The length of the dam crest is about 480 feet. The ends of the embankment tie into the remains of an older and higher dam which failed, due to overtopping, in 1862. A cutoff trench was excavated to solid rock and a grout curtain was installed. The spillway, which has a crest elevation of 14.5 feet below the top of the dam is located in the west (left) abutment, and is separated by a high ridge from the embankment. The 50 feet wide spillway was excavated in rock and is unlined. An intake tower is located at the toe of the upstream slope. The intake is controlled by two 18-inch gate valves and the discharge is through a concrete conduit beyond the downstream toe of the dam.

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: The purpose of this inspection is to determine if the dam constitutes a hazard to human life and property. The dam's location is Tremont Township, Schuylkill County U.S.G.S. Quadrangle - Pine Grove, Pa. The general appearance of the Black Creek Intake Dam is good, except that a heavy growth of trees covers the downstream slope of the embankment (Photograph No. 2). The slopes appear to be stable. A considerable amount of water was flowing through rocks just beyond the downstream toe near the spillway. This water appears to originate from the spillway channel. The overall visual evaluation of these facilities indicates that the dam is in good condition. The trees on the embankment should be removed. The observed steady seepage beyond the toe of the embankment adjacent to the spillway outlet channel has been a long standing condition (40 + or - years) and is not considered to be a serious problem at this time.

Book excerpt: Park Place Dam No. 3 is a 32 foot high earthen embankment across an unnamed tributary of North Mahanoy Creek. The 685 foot long embankment impounds a 109 acre-foot reservoir within an 0.27 square mile drainage basin. Since very limited data exists regarding the physical features of the dam and appurtenances, most of the features described herein were obtained from field measurements taken during this inspection. The downstream slope was measured to be 2H:1V and is covered with riprap and occasional vegetation, as shown on Photographs 7 and 8. The average upstream slope could not be measured as the very small freeboard between the crest of the dam and the normal pool prevented a reliable measurement. The upstream portion of the slope between the crest and to at least several feet below the waterline was covered with riprap. The crest width ranges from 10 to 12 feet and averages approximately 11 feet wide. A 10-inch water supply pipe is located at the base of the embankment. An eight-inch water supply line branches off in the downstream valve house at the downstream toe. Discharge through both lines is controlled by valves located in the valve house.

Book excerpt: The assessment of Brown's Lake Dam is based upon visual observations made at the time of inspection, hydraulic and hydrologic computations and past operational performance. The inspection and review of data of the Brown's Lake Dam did not reveal any problems which would require emergency action. The dam appears to be in good condition and adequately maintained. Brown's Lake Dam is a high hazard-small size dam. The spillway design flood (SDF) for a dam of this size and classification is 1/2 PMF to the PMF. the PMF has been selected as the spillway design flood based on downstream potential for loss of life. The spillway and reservoir are capable of controlling the spillway design flood (PMF). Based on criteria established by the Corps of Engineers, the spillway is termed adequate. The following recommendations and remedial measures should be instituted as soon as possible. (1) A regularly scheduled maintenance program should be developed and implemented. (2) A warning system should be developed to warn downstream residents of large spillway discharges or imminent failure of the dam. (3) A safety inspection program should be implemented with inspections at regular intervals by qualified personnel. (4) The drainline valve should be operated on a regular basis.

Book excerpt: The condition of Bel-Air Lake Dam is considered to be unsafe/ nonemergency due to a seriously inadequate spillway capacity. The overall condition of the dam is considered to be fair. Although at this time no signs of significant distress were observed, it appears that the center of the dam has significantly settled. The design drawings show the design freeboard to be five feet, but the available freeboard is only approximately two feet. It also appears that fill has been placed on the downstream slope, increasing the crest width from the design value of 10 feet to about 30 feet near the center line of the dam and greater than 50 feet near the left abutment. The dam has no functional low level outlet facility.

The recommended SDF for this structure is one-half the PMF. The spillway capacity is adequate for passing only 10 percent of the PMF peak inflow without overtopping the dam. The spillway is considered to be seriously inadequate. The dam is therefore unsafe, non-emergency.

The upstream side of the dam is protected by a small amount of riprap and the downstream face is covered with grass. The flood discharge facilities for the dam consist of a concrete open-channel spillway located near the left abutment. The outlet facilities consist of a 12-inch-diamenter corrugated metal pipe extending through the embankment along the original streambed. Based on the evaluation of existing conditions, the condition of Lake Sophia Dam is considered to be unsafe/nonemergency due to seriously inadequate spillway capacity. The flood discharge capacity was evaluated according to to the recommended procedure and was found to pass approximately 15 percent of the PMF without overtopping the dam. Results of the breach analysis indicate that downstream damage would be significantly increased due to a dam failure.

Book excerpt: Pole Run Dam No. 4 is a 30 foot high earthen embankment across Pole Run, a tributary of North Mahanoy Creek. The 780 foot long dam impounds a nine acre reservoir within an 0.58 square mile drainage basin. Very limited data exists regarding the physical features of the dam. The average measured downstream slope is 2H:1V, and the slope is covered with stone and vegetation. Since the average freeboard of the structure is less than one foot at normal pool level, the upstream slope could not be measured. A portion of the upstream slope is covered with stone, and this stone could be seen to extend at least two feet below the water and it is probable that it extends several feet below that elevation. The crest width ranges from 15 feet to 27 feet. The spillway is located at the right abutment, and consists of an open concrete channel with concrete retaining walls. The water supply intakes could not be inspected as they are below water and the pipes are buried in the embankment. The outlet pipes were inspected and three pipes were found. Near the toe of the dam, valves were located which are presumably used to control flow through the pipes.

The condition of Lake Walter Dam is considered to be good. The only significant condition noted which requires further attention is the lack of erosion protection in the emergency spillway discharge channel. The swampy condition at the toe of the dam is not considered to be significant at this time; however, the condition should be monitored to document if seepage is developing.

Although some deficiencies were observed such as seepage from the toe and lack of upstream erosion protection, none were considered to be significant relative to the overall stability of the dam at this time. Periodic inspection and evaluation of the seepage conditions are recommended. It was found that the dam has no now level outlet facilities. The flood discharge capacity of the dam was evaluated according to the recommended criteria and the spillway was found to pass 10 percent of the PMF without overtopping the low spot of the embankment. This capacity is less than the recommended spillway capacity of 50 percent of the PMF which was based on the size and downstream hazard classification of the dam. Therefore, the spillway capacity is classified to be inadequate.

The embankment is 490 feet long and 23 feet high at the maximum section. The spillway is an irregular excavated channel at the right abutment of the dam. An outlet works with a 24-inch diameter cast-iron pipe and a valve at the downstream toe is located near the middle of the embankment. A dike is located about 500 feet right of the embankment. It is 284 feet long and 3 feet high at maximum section. The dam is located on Rattlesnake Creek approximately 3.4 miles west of Moscow, Pennsylvania.
