Book excerpt: Nerve growth factor (NGF) has a key role not only in the development of sensory and autonomic neurons, but also in the processes of nociception. Several central and peripheral mechanisms have been postulated as the basis of effects of NGF in nociceptive pathways. It has been implicated both in inflammatory and neuropathic pain mechanisms and strategies against NGF, its receptors, and down-stream intracellular signalling activated by this neurotrophin (NT) have been proposed for the treatment of these pathological conditions. This book examines recent evidence of the involvement of NGF in painful diseases and suggests the potential usefulness of anti-NGF strategies as novel analgesics for these disabling pathological conditions.

Upregulation of nerve growth factor (NGF) in inflammatory states has been associated with increased nociception. NGF promotes the growth and survival of sympathetic and primary sensory neurons by exerting its biological activity through its cognate receptor TrkA. Previous research has shown that, in the CNS, NGF is released from cells as a precursor (proNGF) together with the enzymes required for its conversion into the mature form (mNGF) and for its degradation. Interestingly, in a rat model of poly-arthritis, our lab demonstrated that sympathetic fibers invaded the upper dermis of the skin over the inflamed joints, a region from where they are normally absent. These ectopic fibers wrapped around peptidergic nociceptive fibers.

Substances released from activated immune cells (cytokines) stimulate peripheral nerves, thereby signaling the brain and spinal cord that infection/inflammation has occurred. Additionally, peripheral infection/inflammation leads to de novo synthesis and release of cytokines within the brain and spinal cord. Thus, cytokines effect neural activation both peripherally and centrally. Through this communication pathway, cytokines such as interleukin-1, interleukin-6 and tumor necrosis factor markedly alter brain function, physiology and behavior. One important but underrecognized aspect of this communication is the dramatic impact that immune activation has on pain modulation. The purpose of this book is to examine, for the first time, immune-to-brain communication from the viewpoint of its effect on pain processing. It is aimed both at the basic scientist and health care providers, in order to clarify the major role that substances released by immune cells play in pain modulation.
