Book excerpt: The Patient Safety Improvement Corps (PSIC), part of the Agency for Healthcare Research and Quality's (AHRQ's) patient safety initiative, is a program of three one-week sessions (didactic lessons, homework, and a team project) operated collaboratively by the AHRQ and the Veterans' Affairs (VA) National Center for Patient Safety (NCPS). Its purpose is to improve patient safety in the nation by increasing the number and capacity of health care professionals with patient safety knowledge and skills, achieved through training teams from all 50 U.S. states over three years. This report presents findings from RAND's evaluation of the first two years of the PSIC. Data were collected through in-person, group interviews with trainees at the final training session in May 2004 and May 2005, and through individual telephone interviews with the first-year trainees one year later. Overall, reported experiences were positive. Participants valued the broad perspective gained, and the tools and skills they learned and continue to use. They appreciated and continued to draw upon the technical aspects, the hands-on exercises, the knowledge gained through team projects, and the reference materials. Additionally, they value the networking opportunities, and they have made efforts to spread their knowledge. Significantly, there are strong indications that the program has contributed to actions in the field to improve patient safety. Key barriers challenging trainees' program participation and ability to make changes at their home organizations included lack of resources and cultural obstacles (such as blaming individuals for system problems). A need for continued training and programs to train larger, more-diverse teams was also noted. The findings suggest that the PSIC is making important contributions toward building a national infrastructure to support implementation of effective patient safety practices.

Book excerpt: In my original proposal to Springer for a book on Quantal Density Functional Theory, I had envisaged one that was as complete in its presentation as possible, describing the basic theory as well as the approximation methods and a host of applications. However,after workingon the bookforabout ?ve years, I realizedthat the goal was too ambitious, and that I would be writing for another ?ve years for it to be achieved. Fortunately,there was a natural breakin the material, and I proposed to my editor, Dr. Claus Ascheron, that we split the book into two components: the ?rst on the basic theoretical framework, and the second on approximation methods and applications. Dr. Ascheron consented, and I am thankful to him for agreeing to do so. Hence, we published Quantal Density Functional Theory in 2004, and are now publishing Quantal Density Functional Theory II: Approximation Methods and Applications. One signi?cant advantage of this, as it turns out, is that I have been able to incorporate in each volume the most recent understandings available. This volume, like the earlier one, is aimed at advanced undergraduates in physics and chemistry, graduate students and researchers in the ?eld. It is written in the same pedagogical style with details of all proofs and numerous ?gures provided to explain the physics. The book is independent of the ?rst volume and stands on its own. However, proofs given in the ?rst volume are not repeated here.

Book excerpt: Star Gate is the largest funded program in the history of psi research receiving about $19.933 million in funding from 1972 to 1995. Researchers from SRI International, and later at Science Applications International Corporation, in association with various U.S. intelligence agencies participated in this program. Using the remote viewing method, research focused on understanding the applicability and nature of psi in general but mostly upon informational psi. Volume 1: Remote Viewing (1972-1984) and Volume 2: Remote Viewing (1985-1995) include all aspects of RV including laboratory trials and several operational results. Volume 3 focuses on laboratory investigations on psychokinesis. Volume 4: Operational Remote Viewing: Government Memorandums and Reports includes an analysis of the applied remote viewing program and a selection of documents that provide a narrative on the behind the scenes activities of Star Gate. In a total of 504 separate missions from 1972 to 1995, remote viewing produced actionable intelligence prompting 89% of the customers to return with additional missions. The Star Gate data indicate that informational psi is a scientifically valid phenomenon. These data have led to the development of a physics and neuroscience based testable model for the underlying mechanism, which considers informational psi as a normal, albeit atypical, phenomenon. The Star Gate data found insufficient evidence to support the causal psi (psychokinesis) hypothesis.
