Book excerpt: We are pleased to present our readers the Proceedings of the International Symposium "New Perspectives in Hemodialysis, Peritoneal Dialysis, Arteriovenous Hemofiltration, and Plasma pheresis" which was held in Freiburg i. Br. (FRG) during Oct tober 6-8, 1988. The meeting was held on the occasion of opening the new dialysis unit of the University Hospital of Freiburg i. Br .. The topics discussed included membrane biocompatibility, catabolic factors associated with dialysis therapy, phar macological therapy in dialyzed patients, erythropoietin and renal anemia, new developments in CAVH, CAPD and plasmapheresis, renal replacement therapy in acute renal failure, and plasmapheresis therapy in systemic diseases. It was unfortunately impossible in this volume, to include the extended, lively and stimulating discussions which were enjoyed by the participants during the conference. The meeting has provided an unique framework for close interaction between scientists from various disciplines, including nephrology, pharmacology, hematology, cardiology, anesthesiology, surgery, intensive care medicine, and patho logy. We would like to express our gratitude and appreciation for all those who have stimulated, encouraged and supported us to hold the symposium in Freiburg. We were encouraged to proceed in our editorial venture by reviews of the previous issues which have appeared in various Nephrology Journals. These reviews have pointed to the successful use of the International Editorial Board, the broad range of topics of current interest which have been covered and the comprehensive and practical nature of the reviews. The principal aim of the Yearbook remains to provide reviews which are more current than those which appear in Nephrology textbooks and which can be in the hands of the readers a few months after the authors have completed the manuscripts. The appointed authors are always experts in the field, who are asked to give an objective review of the topic, up-dating the readers on the world-wide literature and providing them with a complete, accurate and up-ta-date list of important recent references. We have decided to maintain the successful format of the first three issues. Book excerpt: Over the last 10 years the syndrome of severe acute renal failure has progressively changed in its epidemiology. It is now most frequently seen in critically ill patients, typically in the context of sepsis and multiorgan failure. This epidemiologic change has meant that intensive care physicians and nephrologists must now work in close cooperation at all times and must take many com plex issues of prevention, pathogenesis, and management into account that they did not previously have to tackle. Simultaneously, the last 10 years have seen the development of major technical and conceptual changes in the field of renal replacement therapy. There are now previously unavailable therapeutic options that provide physicians with a flexible and rapidly evolving armamentarium. The nutrition of these patients, previously limited by the par tial efficacy of renal replacement therapies, has also become more aggressive and more in tune with the needs of critically ill patients. Increased understanding of the pathogenesis of the multi organ failure syndrome has focused on the role of many soluble "mediators of injury" (cytokines, leukotrienes, prostanoids etc.). These molecules are likely to participate in the pathogenesis of acute renal failure. Their generation and disposal is also affected by different techniques of artificial renal support.

Book excerpt: Twenty years ago, the enzyme superoxide dismutase which uses the superoxide radical anion as its specific substrate was reported. With this discovery was born a new scientific field, in which oxygen, necessary for aerobi c 1 ife on thi s planet, had to be cons i dered also in terms of its toxicity and stresses. This stimulated the search for knowledge of active oxygen species in biology and medicine. Superoxide and other reactive oxygen species are now implicated in many disease processes. Major advances have been achieved during these past years with respect to free radical generation and mechanisms of free radical action in causing tissue injury. In parallel, the possibil ity of influencing free radical related disease processes by antioxidant treatment was studied in various in vitro and in vivo systems. This was the unique theme of a conference organized in Paris by the Society for Free Radical Research (December 9-10, 1988) which brought together experts from basic sciences and clinicians in order to evaluate the current status of antioxidant therapy. The conference emphasized fundamental processes in antioxidant action. Among the major topics were superoxide dismutase (SOD) and low molecular weight substances with such activity, called SOD mimics. Other antioxidant enzymes were also considered. Antioxidant vitamins, in particular vitamins E and C, other naturally occurri ng antioxidants and vari ous synthet i c antioxidants were included in the presentations as there is now a rapidly developing series of compounds with potentially interesting clinical applications.

Vitamin and mineral requirements were often related to the intakes needed to prevent overt signs of deficiency diseases such as beriberi or scurvy. However, in the past decade or so, there have been intensive investigations to determine the subtle changes in physiological functions associated with marginal micronutrient intakes. There is a growing consensus that immune system activities are very sensitive indicators of micronutrient status. During this decade, there has also been a rapid expansion of research in the role of free radicals and antioxidants in the major chronic diseases which afflict mankind( i.e. cancer, cardiovascular disease,and autoimmune disease). The main function of antioxidant nutrients in an appropriate diet is the prevention of oxidative damage to cells and their physiological functions. Antioxidant nutrients counteract free radicals and damaging oxidative actions on cell membranes. Since the cells of the immune system are rapidly differentiating and proliferating, such dividing and transforming cells are particularly susceptible to damage by oxidation. The interactions of antioxidant nutrition and immune system activities and disease resistance are therefore logical areas for research. Thus, the objective of this symposium was to bring together the leading investigators who have examined the immunological effects of dietary essential nutrients which share the capacity to act as antioxidants.

The institution of the Diagnosis Related Group (DRG) method for reimbursement by both government agencies and private insurance carriers has provided a financial aspect to the established clinical reasons for rapid laboratory diagnosis. Now the health of the institution, as well as the patient, is dependent on a timely diagnosis and, hopefully, cure. Accordingly, the goal of this symposium was to present the latest developments in "same-day microbiology". In the face of stable or diminishing resources, the laboratory director is presented with many choices. Do nucleic acid probes, non instrumental ELISA techniques, or time-resolved fluorometry have a place in his or her laboratory? Should the laboratory test for newly described human pathogens such as human immunodeficiency virus or human papilloma virus? Can rapid techniques supplant conventional methods?
