Vast stockpiles of hazardous or toxic wastes are currently residing in insecure landfills, thus imperiling our drinking water supplies. This handbook is written with the user in mind. An in-depth review of regulatory and technical requirements is presented with later sections regarding permitting and operation of incineration facilities. A comprehensive description of established and emerging incinerator technologies is included along with a number of alternatives. One of the key sections involves a detailed procedure for choosing an incinerator for a specific job, including engineering calculations and going through the bid process. Rationale for whether to buy or lease incineration equipment is included as well as details on trial burns, permitting strategies, and startup and operation of incinerators. A number of typical case histories of incinerators are presented for such diverse applications as cleaning up individual sites with transportable units, stationary facilities for in-house wastes, and incinerator ships. Appendices provide a convenient reference to physical properties, combustion parameters, detailed equipment performance nomographs and several sample permits including RCRA, TSCA and local permit applications. Gerrard tackles the thorny issue of how and where to dispose of hazardous and radioactive waste. Gerrard, who has represented dozens of municipalities and community groups that have fought landfills and incinerators, as well as companies seeking permits, clearly and succinctly analyzes a problem that has generated a tremendous amount of political conflict, emotional anguish, and transaction costs. He proposes a new system of waste disposal that involves local control, state responsibility, and national allocation to deal comprehensively with multiple waste streams. Gerrard draws on the literature of law, economics, political science, and other disciplines to analyze the domestic and international origins of wastes and their disposal patterns. Based on a study of the many failures and few successes of past siting efforts, he identifies the mistaken assumptions and policy blunders that have helped doom siting efforts. The original Blue Book was a comprehensive publication used widely in health-care centers and government agencies to assist in the adoption of national guidance. It also provided support to committed medical directors and managers to make improvements and presented practical information on waste-management techniques for medical staff and waste workers. It has been more than ten years since the first edition of the Blue Book. During the intervening period, the requirements on generators of health-care wastes have evolved and new methods have become available. Consequently, WHO recognized that it was an appropriate time to update the original text. The purpose of the second edition is to expand and update the practical information in the original Blue Book. The new Blue Book is designed to continue to be a source of impartial health-care information and guidance on safe waste-management practices. The editors' intention has been to keep the best of the original publication and supplement it with the latest relevant information. The audience for the Blue Book has expanded. Initially, the publication was intended for those directly involved in the creation and handling of health-care wastes: medical staff, health-care facility directors, ancillary health workers, infection-control officers and waste workers. This is no longer the situation. A wider range of people and organizations now have an active interest in the safe management of health-care wastes: regulators, policy-makers, development organizations, voluntary groups, environmental bodies, environmental health practitioners, advisers, researchers and students. They should also find the new Blue Book of benefit to their activities. Chapters 2 and 3 explain the various types of waste produced from health-care facilities, their typical characteristics and the hazards these wastes pose to patients, staff and the general environment. Chapters 4 and 5 introduce the guiding regulatory principles for developing local or national approaches to tackling health-care waste management and transposing these into practical plans for regions and individual health-care facilities. Specific methods and technologies are described for waste minimization, segregation and treatment of health-care wastes in Chapters 6, 7 and 8. These chapters introduce the basic features of each technology and the operational and environmental characteristics required to be achieved, followed by information on the potential advantages and disadvantages of each system. To reflect concerns about the difficulties of handling health-care wastewaters, Chapter 9 is an expanded chapter with new guidance on the various sources of wastewater and wastewater treatment options for places not connected to central sewerage systems. Further chapters address issues on economics (Chapter 10), occupational safety (Chapter 11), hygiene and infection control (Chapter 12), and staff training and public awareness (Chapter 13). Beginning in the late 1960s, toxic chemicals such as PCBs and other harmful industrial byproducts were taken out to sea to be destroyed in specially designed ships equipped with high-temperature combustion chambers and smokestacks. But public outcry arose after the environmental and health risks of ocean incineration were exposed, and the practice was banned in the early 1990s. Smoke on the Water traces the rise and fall of ocean incineration, showing how a transnational environmental movement tested the limits of U.S. political and economic power. Dario Fazzi examines the anti-ocean-incineration movement that emerged on both sides of the Atlantic, arguing that it succeeded by merging local advocacy with international mobilization. He emphasizes the role played at the grassroots level by women, migrant workers, and other underrepresented groups who were at greatest risk. Environmental groups, for their part, gathered and shared evidence about the harms of at-sea incineration, building scientific consensus and influencing international debates. Book excerpt: This preface is being written at a time of exceptional public interest in the North Sea, following media head lines on toxic algal blooms, the mass mortality of common seals, and concern over pollution levels. These headlines may suggest that pollution of the North Sea is a recent event. This is not the case. Although no data are available (methods simply did not exist), it is safe to assume that emission (both into air and water) of heavy metals already started to increase in the 19th cen tury. The growth of cities and introduction of sewer sys tems led to the discharge of raw sewage and sewage sludge. The introduction of man-made (xenobiotic) organ ic chemicals and their subsequent emission into the North Sea commenced before the second world war. The shallower and coastal areas of the North Sea receive the highest concentrations of these pollutants. Not unexpectedly, these areas - some Norwegian fjords, the Dutch coast, the German Bight - show signs of ecosystem deterioration and eutrophication. A certain percentage of the pollutants does not remain in the North Sea but is "exported" to the Atlantic. The North Sea therefore con tributes to the global input of pollutants to the world's oceans. The major part of the pollutants accumulate in the North Sea and are incorporated in the bottom sediments. Although they are "out of sight", they should not be "out of mind".
