This book was released on 2019-02-12 with total page 832 pages. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Ophthalmology, 1909, Vol. 5: Essays, Abstracts and Reviews The perception of rainbow colors is a phenomenon of diffraction in the minute opaque particles (epithelium, opaque vesicles) of the edematous cornea. A steep excavation with visual disturbances generally is glau comatous. Although there is no typical pressure excavation which would prove the glaucomatous process with absolute certainty. The author has seen atrophic excavations of a difference of refraction of 2 diopters. The glaucomatous excavation is due not only to me chamical pressure but to the influence of the increased tension on the blood circulation, and a simultaneous affection of the optic nerve. The cavernous atrophy of Schnabel undoubtedly contributes in a number of cases to the formation of excavation, but it occurs also in other diseases and is frequently missing in glaucoma. Book excerpt: Excerpt from Ophthalmology, Vol. 9: Essays, Abstracts And, Reviews; January, 1913 T hese cases might be differently grouped to show corresponding ly different results. If the 23 instances of combined magnet proce dure were considered separately the tabulation would be much less favorable than with the 12 cases of giant magnet procedure added. On the other hand cases could be selected from the small magnet group that would give it a more favorable showing but there is no logical basis for selection, as in the case of the giant magnet. I have endeavored to present these groups to show the situation as it has developed for me, which is that the portable magnet now in general use affords the highest percentage of operative success, so far as the mere removal of a foreign body from the eye is concerned; it is better adapted to the removal of encapsulated foreign bodies of long standing than the giant magnet, or foreign bodies located in the anterior structures it the original wound has united. But. When the foreign body is small - less than 3 or 4 m.i: n. In area and can be drawn into the anterior chamber, and out through the original wound, or by a supplementary magnet through a marginal incision, a higher percentage of good vision and quiet eyes will result with the giant magnet than with the small magnet and posterior incision. If I could have but one magnet at my disposal it would be of the portable type, but I would be as reluctant to be deprived of a giant magnet for suitable cases, as I would to de pend on it for all cases. Book excerpt: Excerpt from The Ophthalmoscope, 1909, Vol. 7: A Monthly Review of Current Ophthalmology The second type has been known for many years; in fact, metastatic was the first of these forms to be recognised, and at one time, all cases of gonorrhoeal conjunctivitis were attributed to infection by metastasis. It has a characteristic clinical appearance. Early in the course of a gonorrhoea, or after some months, or during a relapse, the patient suddenly finds both eyes inflamed, with more or less muco-purulent discharge. Shortly after this, gonorrhoeal rheumatism makes its appearance. The inflammation occurs among males; tends to recur; and may remain a conjunctivitis, or may involve other parts of the eye. Book excerpt: Excerpt from The Ophthalmoscope, Vol. 5: A Monthly Review of Current Ophthalmology; April 1, 1907 My reply to the second question is emphatically in the negative for any skilful surgeon. I have operated on about cataracts, and may therefore Claim to have met with most of the common complications that embarrass the ophthalmic surgeon. I can only say that, weighing, on the one hand, the dangers of couching, and, on the other, the so-called indications for its performance, I can think of no conditions which would lead me to undertake an operation which I regard both as unscientific and as opposed to the dictates of sound surgery. I am aware that this can Claim to be nothing more than an expression of opinion, but this opinion has not been lightly formed. For many years past, I have watched the couchers' results, keeping constantly before me the views of those who differ so widely from me on this question, and my conclusions have been arrived at, after very carefully weighing all sides of a question which I have felt to be of no small importance to those who practise eye-surgery in the East. Book excerpt: Excerpt from The Ophthalmoscope, Vol. 5: A Monthly Review of Current Ophthalmology; October 1, 1907 It would seem that sufficient time has now elapsed for a fairly unprejudiced consideration of the subject to be made, and of the advantages claimed for the newer operation. In accepting the invitation of the Editor to undertake this task, I did so because, having seen Major Smith perform his operation at Jullundur nearly 200 times, I became convinced that, in several important particulars, it materially differed from that I had seen done by various surgeons and had performed myself, under the impression that Smith's teaching was being followed. It is certainly a wonderful sight to go on a pilgrimage to jullundur, and see the Civil Hospital, with 400 cataract patients, during those six weeks of autumn or spring when Smith has what he calls his throng-season.
