Book excerpt: Preface It was a great privilege and honour for me and my deputy chairman Professor Chris Raine to host the 10th International Conference on Cholesteatoma and Ear Surgery (Chole2016) in Edinburgh, Scotland. The Council of the British Society of Otology has helped me tremendously in the preparation of this conference. Over 1000 delegates from 54 countries attended Chole2016 on 5-8 June 2016. The scientific programme included 12 keynote lectures on the basic science of chronic ear diseases; 23 national symposium; 41 round tables; two live temporal bone dissections; 14 video instructional sessions; 18 free paper sessions and poster presentations. I sincerely thank all the faculties who contributed to the scientific programs and also the delegates who provided a lively discussion during the scientific sessions. It was the exchange of knowledge between the speakers and the delegates that makes the conference so worthwhile to me. The aim of Chole2016 was to: Bring basic science on chronic ear diseases to the clinicians. Achieve consensus in definitions, classification and staging of cholesteatoma. The first aim was fulfilled by a scientific programme that focused keynotes sessions on basic sciences. These proved delegates with a fascinating insight into the cellular biology, the role of chronic inflammation, genetic advances, middle ear mechanics/physiology, bacteriology, stem cell research and novel imaging technologies in the context of cholesteatoma. At Chole2016, the joint EAONO/JOS consensus document was presented to the international scientific community for the first time. It led to a lively and productive discussion. A consensus document on classification and staging of cholesteatoma has since been produced. The Proceedings of Chole2016 is available in paper book as well an-PDF (enhanced PDF with links) and EPUB (optimized for tablets). The electronic versions are freely downloadable for all. All events and happenings are embedded in the telling about the author's personal life, his family, homes, travel and university and public citizenship.

On April 16, 2020, the Roundtable convened a workshop to explore the context for their work; to surface key issues and questions that the Roundtable should address in its initial phase; and to reach key stakeholders and constituents. This proceedings provides a record of the workshop.

There has been only sparse attention to the diagnosis and treatment of childhood glaucoma. Both pediatric ophthalmologists and glaucoma specialists provide care for such children. In some instances, they manage these individuals alone and, in others, the management is shared. For this consensus, the participation of both groups was solicited. The global faculty, consisting of leading authorities on the clinical and scientific aspects of childhood glaucoma, met in Vancouver on July 16, 2013, just prior to the World Glaucoma Congress, to discuss the reports and refine the consensus statements. As with prior meetings, it was a daunting task to seek and obtain consensus on such a complicated and nuanced subject. It is unclear how each of us decides how we practice, and evidence to guide us often is sparse. It is remarkable how few high level studies have been conducted on the management of childhood glaucoma. Hence, this consensus, as with the others, is based not only on the published literature, but also on expert opinion. Although consensus does not replace and is not a surrogate for scientific investigation, it does provide considerable value, especially when the desired evidence is lacking. The goal of this consensus was to provide a foundation for diagnosing and treating childhood glaucoma and how it can be best done in clinical practice. Identification of those areas for which we have little evidence and, therefore, the need for additional research also was a high priority. We hope that this consensus report will serve as a benchmark of our understanding. However, this consensus report, as with each of the others, is intended to be just a beginning. As science expands, the emphasis is on data, interpretation, and dogma. We disagree; open minds open new approaches. Using methodologies that are primarily molecular and genetic, we seek to refine the causes of glaucoma as well as how it is best treated, especially incorporating thoughts and hypotheses about new methods of treatment. Glaucoma is a complex disease, and genetics proves that a variety of proteins are culpable at one level. At another level, however, there are likely final common pathways and numerous feedback loops which have defied explanations to date. The search for answers goes on in basic science researcher’s laboratories and clinical ophthalmologist’s offices and operating rooms. We are all well-served by understanding that glaucoma is a neurodegenerative disease. Current attempts to solve the disease have focused on two strategic arenas: the trabecular meshwork function and its impact on intraocular pressure as a major risk factor for the disease; and the optic nerve dysfunction leading to visual loss. Genetic mutations have yielded puzzling clues to the cause, but without resolution. For example, mutations in myocilin and optineurin genes are closely connected to the phenotype, but how do they cause the disease? In the next two years, priority areas of research are signaling pathway discoveries, biomarker panels, epigenetic factors, and continued genomic studies to yield answers to the common final pathways of the disease. The final pathways are complex and redundant, such that the overlap of bio-informatics will be challenging. Current promising leads suggest the innate immune system holds important clues to both trabecular meshwork and optic nerve pathophysiology. When the primary open-angle glaucoma disease pathways are unraveled, drug discoveries and new treatment modalities will be available for better regulation of intraocular pressure and neuroprotection for the optic nerve. This volume discusses the glaucoma pipeline from several perspectives as well as future candidate classes. In the coming era, it seems probable that neuroprotection and neurorestoration will be combined with MIGS to lower pressure. The different types of MIGS are increasing in proportion to their efficacy. To date, there are no fewer than 12 choices available to glaucoma specialists. Choice of procedure depends on the underlying anatomy as well as the patient’s individual characteristics. Truthfully, the number of available surgeries depends upon their taxonomy. Most are based on the outflow system and the canal, as illustrated in this book.
