At the request of the Social Security Administration, the National Academies of Sciences, Engineering, and Medicine's Board on Health Care Services organized a virtual workshop on July 21, 2020, titled The State of the Science of the Use of Biomarkers to Establish the Presence and Severity of Impairments. Workshop discussions focused on the current and potential uses for biomarkers; explored legal and ethical implications associated with biomarker use in clinical decision making; and considered the possible uses of nongenetic biomarkers as tools for the diagnosis or prognosis of the severity of specific physical and mental impairments. This publication summarizes the presentations and discussion of the workshop.

The most important goal of marker metrology in osteoarthritis is that cartilage damage can be recognized at the early stage when it has not yet been detected radiologically. In addition to early recognition, follow-up of disease activity, determination of disease severity, prediction of prognosis, and evaluation of response to treatment are other purposes of marker measurement. Type II collagen is the most important structural element of joint cartilage and is relatively specific to hyaline cartilage. The main event in osteoarthritis pathophysiology is the damage of the Type II collagen network. For this reason, researches aimed at detecting osteoarthritis-specific and specific biochemical markers have focused on Type II collagen. CTX-II is currently the most investigated and promising biomarker in relation to osteoarthritis clinic.

Book excerpt: In the past decade there has been a major sea change in the way disease is diagnosed and investigated due to the advent of high throughput technologies, such as microarrays, lab on a chip, proteomics, genomics, lipomics, metabolomics etc. These advances have enabled the discovery of new and novel markers of disease relating to autoimmune disorders, cancers, endocrine diseases, genetic disorders, sensory damage, intestinal diseases etc. In many instances these developments have gone hand in hand with the discovery of biomarkers elucidated via traditional or conventional methods, such as histopathology or clinical biochemistry. Together with microprocessor-based data analysis, advanced statistics and bioinformatics these markers have been used to identify individuals with active disease or pathology as well as those who are refractory or have distinguishing pathologies. New analytical methods that have been used to identify markers of disease and is suggested that there may be as many as 40 different platforms. Unfortunately techniques and methods have not been readily transferable to other disease states and sometimes diagnosis still relies on single analytes rather than a cohort of markers. There is thus a demand for a comprehensive and focused evidenced-based text and scientific literature that addresses these issues. Hence the formulation of Biomarkers in Disease. Progress in understanding this disease has occurred with recognition that it is not simply a degenerative joint disease. Causative factors, such as joint malalignment, ligamentous abnormalities, overuse, and biomechanical and metabolic factors have been recognized as amenable to intervention; genetic factors, less so; with metabolic diseases, intermediate. Its diagnosis is based on recognition of overgrowth of bone at joint margins. This contrasts with overgrowth of bone at vertebral margins, which is not a symptomatic phenomenon and has been renamed spondylosis deformans. Osteoarthritis describes an abnormality of joints, but the severity does not necessarily produce pain. The patient and his/her symptoms need to be treated, not the x-ray.
