In accordance with the Corps of Engineers' evaluation guidelines, the size classification of this dam is small and the hazard classification is significant. These classifications indicate that the Spillway Design Flood (SDF) should be in the range of the 100 year flood to one-half the Probable Maximum Flood (PMF). The recommended SDF for this structure is the 100 year flood. The spillway capacity is sufficient for passing the SDF peak inflow without overtopping the dam. The spillway, therefore, is considered to be adequate. Recommendations are presented for immediate action by the owner.

Some recommendations presented for immediate action by the owner are: (1) that measures shall be taken to provide an adequate spillway capacity, which shall include the raising of the embankment profile uniformly to at least its design crest elevation; (2) that the downstream toe and an area 20 feet beyond the toe be cleared on a regular basis of all brush, weeds and trees, permitting close observation of the seepage; and (3) that the slab joints in the spillway channel be filled with a joint material.

Quadrangle - Newport, Pa. The general appearance of Negley Dam is good. The embankment appears to be well maintained, except for a small area at the downstream toe around the principal spillway pipe outlet. The principal spillway is a 34-inch drop inlet structure with an 18-inch outlet pipe. The emergency spillway, located in the left abutment, has a well defined channel.

The spillway can pass 29 percent of the Probable Maximum Flood (PMF) without overtopping of the dam. Failure of the dam would cause an increase hazard to loss of life downstream. As a whole, the dam is judged to be in fair condition. The spillway gravity weir does not have any significant deviations from the guidelines for stability, since the toe pressure is well below the allowable. There is no evidence of instability on the embankment.

The main spillway is a drop spillway located near the right abutment of the dam. It consists of a concrete riser connected to a 30-inch diameter reinforced concrete pipe under the embankment. The auxiliary spillway is an excavated channel along the left abutment of the dam. It is a grass-linked excavation in earth. Based on visual inspection, available records, calculations and past operational performance, PA-467 Dam is judged to be in good condition. The existing spillway can pass the Probable Maximum Flood(PMF) without overtopping of the dam. The spillway capacity is rated as adequate. There is a slope stability analysis for the embankment, and it indicates that the embankment has adequate factors of safety. There is no evidence of significant problems threatening the embankment.

Book excerpt: Middle Creek Dam is an earthfill, sheetpile and concrete structure with a total length of about 500 feet. The height of the dam is 18 feet above the streambed with the maximum height of embankment being 10 feet above natural ground. The center half of the structure is a 250 foot long spillway and control structure constructed of two parallel lines of sheetpiling retaining a zoned fill and is capped with concrete. The 15 feet wide control structure is accessible from the left abutment by a walkway. The structure contains a minimum flow sluice gate and a 18-inch by 36-inch drawdown sluice gate. The downstream area, below the vertical drop weir, is protected with an 18-inch thick concrete slab. The spillway is connected with concrete cutoff walls on top of sheetpiles to the earth embankment at the abutments. The top of the dam is 4.0 feet above weir crest.

The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the PMF (Probable Maximum Flood). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 45 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Calculations also indicate that if the spillway were constructed in accordance with available desing drawings, the facility could pass and/or store approximately 63 percent of the PMF.

Quadrangle - Mt. Holly Springs, Pa. The general appearance of Upper Mt. Holly Dam is poor. The earth embankment is overgrown with brush and trees and the slopes are uneven. The riprap at the downstream side of the spillway has several voids and several trees are growing in this area. The reservoir is silted for most of its storage area at normal pool elevation. The drawdown sluice gate leaks and is inoperable. The overall evaluation of these facilities indicates that Upper Mt. Holly Dam is in poor condition. The growth of brush and trees on the embankment and in the downstream area of the spillway should be removed. After this clearing, the embankment should be inspected for seepage and signs of instability. The embankment should be made level and should be protected against erosion with a good grass mat. The riprap at the downstream side of the spillway should be repaired.
