to molecular biologists and virologists as it is to clinicians and pathologists. To bridge the gap among these diverse groups of investigators, we conceived of a book covering a broad spectrum of the basic scientific, clinical, and pathological aspects of diseases associated with papillomaviruses. Although the principal thrust of this book is directed at human papillomaviruses, fundamental knowledge of animal viruses is essential to the current understand ing of the molecular mechanisms of cell transformation. For this reason, a chapter on animal viruses has also been included. Some of the experimental work having to do with the elucidation of transformation and other aspects of interaction between the virus and the cell cannot be based on human papillomaviruses because of a lack of suitable experimental models. Hence, some of the chapters dealing with fundamental aspects of viral molecular biol ogy are based on animal models. We were very fortunate in having persuaded a number of distin guished colleagues to contribute to this work.

Book excerpt: The etiologic involvement of specific human papillomavirus (HPV) types in cancer of the cervix and their role in a substantial proportion of other anogenital cancers and cancers of the oropharynx (most prominently cancers of the tonsils and of the larynx) label HPV as a prime target for the analysis of mechanisms leading to the development of malignant tumors in humans. As in other viral infections linked to human cancers, HPV infection is not sufficient for the induction of malignant growth. It emerges, however, as the main factor, introducing new genes into the latently infected cells whose function as oncogenes have become clearly established during the past few years. The regulation of their expression by specific host cell proteins, stimulated by intra- and intercellular signals, seems to represent a primary defense mechanism against the induction of unrestricted growth. The failure of this host cell control system, on the other hand, by mutational changes affecting the host cell genome appears to predispose for malignant conversion. This volume summarizes various aspects of HPV research and its relationship to human cancers and provides an overview of current topics in an exciting research field.

Due to public health efforts encouraging smoking cessation, the overall incidence of HNSCC has decreased in recent years in many countries. In contrast, the incidence of oropharyngeal squamous cell carcinoma (OPSCC) has increased significantly, and this subtype of HNSCC is commonly associated with human papillomavirus (HPV) infection. Moreover, individuals with HPV-positive OPSCC are generally younger and are frequently non-smokers, suggesting that HPV-associated OPSCC represents a distinct biologic entity. This volume summarizes the spectrum of current HPV-associated OPSCC research from the fundamental basic science to translational surgery and treatment approaches. Chapters are contributed by authoritative leaders in the fields of research and clinical care.
