Download or read book Organic Nitrates written by P. Needleman and published by Springer Science & Business Media. This book was released on 2012-12-06 with total page 206 pages. Available in PDF, EPUB and Kindle. Book excerpt: Nitroglycerin and other organic nitrates have been used for over a century in the treatment of angina pectoris. Millions of patients, throughout the world, have placed nitroglycerin tablets under the tongue and have experienced rapid and dramatic relief from the chest pain that frequently occurs as a manifestation of disease of the coronary arteries. The empirical observation of the safe use of nitrates for tile alleviation of the symptoms of angina have led to their widespread medical acceptance. The use of organic nitrates preceded any knowledge of their mechanism of action or their ultimate metabolic fate. Thus, more simply stated, although sub lingual nitrates helped the patients, little was known concerning what these drugs do to the body or what the body does to the drugs. A substantial number of investigators have focused on these questions especially during the last two decades. We now have considerably more insight into the pathways of degradation of organic nitrates and the relationship of the metabolic processes to the biological action of these agents. Similarly, considerable effort has been expended in understanding the mechanism of action of these agents directly on vascular smooth muscle and on cardiac work and performance. Finally, there is a more substantive understanding of the physiology of the coronary circulation as well as the" pathophysiologic manifestations of myocardial disease.

Download or read book Coronary Microvascular Dysfunction written by Filippo Crea and published by Springer Science & Business Media. This book was released on 2013-08-15 with total page 263 pages. Available in PDF, EPUB and Kindle. Book excerpt: In the past two decades a number of studies have shown that abnormalities in the function and structure of coronary microcirculation can be detected in several cardiovascular diseases. On the basis of the clinical setting in which it occurs, coronary microvascular dysfunction (CMD) can be classified into four types: CMD in the absence of any other cardiac disease; CMD in myocardial diseases; CMD in obstructive epicardial coronary artery disease; and iatrogenic CMD. In some instances CMD represents an epiphenomenon, whereas in others it represents an important marker of risk or may contribute to the pathogenesis of myocardial ischemia, thus becoming a possible therapeutic target. This book provides an update on coronary physiology and a systematic assessment of microvascular abnormalities in cardiovascular diseases, in the hope that it will assist clinicians in prevention, detection and management of CMD in their everyday activity.

Download or read book Nitrates III written by P. Lichtlen and published by Springer Science & Business Media. This book was released on 2012-12-06 with total page 713 pages. Available in PDF, EPUB and Kindle. Book excerpt: Pharmacological and clinical research on nitrates continues to be of growing interest in many centers. This is surprising in view of the fact that their favorable effects in angina pectoris were described by Brunton and by Murrell in the Lancet more than 100 years ago. As expected, a host of new information has been collected since the two previous symposia on nitrates held in Stockholm in 1975 and Berlin in 1978. New insights were gained into the pharmacology, pharmacokinetics and pharmacodynamics of nitrates, as well as into their clinical effects in acute and chronic ischemic heart diseases and in severe congestive heart failure. Relatively little progress, however, was observed in research into the basic action of nitrates. Although most investigators agree that intracellular sequestration of calcium is probably the main mechanism by which nitrates lead to the reduction of vascular smooth muscle tone, the exact site of their action still remains undefined. In contrast, dose-dependent differences in venous and arteriolar tone have long been clearly established. Treatment was again in the main stream of discussion. The question of tolerance following long-term administration was discussed in depth and the term "pseudotolerance" was introduced to describe the adaptation of the body's circulatory system to chronic vasodilation. This is especially important in long term prophylactic antiischemic treatment in stable, as well as in unstable angina pectoris (i.e. during increased vasomotor tone - spasm).

Download or read book The Heart in Rheumatic Autoimmune and Inflammatory Diseases written by Udi Nussinovitch and published by Academic Press. This book was released on 2017-02-10 with total page 768 pages. Available in PDF, EPUB and Kindle. Book excerpt: The prevalence of autoimmune diseases and rheumatic conditions is constantly increasing. Autoimmune diseases affect approximately 7-10% of the population of the United States, while more than 50,000,000 American adults suffer from some type of arthritis. The Heart in Rheumatic, Autoimmune and Inflammatory Diseases examines the complex mechanisms relating to cardiac diseases from a pathophysiological and clinical point of view. Autoimmune rheumatic diseases can affect the coronary vessels, myocardium, pericardium, heart valves and the conduction system. The diagnosis of these unique cardiac complications necessitates medical awareness and a high index of suspicion. Increased risk of advanced atherosclerosis plays a pivotal role in the development of cardiac diseases in systemic, rheumatic and autoimmune illnesses. Yet, other complex immune medicated mechanisms may contribute to the pathogenesis. Patients' optimal care requires coordination between the primary caregiver, the rheumatologist, immunologist and cardiologist. Screening for cardiovascular risk factors, recognition of high-risk patients and identification of subclinical cardiac conditions are of great importance. Moreover, regulation of inflammation, as well as abnormal immune responses and the initiation of early treatments should be the focus of patient management. A continuous attempt to identify novel therapeutic targets and change the natural history of the underlying disease and its cardiac manifestations is in progress. The book aims at providing the readers with a state of the art collection of up to date information regarding clinically important topics based on experts' perspectives. This book was a result of an extended coordinated collaboration of one-hundred and fifty-four distinguished scientists from thirty-one countries around the globe. - A review of common, as well as unusual (yet clinically significant) medical cardiac complications of prevalent rheumatic, autoimmune and inflammatory diseases. - Focuses on aspects of pathophysiological processes, clinical presentations, screening tests, prognostic implications and novel therapeutic approaches. - Presents an up-to-date "level of evidence and "strengths of recommendations for suggested therapies and reviews all randomized clinical trials, meta-analyses and other supporting published clinical findings.

Download or read book Jelly s Gold written by David Housewright and published by Minotaur Books. This book was released on 2009-05-12 with total page 318 pages. Available in PDF, EPUB and Kindle. Book excerpt: Rushmore McKenzie, a retired St. Paul policeman and unexpected millionaire, often works as an unlicensed P.I., doing favors as it suits him. When graduate students Ivy Flynn and Josh Berglund show up with a story about $8 million in missing stolen gold from the ‘30s, McKenzie is intrigued. In the early 20th century, St. Paul, Minnesota was an open city —a place where gangsters could come and stay unmolested by the local authorities. Frank "Jelly" Nash was suspected of masterminding a daring robbery of gold bars in 1933, but, before he could unload it, he was killed in the Kansas City Massacre. His gold, they believe, is still somewhere in St. Paul. But they aren't the only ones looking. So are a couple of two-bit thugs, a woman named Heavenly, a local big-wig, and others. When Berglund is shot dead outside of Ivy's apartment, the treasure hunt turns unexpectedly deadly. In this hard-boiled mystery from David Housewright, Mac McKenzie is looking for more than a legendary stash from seventy-five years ago---he's looking for a killer and the long hidden truth behind Jelly's gold.
