It provides a detailed examination of whether: (1) the Army had experienced problems in identifying, mobilizing, and deploying medical personnel; (2) medical units had their required equipment supplies, and transportation support; and (3) medical units were able to evacuate and direct patients to appropriate hospitals. The report contains recommendations to the Secretary of the Army for improving the readiness and operational effectiveness of Army medical units.

Book excerpt: Pursuant to a congressional request, GAO reviewed the Army's deployment of medical units in support of Operations Desert Shield and Desert Storm, focusing on: (1) problems the Army had in identifying, mobilizing and deploying medical personnel; (2) whether medical units had their required equipment, supplies and transportation support; and (3) whether medical units were able to evacuate and direct patients to appropriate hospitals. GAO found that: (1) the Army had problems in mobilizing and deploying medical personnel because information on them was outdated and unreliable, and many soldiers were undeployable or untrained for their mission; (2) the system to identify and assign active Army doctors and nurses to deploying medical units did not work, medical units did not keep their requirements current, and some units deployed without their full personnel complement; (3) some deployable hospitals were never fully equipped due to supply shortages and supply system malfunctions; (4) medical supply centers lacked supplies, trained personnel and transportation, and also experienced automation problems; (5) the size and weight of the forward hospitals and the lack of support vehicles limited the hospitals' mobility; (6) evacuation of patients from the battlefield was hampered by the rough terrain, lack of navigation equipment, communication difficulties, long distances and the shortage of spare parts for air ambulances; and (7) the effectiveness of patient management was lessened by poor communications.

Book excerpt: Pursuant to a congressional request, GAO reviewed the capabilities of Naval medical personnel that supported Operations Desert Shield and Desert Storm, focusing on whether the medical units: (1) were prepared to perform their assigned missions; (2) had problems in identifying and deploying medical personnel; (3) were staffed with trained personnel; and (4) had required equipment and supplies. GAO found that: (1) the Navy medical units were not prepared to fulfill their assigned missions, although the Navy demonstrated its ability to rapidly provide significant medical capabilities during wartime; (2) the units' missions included handling more casualties than they were designed for, providing noncombat medical care, supporting the evacuation of casualties out of theater, and receiving large numbers of chemically contaminated casualties; (3) the Navy did not efficiently manage its deployment and assignment of medical personnel because full mobilization did not occur; (4) the Navy's personnel information system contained outdated and inaccurate data; (5) medical personnel were deployed without adequate training; (6) the lack of inventory controls allowed incomplete, unmaintained, and incompatible equipment to be sent to the theater; (7) inadequate inventory records hampered the deployment of medical supplies and equipment; (8) the incompatibility of the Navy's automated supply systems with the Army's supply system increased order and shipping times for medical supplies; and (9) although the Navy has initiated improvements in providing in-theater medical support, it has not established time frames to correct identified problems.

GAO found that: (1) 10 of the 15 hospitals reviewed reported shortages of some medical supplies during Operation Desert Storm; (2) the extent of shortages experienced by the hospitals could not be determined due to incomplete and inconsistent data; (3) the Army's policy restricting the use of war reserve stocks for contingencies limited the release of medical supplies during the Persian Gulf conflict; (4) the Army deployed medical supplies that were only available within the Defense Personnel Support Center wholesale supply system; (5) commercial medical suppliers filed most requisitions during the Persian Gulf conflict; (6) the Army plans to fully stock a number of hospitals and at least one medical supply center with medical supplies to ensure rapid deployment; and (7) the Army plans to reduce the number of stockpiles to support regional contingencies and revise the accountability and management policies for its war reserve program.

GAO found that: (1) the Air Force could not have adequately cared for, evacuated, and regulated the predicted number of casualties because its medical and evacuation units were often inappropriately staffed for the assigned missions, logistics and equipment problems, and inadequate training for deployment conditions; (2) several factors caused patient flow problems and ineffective use of hospitalization including the lack of involvement of joint regulating officials in directing patient movement, internal and external theater communications problems, and inadequately integrated and time-consuming computer systems; (3) the Air Force believes that its ability to deploy its medical and evacuation units will improve significantly after prior recommendations have been approved and implemented; (4) the Air Force's system for staffing its medical and evacuation units was problematic, biased, and promoted short-term degradation of unit cohesion; and (5) the Air Force believes that troop morale and other problems associated with its medical and evacuation staffing system will be resolved through better unit education and training.
